CTC eforms for Records & Registration Submitting and Routing eforms Census Rolls

Login to CTC eforms To get to the eforms webpage, you can type the URL directly in the web browser search bar. Or- https://ctceforms.ctcd.edu You can click Forms on the Faculty & Staff link of the CTC Home Page and,

Login to CTC eforms then click the eforms link and, then click the Login link.

The Login Screen Whichever way you choose, it will take you to the login screen shown below. Your username is your WebAdvisor ID and initial password is your date of birth (two digit month, two digit day & last two digits of year). It is highly recommended that you change your password once you are logged in.

Retrieving & Submitting Census Rolls Go to CTC s website and click on Faculty & Staff Scroll down to Forms Click on eforms Click on eforms portal Enter your username and ID

Retrieving & Submitting Census Rolls Scroll down to ST Census_Certification Roll (left hand side) Click on Certification Roll Select term-i.e. Spring 2016 Select location- Central Campus **not Distance Learning** Select course name/number

Retrieving & Submitting Census Rolls Select section number Click on Pull Students. Will include cross-listed students (Go Army Ed). Co-teaching-Census roll will only appear in primary instructor s e-forms account. Student enrolled in your course will appear. Submit day after Census Date, not prior to or on census date If the student has not attended, click on the box Never Attended. Type current date: MM/DD/YYYY and full name

Retrieving & Submitting Census Rolls Click on Destination arrow Dialog box opens with different sites. Select Inbox, Central Census-Cert Roll You can add comments if you like (optional) Click on the Send button. Pop up message will appear that states Are you sure you want to send this document to Inbox Central Census-Cert Roll? Click Yes

Never Attend/Last Attend If the student has not attended, click on the box Never Attended on the census roll. Submit a faculty withdrawal to the Systems Registrar s office via e-forms or enter the grade along with the last attend date or check the never attend box in WebAdvisor. Clicking the Never Attended box on your census roll does not result in the student being withdrawn from the course. You must post a grade via WebAdvisor with the last attend date or check the never attend box in order for the student to be dropped from the course.

Never Attend/Last Attend Faculty may also submit the e-form faculty withdrawal in lieu of posting the grade in WebAdvisor. The completed form should be sent to Inbox, Systems Registrar. Please do not submit both an e-form faculty withdrawal and post the grade along with the last attend date or never attend box checked in WebAdvisor. The eform faculty withdrawal form must be used if the student is being withdrawn due to disciplinary action or scholastic dishonesty.

Retrieving & Submitting Census Rolls All of the census rolls you send will appear in your Report folder. If you click on the multi-colored circle icon in the Letter Library, this takes you to your reports. If you click on My Form Activity you will see the census rolls that have been sent. Census rolls are due 5 business days after the census date. Census date is listed on the census roll (top right hand side).

Approved/Denied Census Rolls Instructors who receive a denied/rejected census roll in their e-form account in-box will need to submit a new census roll. Instructors cannot re-submit denied census roll located in their in-box. Approved census rolls require no further action by the instructor. Approval e-mail will be sent to the instructor s CTC e-mail account. Re-submission: If an instructor realizes an error was made on the submitted census roll, please contact points of contacts listed on slide 15 via e-mail or phone call.

Reports The following 3 reports types are available for E-Form: My Action Summary-Provides chart with current day processing volume to include last 12 months My Activity Detail-Provides total activity detail for specific start and end dates My Form Activity-Provides break down volume of specific forms submitted and processed. Left Click on form type to view form submitted or status.
