The independent contractor HSS refers to you is responsiblefor the performance, fulfillment and guarantee of any job or contract between the contractor and the customer. HSS provides administrative and processing services only. Complete warranty terms and conditions are available upon request. HSS' WARRANTY DOES NOT COVER MOISTURE RELATED DAMAGES. In the event the contractor performs work that is shown to have been inconsistent with industry standards, the contractor is expected to correct the work at no additional cost pursuant to its warranty. The HSS Limited Warranty requires that ALL PAYMENTS ARE MADE THROUGH THE HOME SERVICE STORE, INC. unless otherwise authorized by HSS. HSS' Limited Warranty is voided if 1) The customer supplies materials; 2) The customer has alterations or adjustments made by a Contractor other than the independent contractor; or 3) the customer does not sign an authorized estimate or work order form; or 4) Payment is made directly to the independent contractor. B. Final payment is due at the conclusion of the installation prior to the installation crew leaving your property. The installation crew will do a final walk through with you of the completed installation. After you do your walk through you will then need to call at the conclusion of the installation while the installation crew is still at your property and make the final payment. After you make your final payment you will receive a confirmation code. Please provide this code to the installation crew as proof of payment. In some instances there might be some incorrect or missing trim or minor items that will require the installation crew to come back at a later date when those materials arrive in order to finish this minor work. Final payment cannot be held up for minor completion work. C. Although we will give you an approximate time frame to complete your installation we cannot promise a guaranteed completion time. Please understand this when scheduling and allow for this in your installation process. It is important that we take the required time to complete your installation process satisfactorily. D. The customer has been notified that the manufacturer of your new flooring requires a flat surface for the new floor to be installed upon. This estimate does not include any charges that may be required to fix your sub-floor if it is outside of the manufacturer's warranty requirements. Additional charges could include labor, supplies, tool rental, etc. to get your sub-floor fixed to meet the manufacturer's requirements. We would like to check your sub-floor prior to you finalizing your purchase but this will require all of your existing flooring to be removed. If you are able to have your existing flooring removed so we can check your sub-floor please contact our office for a free site inspection by one of our certified installers. If you are not able to have your existing flooring removed prior to the installation please be aware that additional costs and time could be involved to fix your sub-floor. Your estimate is based upon exact measurements plus 10% waste for a standard installation. This could result in additional materials left over at the conclusion of your installation. There is no refund for labor for any leftover materials. Also, transition moldings are estimated based upon certain conditions at the time of your measure. Sometimes due to the height of your adjoining floor, leveling issues or height of your new flooring, different transitions could be required and customer is aware that these may need to be purchased at an additional cost. 7 Please keep in mind that the manufacturer of your flooring requires us to allow for a perimeter expansion gap equal to the thickness of your new flooring. As a result, this gap will need to be covered up with either wall base or 1/4 round that is larger than this gap. In the event your flooring is thicker than the baseboards (as in the case of 3/4in solid hardwood) you will be required to have both baseboards and 1/4 round installed which will result in additional materials and labor. Please discuss this with our office before finalizing your purchase so we can revise your estimate accordingly. E. The manufacturer of your flooring requires that if you have a crawl space under your home, you will need to install black plastic visqueen over the ground below your home. This is a service that must be completed by you or an outside handyman as this is not a service we provide. This is a requirement of your warranty and your signature below acknowledges that you understand this must be completed. F. According to your estimate you will need to have wall base or quarter round installed. Please keep in mind that if you are purchasing a product that requires painting that you will have to complete this service yourself. SW Contractors does not providing any painting of baseboards or quarter round. We would advise you to pre-paint the baseboards or quarter round prior to the installation and SW Contractors will install them and then caulk the nail holes. You will have minor sand and paint touch up after the baseboards or quarter rounds are installed. This will involve you sanding down the caulking after it has dried and then painting over the nail holes. If you don't want to pre paint the base you can choose to paint them after they have been installed. G. Flooring products need to be in your home (not your garage) prior to installation for proper acclimation for a minimum of 72 hours for laminate and engineered flooring and 5-7 days for solid hardwood. The environment of your home must be controlled to applicable industry recommended temperatures of between degrees and relative humidity standards of 30-50% for at least 7 days prior to installation. H. Your home has existing vinyl flooring that needs to be removed. In the event you choose to do this service yourself, please make sure to remove the entire vinyl completely. If the sub-floor in this area is plywood you probably have particleboard or underlayment that is under the vinyl. You will also need to remove this as well and strip the floor down to the main plywood sub-flooring. In the event we are going to be removing the vinyl for you then we will need to have a "clear" asbestos test completed prior to removing it. You can elect to have this test done yourself (typical fees for this test are under 50). In the event you would like us to do this test for you there is a 125 charge for us to perform this service. Please advise our staff how you would like to handle this. I. In the event you have paid for furniture moving, please keep in mind that we only move basic items such as tables, couches, chairs, beds, etc. We do not move any personal items, electronics, breakables, antiques, knick knacks, clothing, pianos, pool tables, water beds, aquariums, safes, exercise equipment, etc. We do not disassemble any items so please do this prior to your installer arriving. Please remove all bedding and please remove all clothing from the lower level of closets. Any removal of oversized or excessive furniture will result in an additional charge. J. We can disconnect gas and water lines but for safety and liability you (or a licensed plumber or PGE) will be required to reconnect the gas or water line after the installation. In most cases PGE will perform a safety inspection and gas line reconnection at no charge. Any removal of appliances will result in an additional charge.

8 K. Please be advised that you are selecting a floating floor installation. These types of floors are NOT adhered to the sub floor and typically rest on a foam pad. As a result you may see movement and may hear a hollow sound when walked upon. L. If you are considering the Elastilon product, please contact our office before finalizing your purchase.
