1 APPLICATION FOR 2018 FOOD TRUCK GRAPHIC DESIGN WORK (Urban Artist Florida Studio, LLC, is hereafter known as UAFS, LLC) Thank you for your interest in this event. What hours do you serve and to what clientele? (For example, fairs and festivals only, nightlife locations, weekend sports park events, etc. as applicable.) 1

2 Do you operate your truck in the local area or do you travel around the state or even between states? (This is used to further assess whom your clients are for the best design possible to be created.) Why did you become a food truck operator? What is your dream about? Describe your passion. How did you arrive at the name of your truck? What is the meaning? What kind of graphic work are you seeking? (Not all operators are in need of or can afford a full graphics print package. My goal is to provide you with the graphics you can afford to have printed for your business. Note: this graphic design work is for your business. It is not transferrable to new owners. If a new or additional truck is purchased, the graphics ARE applicable to these new assets. There is no pressure to request work that is not needed. If the goal is to have a really nice menu done and that is it, then let s work hard on this together.) Does your company have a tagline that you would like to integrate in your design? 3 Does your business maintain a website or any number of social media outlets? If so, which ones? (The graphics will be most effective applied across these platforms for consistency. I will provide different file formats for use as needed.) Why do you want the graphics work done? What, if anything, needs to be changed with your current set-up? How do you hope new graphics will positively impact your business? Stand outside your truck service window, a few feet back as if you are a customer outside waiting to order. What do you see? What do you want your customers to see? What would you like them thinking about while they wait? 3

4 Describe your ideal customer. Why would your ideal customer choose your truck? How would a new graphic design improve the value of your food truck? What do you want people to remember after eating food from your business? Why should your business be chosen to have graphics created? 4

5 Have you read through and answered all of the above questions? Do you understand all of the rules and notices posted on both the website and on this form? Circle choice: YES INITIAL NO INITIAL NOTICES: * Neither Deb Von Cannon, nor UAFS, LLC, are covering the cost of any printing, paint, etc. My offering is the creation of the design files only that the operator will take to a print shop and have printed at their own expense. ALL printing and final work for the business is at the expense of the Food Truck business and its operator(s). The final design files will be provided for you. The original art files will remain property of Deb Von Cannon and UAFS, LLC and will not be sold but may appear in my portfolio. * This offer is to help promote a food truck operator s business by promoting or improving the food sales and curb appeal. It is NOT for someone who is trying to build up a truck to sell to someone else. It is NOT for a food truck manufacturing company or a resale company. The graphic work and design ideas are not transferrable. * Both UAFS, LLC and myself, Deb Von Cannon, maintain the right to terminate the project at any time in which it becomes necessary: 1) The winner cannot or is unwilling to meet, 2) The winner does not have an idea of what they want, 3) The operator and I cannot find a mutually agreeable time and place to meet, 4) Both parties cannot agree on many points of the project or 5) Other difficulties arise that do not allow for the completion of the project. This includes failure to meet and abide by the requirements of this application or the inability to agree upon a contract by which to begin the project. * It is intended that one winner and two runners-up will be chosen by May 31st and a 6-month window (through November 1st, 2018) will be given to meet and work through completion of the project. If the project is completed sooner, that s fine. This project is not intended to be rushed but must be completed in a timely fashion. Should it be necessary, additional time will be provided to complete the work. Both parties will agree to and sign an amended contract as necessary. Additionally, should circumstances not work with the winner, the design graphics will be rewarded to the runners-up, in succession. 6 Memo of Understanding I,, understand that this is a memorial promotion to receive free graphic design work for my food truck business based on the responses provided to UAFS, LLC and that by submitting this application, there is no guarantee that my food truck will be the chosen business to receive the graphics design work described herein. Initial: I,, understand that before any formal design work is started, I will have to meet with Deb Von Cannon of UAFS, LLC on a number of occasions and am responsible for returning phone calls and s that pertaining to the gaining of information for the creation of the design work for my food truck business. Initial: I,, understand that I am applying for a memorial promotion to have graphics work designed for my mobile food truck and that this offer includes nothing more than digital files for me, the operator, to take to my chosen printer and have printed at my own expense. Initial: I,, understand that while there should be no activities expected that would cause harm to either party, I hold harmless, Deb Von Cannon and UAFS, LLC, from any injury that may result during the completion of this project. No meetings will occur onsite at UAFS, LLC. All meetings will be held at a public place of business or at my food truck location to gain truck measurements and other design related information.
