2 CITIZENS AND INSTITUTIONS NOTE: REGISTRATION OF EVERY BIRTH/STILL BIRTH AND DEATH IS MANDATORY UNDER THE REGISTRATION OF BIRTHS AND DEATHS ACT, 1969 AND KARNATAKA REGISTRATION OF BIRTHS AND DEATHS RULES A. Why register births & deaths? Birth Certificate is necessary for personal identification and protection of the persons concerned in the following ways: i. TO PROVE THE FACT OF BIRTH FOR - Parentage and family relationship, settlement of inheritance and property right etc. ii. TO ESTABLISH THE DATE OF BIRTH FOR - Admission to schools, admission to Government/Semi Government/Private Services, recruitment to Army, right to vote, taking an insurance policy, obtaining driving licenses, other purposes where maximum or minimum age limits are prescribed, claiming right to marry after attaining marriageable age etc. iii. TO PROVE THE PLACE OF BIRTH FOR Right to vote, domicile, passport etc. Death certificate is necessary to prove the fact about death in the following ways; i. To prove date of death. ii. To establish the fact of death for relieving the individual from social, legal & official obligations. iii. To establish the claim on property, insurance and social security benefits etc. iv. Every birth / still-birth and death is to be reported and registered within 21 days at the place of its occurrence in the prescribed reporting forms. B. In-time registration of births in residences (births, still births & deaths). Table-1 Sl. Obtaining birth/still birth/death certificates 1. If the birth/still birth/death occurred in a city hospital, first check if that hospital is a designated hospital that holds the event records. (Table-8 and Table-9) 2. If not, check with the nearest BMP sub-health office (Table-7) if they hold the records for the event date of your interest. BMP Publication 103 has addresses of facilities including designated hospital and SHOs. 3. If the SHO also does not hold the specific records then the BMP Birth & Deaths Central Office, Statitstical Section at 10th Floor, Public Utility Building, M G Road, Bangalore will be in possession of your records. 4. Fill the application form (English or Kannada) supplied by Bangalore Mahanagara Palike. Form 110 is the birth certificate application form. Form 111 is the death certificate application form. Instruction sheets are available for these forms. i. File the form and pay the prescribed fees at office that holds the records for the event of your interest. ii. If your birth/still-birth/death event is part of the manually entered record system, you will pay fees as per table 3a. If the event is in the computerized records, you will need to pay fees as per table 3b. 2

Delayed registrations of residence events. Delayed registrations are those that are being registered after the 21-day period has passed. TABLE 4 S. Correctecting Birth and Death events? If your birth or death events has been reported incorrectly (name of parents, date, etc), you may use this table for information on the correction process.
