Structure Information Worksheet Use this form to provide LWS engineers information regarding your structure and unique lift requirements. LWS can provide a complete computer analysis on your structure(s) and will recommend & supply complete material kits that will: raise, raise and reinforce the ground line, or raise, reinforce and reclassify the poles for each structure. LWS designers may recommend additional X-braces or foundation systems with the material kits. Complete the following form and fax it to LWS at: 402-643-4374 - OR fill out ONLINE at www.lwsinc.com - OR submit simple spreadsheet containing quantity and size required from charts on pages 3 & 5 Customer Information Customer Name: Contact Name: Project Name: Project Address: City / State: Phone: Fax: Email: Constr Start Date: Delivery Location: City / State / Zip Structure Type No. of Vee Braces 0 2 4 Send detailed drawing if available Pole Information Pole Height(s): Pole Class(es): Species: Douglas-fir Southern Pine Cedar Pole Condition at Groundline: Single Pole H-Frame Inspection Report Available? Materials, Equipment & Manpower Required for Typical Installations Materials Equipment Safety Cap Steel PhaseRaiser Units 7/8 Anti-split Bolts 7/8 Cross Bolts 7/8 Threaded Rods Covers Cover Band The PhaseRaiser system requires specialized training, tools and equipment for installation. This equipment is available for purchase or lease and includes on-site training by LWS field personnel as well as the following specialized tools and equipment: Custom Equipment Trailer w/ Tool Boxes - Skid Mount Hydraulic Lifting Unit - Hydraulic Hoses - 4 x 101 Hydraulic Cylinders Cylinder Extensions Galvanized Lifting Vangs Cylinder Pins Treated Timbers Nylon Ratchet Straps Banding Dispenser Banding Shear Drilling Alignment Tools 4 Working Ladders In addition to the equipment trailer, a line or boom truck, chain saw and air compressor are also needed. PhaseRaiser units are made of high strength galvanized steel. All bolts conform to ANSI C135.1 The PhaseRaiser units are also available in painted and weathering steel. Personnel For more detailed information regarding installation, see the PhaseRaiser Installation Manual. The PhaseRaiser system enables a crew as small as three for single pole or a crew of five (or six) for H-frame structures to cut the existing poles, install custom designed galvanized or painted steel PhaseRaiser units, then raise and secure the structure to the desired height up to an additional twenty feet. LWS Field Training - A skilled field trainer from LWS works with qualified lineworkers until they develop a comfortable skill level - normally after raising two to three structures. The strength and patented design of PhaseRaiser has been proven in full-scale testing. In 1997, LWS, Inc. conducted full scale structure tests on both single pole and H-frame PhaseRaiser installations. The tests proved that the strength and design of the PhaseRaiser lifting system exceeded NESC load requirements. 345kV Double X-braced Structure Raised 5 feet Maximum Transverse Load 24,000 Lbs Based on the following design parameters: Shield Wire...3/8 steel strand Conductor...795 ACSR 26/7 (2 per phase) NESC Medium Loading with steel overload capacity factors. 5 Lift Maximum Span...2,346 feet Maximum Groundline Moment...194,400 ft-lbs PhaseRaiser Rated Capacity...220,000 ft-lbs X-Braced Load...24,019 Lbs. 50ft Class 3 Pole Raised 8 feet Pole Failure... 3,500 pounds Loaded... 48-0 AGL Groundline Moment Failure... 170,400 ft-lbs No permanent Deformation Page 11 U.S. PATENTS 6,115,988 6,151,860 CANADIAN PATENTS 2,252,115 2,297,318
