For parents/carers who answered the questionnaire on behalf of the child/young person Thank you for helping this young person to complete the questionnaire. We aim to provide safe, enjoyable and effective placements; therefore it is important that we gather the views of children and young people. Because this questionnaire is designed for children and young people who have a range of communication methods, it would be helpful if you would answer the following questions (please use additional sheets if necessary). How did you assist the young person to understand the questions? How have you been able to gather the answers to the questions? Has the young person said or done anything which gives you a clue to how they are feeling about their placement? Is there anything about the placement that you think makes the young person feel unhappy, anxious, confused, frustrated or sad? If so how might this be changed? Is there anything that the young person particularly enjoys about the placement? Signed Date 8
