Accommodation & Spherical Aberration Natural accommodation results in an increase in the optical power of the crystalline lens along with increase in negative spherical aberration The central region of the natural lens exhibits greater change in optical power compared to more peripheral regions Cheng, A population Study on Changes in Wave Aberrations with Accommodation. Journal of Vision April 4 (4): Vilupuru, Spatially Variant Changes in Lens Power During Ocular Accommodation in a Rhesus Monkey Eye. Journal of Vision April 22 (4):
SYNCHRONY Vu SYNCHRONY Vu dual optic IOL is designed to restore accommodation by causing a change in optical power during ciliary muscle action SYNCHRONY Vu IOL comprises a positive front optic and a negative back optic Forward movements of the front optic create an accommodative change in optical power SYNCHRONY Vu Dual Optic IOL 4
Clinical Evidence of Accommodation Ultrasound Biomicroscopy was performed on a series of patients to evaluate lens movement in response to different visual cues Objective accommodation is demonstrated by the separation of the two optics The lower chart demonstrates the separation between the two optics 5
Next Generation Synchrony Vu The current SYNCHRONY Vu IOL is designed as a monofocal IOL and results in a uniform change in optical power with accommodation It is desirable to mimic the spatially variant characteristics of natural accommodation to further improve the benefit provided by the SYNCHRONY Vu IOL A key consideration is to preserve a monofocal optical quality while improving near depth of focus 6
Study Purpose Incorporate a spherical aberration feature to the SYNCHRONY Vu IOL to improve near visual benefit while maintaining a monofocal optical quality per ISO standards A central aspheric zone with negative spherical aberration blending into a peripheral spherical optic was chosen for the following reasons: Utilizes pupil constriction during near viewing to maximize near benefit from the central aspheric zone Maintains maximum optical imaging quality under mesopic conditions Schematic of the SYNCHRONY Vu IOL with a central progressive aspheric zone. 7
Methods & Materials Zemax based optical modeling was performed to optimize the degree of asphericity, central zone diameter, and blend region for maximum near benefit and optical quality The Le Grand- El Hage schematic human eye was used SYNCHRONY lenses with appropriate aspheric profiles were manufactured and tested With interferometry to confirm surface profile In an ISO Eye Model to evaluate optical quality and depth of focus 8
Results: Description and Evaluation The central aspheric zone was modeled as a conic surface with polynomial extension to achieve central negative spherical aberration and optimal blend region Various designs were evaluated for: peak optical quality (MTF at best focus (0D) position Peak MTF Parent Design 100 cycles/mm (or) 20/20 acuity range of focus at 10% MTF limit DOF 9
Results: Evaluation of different designs Various combinations of zone diameter and peak vertex powers led to different depth of focus signatures Peak MTF Parent Design DOF Aspheric SYNCHRONY Vu IOL Designs 2.8 mm, 2.3D 0.9 mm, 3.5D 0.9 mm, 5.5D 10
Results: Mesopic Benefit The optimal design added additional depth of focus while satisfying ISO requirements for optical quality. The aspheric SYNCHRONY lens exhibited slightly better optical quality in mesopic conditions compared to the parent SYNCHRONY model 11
Conclusions The new aspheric SYNCHRONY Vu IOL has been developed to mimic the increase in negative spherical aberration during natural accommodation The new SYNCHRONY optical design can offer Improved depth of focus in the near direction Improved mesopic optical quality Monofocal imaging quality under photopic conditions This product is not available in the U.S. SYNCHRONY and SYNCHRONY Vu are trademarks owned by or licensed to Abbott Laboratories, its subsidiaries or affiliates. 12
