If you are facing any identification issue, like the ones above, or are providing assistance to those who are seeking proper government-issued identification, then this guide is for you. This guide is full of information about different types of identification and how to obtain them. Introduction This Do It Yourself Guide to Identification was developed to assist individuals and organizations with navigating the complicated processes and systems associated with accessing formal government identification. As identification is delivered and offered through various systems of government, be it provincial or federal, the processes for accessing ID are not always clear, and are often changing. The information contained in this guide was as current as possible at the time of release. This includes things like what ID is good for, why you need certain identification pieces, and how best to keep your identification documents safe. Section II explores and breaks down the requirements and procedures for birth certificate applications for each province and territory. And finally, Section III explores all other identification documents and Cards of Entitlements such as Alberta Health Care Cards, Photo Identification, Canadian Passports, Social Insurance Cards and Aboriginal Status Cards. 5 The Three Types of Identification Generally speaking, when it comes to identification documents, there can be three types of requirements, or classifications of documents. The three classifications are: 1. Proof of Identity examples A piece of identification that contains your photo, legal name and birth date. This may include some of the following examples of pieces of identification (this is not an exhaustive list): Alberta Identification: Operator s licence or Identification Card Photo driver s licence from another province or jurisdiction Valid Canadian Passport Indian Status Card Valid foreign passport with appropriate Immigration documentation Permanent Resident Card Nexus Card 2. 6 Identification Why Do I Need Identification? Identification is your tool to accessing services, resources and opportunities within your community. Different types of identification may operate as cards of entitlement and ensure service delivery. Other types of identification, such as a Social Insurance Number (SIN) card, are a legal requirement for employment in Canada. In fact, Identification is required for nearly everything in society. If you need to access a food bank, you need to have identification; if you need to see your doctor, you need to have a health care card; if you want to vote, you need proof of residency. Not having identification is a significant barrier that prevents a person from accessing a wide range of community and government services. For example, renting a home, securing a job, opening a bank account, accessing a lowincome bus pass, receiving your child tax credit, or receiving income supports such as AISH and Alberta Works all require identification. ID is your tool to accessing the services you need and pursuing the opportunities you want in support of greater wellbeing for you and your family. Paying for Identification Documents Many ID documents require payment. There are several ways you can pay for an identification document, including by cash, debit, cheque, money order, or credit cards like Visa, MasterCard or American Express. Payment restrictions may apply at some Alberta Registry Agents for certain services. Contact the registry of your choice and confirm your payment options. At many grocery stores, pharmacies and Canada Post offices you can purchase a pre-loaded credit card. For example, if you want to purchase a birth certificate from New Brunswick, you can use a $25.00 pre-paid credit card and purchase your birth certificate on-line from a public computer at your local library. If you are have trouble paying for an identification document, contact 211 to be connected with identification assistance programs in your area that may be able to cover the cost of your identification document. However, if you feel you can afford an identification document, always choose a method of payment you are comfortable with. 4 United Way of the Alberta Capital Region, November 2012

7 How to Store Identification Documents Unfortunately identification fraud is a reality that we need to protect ourselves against. It is important to keep your identification documents in a safe and private place. Examples of places to store your documents might include in a locked drawer in your office or a file folder in your home. It is best not to carry certain documents with you on a daily basis. Your Social Insurance Card or Birth Certificate should not be carried with you in your wallet or purse unless you need the document for a specific purpose. Generally, it is recommended that you never carry original documents with you, except for Alberta Photo Identification or Driver s Licence. If you have no safe space for your identification, contact 211 and enquire about agencies/programs that will legally store your identification for you. If an identification storage solution is not available in your community, explore your options and the safest place to store your identification, which may include consider leaving it with your social worker, probation officer, relative or friend. In addition to securely storing your original documents, you can always register for a free electronic virtual safe deposit box with Alberta Virtual Vault. You can apply for a free 50MB starter virtual vault, or pay $14.00 $50.00 for 500MB of virtual space to store your copies of your identification documents. All you need is a valid address and you can sign-up on line or with any Alberta Registry Agent in your community. You control access with password-protection and powerful encryption. Backup, store, retrieve and access all your files anytime, anywhere you have an internet connection. And your files are safe because only the proper password can open them. If your originals are lost or misplaced, the electronic copies will help you get your ID replaced. It is also important to ensure your ID document remains valid and useable by ensuring that it remains in good condition. For example, you should not fold or laminate your birth certificate. 8 Certificates BIRTH 6 Birth Certificates This section of the guide outlines the requirements associated with applying for a Birth Certificate from all Canadian provinces and territories. There are two types of birth certificates you can order. A regular or short format birth certificate which includes your personal information, or a long format birth certificate which includes your personal information and your parental information. A long format birth certificate is important when ordering a birth certificate for your child, to register for Secure Certificate of Indian Status for the first time, or for income tax purposes to prove you are a parent. In addition, some provinces allow you to pay an additional price for fast delivery, usually called priority or express service. 22 Driver s PHOTO ID Licence Alberta Photo ID or Driver s Licence If you have previously held an Alberta Operator s or Driver s Licence or Photo Identification Card it is easier to obtain ID again. If your identification card expired more than six months ago, it is treated as a new application and you must provide proof of identity, proof of entitlement to be in Canada, and proof of Alberta residency. If you have outstanding fines on a driver s license, unpaid municipal or low-end criminal fines you can re-class your Operator s Licence to a Photo Identification Card and deal with the fines at a later date. Resistry Agents must lift the fine restriction in order to re-issue or re-class an ID document. You must remain at the Registry office and it can take a significant amount of time. You can request a Photo Identification Card that is valid for up to five years. If your Photo Identification Card is lost it will cost $22.45 for a replacement card. However, you are able to hold a Photo Identification Card for a one year term only without a payment arrangement. In order to re-class your identification card you must contact the Maintenance Enforcement Program (MEP). If a person has Maintenance Enforcement Arrears and Restrictions at a Registry, they can call the Client Services Centre of the Maintenance Enforcement Program (MEP) between the hours of 0745 to 1100 and 1200 to 1600 Monday, Tuesday, Thursday and Friday and on Wednesdays. A person with MEP Restrictions would need to provide their Maintenance Enforcement Program file number and PIN to contact the Program. THE MEP will give a partial restriction, usually within 48 hours. If you have been in touch with the MEP to receive a partial restriction and when you arrive at the Registry there is still an issue that is preventing your access, you, or the Registry agent can call back to the MEP to clear up the problem. * fees subject to change 20 United Way of the Alberta Capital Region, November 2012

23 Alberta Health In order to receive hospital and physician care in Canada you need provincial health care coverage. Alberta Health Cards entitle you to such coverage. Accordingly, Alberta Health Cards are cards of entitlement not identification cards. Provincial Health Care in Alberta is also known as the Alberta Health Care Insurance Plan (AHCIP) or Alberta Health Care Registrations. AHCIP is available to all eligible residents and their dependents. If you are new to Alberta, or a returning Alberta resident you must apply for AHCIP. If you have moved from any Canadian province or territory to Alberta, your AHCIP will become effective on the first day of the third month that you are a full-time resident of Alberta. For example, if you arrive in Calgary on January 5 th your AHCIP will become effective April 1 st. To register for AHCIP you need to complete an application form. Application forms can be found on-line at pdf or with participating Alberta Registry Agents. You must complete the form in full and provide the required documentation with your application, which includes proof of identity, proof of entitlement to be in Canada and proof of Alberta residency. The completed application form and originals of supporting documentation can be taken in person to one of the participating Alberta Registry agents. Applications can also be made by fax, or sent in the mail with photocopies of your supporting documentation. To be eligible for Alberta Health: You must be legally entitled to be or remain in Canada and make your permanent home in Alberta. You must be physically present in Alberta for at least 183 days in a 12 month period. You must not claim residency or be obtaining benefits in another province, territory, or country Non-eligible residents may include: Members of the Armed Forces Royal Canadian Mounted Police Federal penitentiary inmates Certain immigrants with non-permanent status covered by the Interim Federal Health Program You are not covered under AHCIP until all required information is received. However, any medical services you receive and pay for before your application has been successfully processed may be reimbursed once you have received your personal health card, and can prove eligibility for the period your received the services. 25 Social Insurance Card The Social Insurance Number (SIN) is a nine-digit number that you need to work in Canada or to have access to government programs and benefits. Each SIN is issued to one person only as a card. You are responsible for protecting your SIN and it is recommended to store your SIN card in a safe place and not keep it on you. To apply for a SIN card, amend your personal information or replace a lost or stolen SIN card, you can apply in person to any Service Canada Centre or by mail. You MUST provide an original primary document to apply for a SIN, replace a SIN or make changes to a SIN, such as a name change to your married name. A primary document is an official document that proves your identity and status in Canada. 28 Passport CANADIAN Requirements: Passport Photos o You are responsible to submit two photos of yourself, one will be signed by your Guarantor and the other, will appear in your passport. o The photo must have a frame height of 70mm and a frame width of 50mm o Many photo and camera shops will take your passport photo at a small cost o There are significant photo requirements. If you choose to take the photo yourself, be sure to read fully regarding all photo requirements Declaration of Guarantor o A Guarantor is a person other than yourself who has known you for at least two years and can confirm your identity. A Guarantor must hold a five-year Canadian passport that is valid or has been expired for less than one year. A guarantor must be at least 16 years of age. A family member or person residing in your home can be your Guarantor provided they meet the above requirements. o A Guarantor must complete Section 2 of the application: Declaration of Guarantor, sign the back of one of your two passport photos, and if needed sign a copy of your documents to support your identity. o If you do not have a Guarantor who has known you for at least two years, you need to complete at Statutory Declaration in Lieu of Guarantor Form available at any Passport Canada Office. 30 Indian SECURE CERTIFICATE Status OF Secure Certificate of Indian Status An Individual recognized by the federal government as being registered under the Indian Act is referred to as a Registered Indian or Status Indian. Any individual who is registered, as an Indian is eligible for a status card, regardless of age. Obtaining Indian Status and having an Indian Status Card ensures that you are able to receive the range of programs and services specifically offered by federal and provincial governments to those individuals considered Registered or Status Indians. Status cards are an identity document delivered by Aboriginal Affairs and Northern Development Canada (AANDC). The Certificate of Indian Status is undergoing an upgrade to improve security features, protect against identity theft and to meet the changing requirements at the border with the United States. The Secure Certificate of Indian Status (SCIS) will be valid for 10 years for adults, and valid for three years for children under 15 years old. You are not yet required to change your old Certificate of Indian Status into the new Secure Certificate of Indian Status (SCIS). However, in the future it will become mandatory to obtain a SCIS in order to receive benefits and cross into the United States. Persons with status are strongly encouraged to make an appointment to secure the new SCIS card. 33 Temporary Community Identification Document Sometimes called an Affidavit for Identification or a Statutory Declaration for Identification this community identification document is a temporary identification piece that can be extremely helpful accessing services in the community. A temporary community identification document is issued by an agency, notarized or commission and is used to support your identity. Temporary community identification documents are useful when applying for social benefits, applying for Alberta Health, accessing food services, or securing a transit pass. As community identification documents are not government issued they are not always accepted at places like banks or registries but they can be extremely helpful when you have no identification and need a document in the short term until you are able to secure your formal government issued identification. The only requirement is the correct spelling of your name, date of birth and a current photograph. See the list of agencies that can assist you for more details on Community Issued Identification Documents.
