2 Original Sheet 1 DEFINITION OF TERMS ACCESS LINE This denotes the line between the serving central office and the customer premises used to receive or place local or inter and intra state calls. ACCESSORIES The term "Accessories" denotes devices which are mechanically attached to, or used with, the facilities and equipment furnished by the Telephone Company and which are independent of, and not electrically, acoustically, or inductively connected to the communications path of the telecommunications systems. These devices may not replace any of the component parts of the Telephone Company facilities or equipment nor be injurious to the telecommunications network. AIRLINE A measurement used to compute mileage charges on a point to point basis. An adjective used to indicate that a measurement is made as the shortest distance between two points. AUTHORIZED PROTECTIVE CONNECTING MODULE The term "Authorized Protective Connecting Module" denotes a protective unit designed by the Telephone Company and manufactured under the control of Telephone Company quality assurance procedures, which unit is to be incorporated in a Conforming Answering Device. AVERAGE BUSY SEASON - BUSY HOUR TRAFFIC The average traffic volume for the busy season, busy hour. AUTHORIZED USER A person, firm or corporation (other than the customer) on whose premise a telephone, Private Branch Exchange, or private line service or channel is located and who may communicate over such channels in accordance with the terms of this document. AVERAGE BUSY SEASON BUSY HOUR TRAFFIC The average traffic volume for the busy season, busy hour. BASE RATE AREA The developed portion or portions within each exchange service area as set forth in this document, maps or descriptions. Service within this area is furnished at uniform rates without mileage charges. BASIC EXCHANGE SERVICE Residence and Business Service Access Lines and Local Messages furnished in accordance with the regulations and charges set forth in this document. Except as otherwise noted, Residence Basic Exchange Service includes one white pages directory listing and Business Basic Exchange Service includes one white pages and one Yellow Pages directory listing. BUILDING A building or buildings in which there is free access between all parts of the structure by means of doors, open archways, elevators, stairways or continuous corridors. Heating tunnels or passageways under or over a thoroughfare or other open space do not make two separate buildings the same building. BUSINESS SERVICE Telephone service furnished to customers where the actual or obvious use is principally or substantially of a business, professional, or occupational nature.

CENTRAL OFFICE A switching unit in a telephone system which provides service to the general public, having the necessary equipment and operating arrangements for terminating and interconnecting customer lines and trunks or trunks only. There may be more than one central office in a building or exchange. CENTRAL OFFICE CONNECTING FACILITY A facility furnished to an Other Common Carrier by the Company between the terminal equipment location of the Other Common Carrier and a point of connection on the Company premises. CENTRAL OFFICE LINE A switching unit in a telephone system which provides service to the general public, having the necessary equipment and operating arrangements for terminating and interconnecting customer lines and trunks or trunks only. There may be more than one central office in a building or exchange.. CHANNEL OR CIRCUIT A path for communication between two or more stations or Telephone Company offices, furnished in such a manner as the Telephone Company may elect, whether by wire, radio or a combination thereof and whether or not by a single physical facility or route. CLASS OF SERVICE There are two classes of Basic Exchange Service, Business and Residence Service. The Class of Service is determined by the nature of the use to be made of the service and governs the type of directory listing furnished and the charges for basic and ancillary services. COMMISSION The Public Service Commission of Wisconsin COMMON BATTERY SERVICE Telecommunications service in connection with which electrical energy for talking and signaling is supplied from a central point. COMMON CARRIER A supplier that undertakes to carry goods, services, or people from one point to another for the public in general. In telecommunications, such carriage relates to the provision of transmission capabilities over the Telecommunications Network.

4 Original Sheet 3 DEFINITION OF TERMS COMMUNICATIONS SYSTEMS The term "Communications Systems" denotes channels or other facilities and equipment which are capable, when not connected to Telecommunications Service, of two-way communications between customer-provided terminal equipment or Telephone Company lines. COMPOSITE DATA SERVICE VENDOR A Customer who has been certificated by the Federal Communications Commission pursuant to Section 214 of the Communications Act of 1934, as amended, to acquire and operate facilities to perform data switching for others. A Customer shall be classified as a Composite Data Service Vendor only with respect to those services which are utilized for the provision of Composite Data Service. CONFORMING ANSWERING DEVICE The term "Conforming Answering Device" denotes a customer-provided device which automatically answers incoming calls; transmits a pre-recorded voice message or appropriate audible signal to the calling party; records a voice message from the calling party is so designed and arranged; and automatically disconnects from the line in a prearranged manner on completion of the last of the functions for which it was designed and arranged as described in this paragraph. The Conforming Answering Device may include remote interrogation and/or device function control. A Conforming Answering Device must incorporate an Authorized Protective Connecting Module and must bear a valid conformance number. CONNECTING ARRANGEMENT The term Connecting Arrangement denotes the equipment provided by the Telephone Company to accomplish the direct electrical connection of customer-provided facilities with the facilities of the Telephone Company. CONNECTING COMPANY A corporation, association, partnership or individual owning or operating one or more exchanges and with whom traffic is interchanged. CONNECTION The term "Connection" denotes the establishment of telephone service. A move of existing service to a different premise requires a connection. CONTINUOUS PROPERTY A continuous plot of ground including any buildings thereon, which is used exclusively by the customer or by an authorized user of the customer's service and which is not separated by property occupied by others or by a public thoroughfare. COST Cost, as used in this document covers the cost of labor, materials, incidentals, supervision and overhead. CUSTOMER OR SUBSCRIBER The term "Customer" denotes the person, firm or corporation responsible for the payment of charges and compliance with the regulations of the Telephone Company.

5 Original Sheet 4 DEFINITION OF TERMS CUSTOMER PREMISES EQUIPMENT (CPE) Telecommunications equipment located on the Customer s Premises which is not provided by the Company. CUSTOMER-PROVIDED TERMINAL EQUIPMENT The term "Customer-Provided Terminal Equipment" denotes devices or apparatus and their associated wiring which are provided by a customer and when connected to the communications path of the telecommunications system, are so connected either electrically, acoustically, or inductively. DATA ACCESS ARRANGEMENT The term Data Access Arrangement denotes a protective connecting arrangement for use with the network control signaling unit, or in lieu of the connecting arrangement, an arrangement to identify a central office line and protective facilities and procedures to determine compliance with the criteria set forth in other parts of this document. DATA SWITCHING As used in connection with Composite Data Service, the switching of data (non-voice) messages by the interchange, controlling and routing of data messages between two or more Stations, via Telecommunications Network facilities wherein the information content of the message remains unaltered. DEMARCATION POINT The term Demarcation Point means the point of connection, provided and maintained by the Telephone Company, at which the Telephone Company service and the property owner s or customer s facilities are connected. The Demarcation Point is to be mutually agreeable to the Telephone Company and the subscriber or property owner, and is normally located near the point where the Telephone Company facilities enter the building or property, on the subscriber s side of the Company s protector, or its equivalent. For multiple buildings constructed on continuous property, such as industrial parks and campuses, or multiple unit buildings with enclosed commons areas, such as shopping malls and condominiums, the Telephone Company will establish a single Demarcation Point or may designate one or more existing terminating connections on a property as Demarcation Points. The property owner or customer is responsible for service on the customer side of the Demarcation Point(s). Where feasible, one or more alternate Demarcation Points may be placed or reinforced by the Company at the request of the customer or property owner. Charges will be applied to cover additional costs of placing or reinforcing alternate Demarcation Points requested by the customer or property owner. DETERMINATION OF SERVING AREA A customer located on the opposite side of the thoroughfare from the facilities is considered to be within the area if he can be served by a drop wire using not more than one additional pole. A customer located on the same side of the thoroughfare as the facilities is considered to be within the area if he can be served by a drop wire not requiring any additional poles. In no case will a location beyond one hundred fifty (150) feet from the boundary be considered as within the respective Base Rate Area or Exchange Area. DIRECT ELECTRICAL CONNECTION The term "Direct Electrical Connection" denotes a physical connection of the electrical conductors in the communications path.

6 Original Sheet 5 DEFINITION OF TERMS EXCHANGE A unit established for the administration of telephone service in a specified area which usually embraces a city, town or village and its environs. EXCHANGE AREA The territory included within the boundaries of an exchange. EXCHANGE SERVICE The service of furnishing facilities for telephone communication within a local service area, in accordance with the regulations and charges specified in this document. Exchange service includes the furnishing of the local facilities required to establish and maintain connection between an exchange station and the toll plant in connection with toll calls. EXCHANGE TELEPHONE SERVICE LINE A central office line which provides access to the exchange telephone network for local and long distance telephone service and includes the serving central office equipment and all outside plant facilities through and including the Standard Network Interface. EXTENDED AREA SERVICE Interexchange telephone service furnished at flat or message rates between one or more exchange areas. EXTENDED COMMUNITY CALLING (ECC) SERVICE Interexchange local telephone service is furnished at a per minute rate within the Extended Community Calling (ECC) Area. FLAT RATE SERVICE Service furnished at a fixed monthly charge. FOREIGN EXCHANGE SERVICE Foreign exchange service is exchange service furnished from a central office of an exchange other than the exchange normally serving the area in which the customer is located. It is not available in connection with public or semi-public telephone service. The rate for foreign exchange service is the rate applicable in the foreign exchange for the class of service furnished. The foreign exchange telephone company providing the customer the telephone number, is considered the switching telephone company. GRADE OF SERVICE The type of service furnished a customer with respect to the number of main telephones which may be connected to a central office line. (1-party, 2-party, 4-party and multi-party) HEAD SET A hands free, multiwire device containing acoustic-to-electric (transmitter) and electric-to-acoustic (receiver) transducers, normally worn on the head of the user for close talking, which provides 2-way transmission of live human speech. INDIVIDUAL LINE A central office line designed for the connection of only one main station (not a private automatic branch exchange trunk line).

7 Original Sheet 6 DEFINITION OF TERMS INSTALLATION CHARGE A nonrecurring charge made for the placing or furnishing of regulated equipment, which may apply in place of or in addition to service connection and other applicable charges for regulated services. INTERFACE Denotes that point at the Demarcation Point serving the premises of the customer or user at which provision is made for connection of other than Telephone Company-provided facilities to facilities provided by the Telephone Company. INTRASTATE END USER CHARGE The flat rate monthly charge that is added to each access line for providing nontraffic sensitive plant to subscribers for the availability to make toll calls which are within the local access and transport area. (LATA) INTRA-EXCHANGE SERVICE Intra-exchange local telephone service is furnished at a flat rate within one basic exchange area. LINE HUNT SERVICE Line Hunt Service is any One-Party or Trunk service so arranged at the central office that if the number called is busy, the central office equipment selects another line in the customer's group that is idle. Special central office facilities or arrangements are required to provide line hunt service. LOCAL MESSAGE A communications between a calling telephone and any other telephone within the local service area of the calling telephone. LOCAL MESSAGE UNIT The unit of measurement for charging for local message use. A local message, unless furnished as an unlimited local message, may carry a charge of one or more local message units, depending upon destination and length of conversation. LOCAL SERVICE Telephone service furnished between customers' stations located within the same local service area. LOCAL SERVICE AREA The Exchange Area specified in this document within which customers may place local messages at Residence or Business Local Message or Extended Community Calling (ECC) Local Message charges. The Local Service Area for an Exchange consists of the basic exchange area, the Extended Area Service area and the Extended Community Calling Service area defined for the exchange. LOCALITY RATE AREA That portion of an exchange located beyond the Base Rate Area, and either contiguous or noncontiguous thereto, within which specified urban grades of exchange service are offered at rates which do not vary with the distance from the Base Rate Area, but not necessarily at identical rates to the base rate area. MESSAGE A completed customer telephone call. MESSAGE RATE SERVICE A service for which charges are based upon the number of originated messages placed by the customer to access lines within the same local or message rate calling area.

8 Original Sheet 7 DEFINITION OF TERMS MILEAGE CHARGES Rates charged for exchange and interexchange lines for various services based on distance. MINIMUM CONTRACT PERIOD The minimum length of time for which a customer is obliged to pay for service, facilities, and equipment, whether or not retained by the customer for such minimum length of time. NETWORK ACCESS LINE A Network Access Line consists of the serving Central Office line equipment, and all Company plant facilities up to and including the Company-provided Standard Network Interface. These facilities are Company-provided and maintained and provide access to and from the Telecommunications Network. NETWORK CONTROL SIGNALING The term "Network Control Signaling" denotes the transmission of signals used in the telecommunications system which performs functions such as supervision (control, status, and charging signals), address signaling (e.g., dialing), calling and called number identification, and audible tone signals (call progress signals indicating reorder or busy conditions, alerting, coin denominations, coin collect and coin return tones) to control the operation of switching machines in the telecommunications system. NETWORK CONTROL SIGNALING UNIT The term Network Control Signaling Unit denotes terminal equipment furnished, installed, and maintained by the Telephone Company for the provision of network control signaling. NETWORK INTERFACE The Network Interface (standard registration program jack, protector or equivalent) provided by the Telephone Company as a part of Exchange Access, Wide Area Telecommunication, or Private Line Services. The network Interface will be located at the subscribers premises. All premise services will connect to the Telecommunications Network through the Network Interface. NETWORK INTERFACE DEVICE (NID) The network interface device is provided to serve as a delineation between customer provided equipment and wiring, and telephone company facilities. A NID shall be installed at any premises not already equipped with a NID during any nonemergency premises visit. NON-SELECTIVE ACCESS LINE A Non-Selective Access Line is a central office line that provides communications capacity between the serving central office and communications switching equipment normally installed on the customer's premises that is arranged in such a manner that the station user has no control over which access line is connected to the station or other equipment for incoming or outgoing calls. OTHER COMMON CARRIER (OCC) A telecommunications Common Carrier authorized by the Federal Communications Commission (FCC) to provide a variety of services. The FCC refers to these carriers as "domestic satellite carriers," "miscellaneous common carriers" and "specialized common carriers."

9 Original Sheet 8 DEFINITION OF TERMS OUTSIDE PLANT The telephone equipment and facilities installed on, along, over, or under streets, alleys, highways, or on private right-of-ways between the central office and customer locations or between central offices. PATRON Used in connection with Composite Data Service, is a Customer of the data switching services of a Composite Data Service Vendor. PHYSICAL PREMISES LOCATION A Physical Premises Location consists of all space occupied at the same address, same building, or the same commonly owned or commonly managed geographically contiguous property. POINT OF MINIMUM PENETRATION The location on a customer s premises where the Telephone Company facilities terminate. This location is at a minimum distance from the point at which Telephone Company facilities physically penetrate the premises, consistent with Company installation practices. PREMISES The building, portion or portions of a building, used and occupied at one time by the subscriber in the conduct of his business or as a residence. Where floor space in adjoining buildings is made continuous in extent at one or more floor levels, all floor space in both buildings is considered as the same premises insofar as the subscriber who uses and occupies such continuous floor space is concerned, the two buildings otherwise being considered as separate buildings. PREMISES INSIDE WIRE Premises inside wire is defined as that wire which extends: 1. between the Standard Network Interface and telephone station or jack locations on the customer's premises, or 2. between telephone stations and/or jack locations on the customer's premises. PRIVATE BRANCH EXCHANGE SYSTEM (PBX) A "Private Branch Exchange", or Private Branch Exchange System, is an arrangement of equipment, situated on a customer's premises, consisting of a switchboard with an operating telephone, telephones connected with the switchboard, and connected by trunks with a central office, providing for intercommunication between these telephones (except that inter-communication is not provided between secretarial lines terminated on secretarial switchboards) and for communication with the general exchange system and for toll service. PRIVATE LINE SERVICE Private line service is that of furnishing facilities, to enable the customer and authorized users to communicate between specified locations for continuous use. It is furnished, subject to the availability of such facilities and the requirements of the Company. PUBLIC ACCESS LINE SERVICE Services provided through Telephone Sets installed at locations where public need exists, such as airports, bus and train stations, hotel lobbies, large business offices, public streets and highways.

10 Original Sheet 9 DEFINITION OF TERMS PUBLIC AND SEMI-PUBLIC SERVICE Exchange service in connection with which local message use is measured in terms of local messages or message units for the purpose of charging for the service, and in connection with which a coin collecting device is included as a part of the telephone equipment. REGISTERED EQUIPMENT Equipment which complies with and has been approved within the Registration provisions of Part 68 of the Federal Communications Commission s Rules and Regulations. REGRADE An application for a different, usually better, grade of service. RESIDENCE SERVICE Telephone service furnished to customers where the actual or obvious use is for domestic purposes. RESALE OF SERVICE An activity wherein one entity subscribes to the communications services and facilities of another entity and then reoffers communications service and facilities to the public (with or without "adding value") for profit. RURAL LINE SERVICE Multi-party service, other than two or four-party services, furnished in certain sections outside the base rate area, but within the exchange area. SAME BUILDING The term Same Building means a structure built as a unit. Abutting buildings are treated as the Same Building when there is free internal access between the buildings. SAME CUSTOMER In order for different services to be considered as furnished to the Same Customer, such services must be billed in the same name. Note: The business service of a customer, furnished in other than his own name and his residence service qualify as being furnished to the Same Customer. SERVICE CHARGES Service charges are non-recurring charges applicable to service connection, move or change. SELECTIVE ACCESS LINE A Selective Access Line is a central office line that provides communications capacity between the serving central office and communications switching equipment installed on the customer's premises that is arranged in such a manner that the station user has selective capability over which access line is connected to the station or other equipment for incoming or outgoing calls.

11 Original Sheet 10 DEFINITION OF TERMS SERVICE STATION A telephone customer located outside the Base Rate Area receiving switching service only, the channel beyond an agreed upon junction point at or near the Base Rate Area Boundary, as well as the telephone equipment, being provided and maintained by the customer. SERVICE TERMINATING ARRANGEMENT Company-provided equipment which terminates Basic Exchange Service at a customer's premises. The Service Terminating Arrangement provides a clearly delineated Interface which facilitates design, isolation, and testing. Where a protective connecting arrangement is required, the Service Terminating Arrangement is provided as a part of the protective connecting arrangement. SINGLE LINE CENTREX SERVICE A Centrex Main Station Line, Fully-restricted Station Line, or Dormitory Station Line, that has a single line type termination and which is not associated with any common equipment on the customer's premises. SHARING OF SERVICE (SHARED USE) Sharing is a non-profit arrangement in which several users, perhaps having no community of interest other than to communicate between the same two geographic points or to communicate with each other, collectively use communications services and facilities obtained from an underlying carrier or a miscellaneous telecommunications carrier, with each user paying the communications-related costs associated with subscription to and collective use of communications services and facilities according to its prorata usage of such communications services and facilities. SINGLE-PARTY REVERT CALLING The term "Single-Party Revert Calling" denotes a service which enables one-party customers to complete calls to extension telephones on their access line. STANDARD NETWORK INTERFACE Company-provided wiring and standard jack normally located in close proximity to the protector or equivalent at the customer's premises and under the customer's control. STATION The term "Station" denotes the network control signaling unit, and other equipment at the customer's premises which enables the customer to establish the communications connections and to effect communications through such connections. STATION EQUIPMENT Telecommunications equipment located on the customer premises, on the user side of a Standard Network Interface, that permits a customer to access the Telecommunications Network and its available services. The most common Station Equipment is the ordinary single-line Telephone Set. SUBSCRIBER See Customer

12 Original Sheet 11 DEFINITION OF TERMS TELEPHONE A telephone instrument, consisting of a transmitter, receiver, and associated apparatus normally provided by the customer, so connected as to permit the transmitting and receiving of telephone messages and which receives service from and through central office equipment and lines or other equipment and lines normally owned, maintained and operated by the Company, and provided as a part of the Company's service function. Service stations are not included under this classification. TELEPHONE COMPANY/TELEPHONE UTILITY A person, firm, partnership, cooperative organization, or corporation engaged in the business of furnishing telephone service to the public under the jurisdiction of the Public Service Commission. TELEPHONE STATION A telephone instrument, consisting of a transmitter, receiver, and associated apparatus, so connected as to permit the transmitting and receiving of telephone messages. TERMINATION CHARGE A charge applied under certain conditions, when a contract for service is terminated by the customer before the expiration of the minimum contract period. TIE TRUNK A tie trunk is a voice grade communication channel between PABX systems, Centrex systems, or between PABX and Centrex systems. TOLL MESSAGE A communication from a calling station to a station located in a different local service area for which a long distance usage charge applies. TOLL RATE The initial period charge prescribed for toll messages usually based upon the duration of the initial period and distance between exchanges. TOLL SERVICE Telephone service between exchanges or locations for which a toll rate is charged. TRIP A trip is a visit to a customer s continuous property at which time certain work ordered by the customer is to be performed. TRUNK A telecommunications channel between two switching systems. These switching systems may be Central Office types, Toll switching systems, PBX s manual or automatic switchboards, concentrators, etc. TWO LINE VOICE CARRYOVER The technique of using 3-way calling and 2 telephone lines, one for voice and one for TTY or similar equipment, to connect a caller who is deaf or hard of hearing but can speak, with another caller via the telecommunications relay service.

13 Original Sheet 12 DEFINITION OF TERMS WIRE CENTER The point at which Customers Network Access Lines converge. Used loosely, the building at that location containing one or more Central Office switching modules. A Wire Center may serve one or more Exchange Areas. WORKING SERVICE POINT A Company-provided standard miniature modular jack or equivalent, on Company facilities, through which Station Equipment is connected to the Telecommunications Network. ZONING Zoning involves the establishment of bands or relatively concentric areas throughout an exchange, and the offering of only graded service to the customers at specified rates within these particular zones.

14 Original Sheet 13 GENERAL REGULATIONS 1. OBLIGATIONS OF THE COMPANY TO FURNISH SERVICE A. OBLIGATION TO FURNISH SERVICE The Company s obligation to furnish service or to continue to furnish service is dependent on its ability to obtain, retain and maintain suitable rights and facilities and to provide for the installation of those facilities required incident to the furnishing and maintenance of that service, and its ability to secure and retain without incurring unreasonable expense suitable facilities and rights for the construction and maintenance of the necessary poles, lines, circuits, and equipment. The obligation is further dependent on the Customer s furnishing a location and environment for Company equipment and facilities which will not damage them nor interfere with their operation. B. INSTALLATION, MAINTENANCE AND REPAIR Equipment and facilities on the Customer s Premises necessary to furnishing of service, excluding power wiring and outlets for which the Customer is responsible, are provided by the Company, except as expressly specified elsewhere in this document. All work of installation, relocation, changing, and maintenance of such equipment and facilities on the Customer s Premises is done by the Company. Equipment and facilities furnished by the Company on the Customer s Premises are the property of the Company, whose agents and employees shall have the right to enter the Premises at any reasonable hour for the purposes of installing, inspecting or repairing, or upon termination of the service, of removing the equipment and facilities. The Company will maintain and repair equipment and facilities which it furnishes to Customers, for as long a period as replacement components are available from the manufacturer or from stock. When the manufacturer will no longer provide the required components, and none are available from stock, components required will be provided at charges based on cost when the Company is able, by special manufacture or procurement from outside sources, to procure the require components.

15 Original Sheet 14 GENERAL REGULATIONS 2. ESTABLISHING SERVICE A. INITIAL SERVICE PERIOD The Initial Service Period is the minimum length of time for which a Customer is obliged to pay for service, facilities, and equipment, whether or not retained in place by the Customer for such minimum length of time. The Initial Service Period from date of installation is one month, except where a longer period is required elsewhere in this document for particular services. B. BASIC EXCHANGE SERVICE Customers shall apply for the Class of Service, as defined elsewhere in this document, which accords with the manner in which the service will be used by the Customer. Residence Service is furnished only for social or domestic use by the Customer, members of the Customer s household, or guests or sublessees of the Customer, at the location of the Customer s residence. Business Service is furnished for use by the Customer, its employees, representatives, patients or guests, of a business, professional, institutional or occupational nature, and/or where a business directory listing is furnished. C. POWER SUPPLY When Company equipment, installed on the premises of a Customer or authorized user, requires power for its operation, the Customer is required to provide such power wiring and power outlets at the locations where the equipment is to be installed. D. SPECIAL PROTECTIVE EQUIPMENT Special protection may be required for Network Access Lines furnished at power generating stations, substations, or other locations which may be subject to high ground potential during fault conditions. When the Company determines that protective equipment is required, it will be furnished, installed and maintained by the Company except that the Customer may furnish such equipment subject to the approval by the Company of the proper levels of protection and the equipment provided by the Customer.

16 Original Sheet 15 GENERAL REGULATIONS 3. TERMINATION OF SERVICE Equipment furnished by the Company shall, upon termination of service from any cause whatsoever, be returned to it in good condition. Except as provided below, no charges apply for removal of service or equipment. A. CUSTOMER INITIATED TERMINATION 1. Service may be terminated after the expiration of the Initial Service Period upon notice being given to the Company a reasonable period in advance and upon agreement to pay the charges due to the date of termination of the service. 2. When service is terminated by discontinuance or relocation prior to the expiration of the Initial Service Period, in addition to all charges due for service which has been furnished, termination liabilities apply as shown elsewhere in this document for the service prevailing at the time of installation: a. Service for which the Initial Service Period is one month: Charges are due for the entire Initial Service Period. b. Service for which the Initial Service Period is more than one month: Prorated termination liabilities apply to the unexpired portion of the Initial Service Period. c. Special equipment for which the Initial Service Period is in excess of one month at the same location: Such proportion of the sum of the cost of the equipment, cost of installation, and the cost of removal, less the salvage value of the equipment removed, as the unexpired portion of the Initial Service Period bears to the full Initial Service Period. 3. Cancellation of application for service, in whole or in part: prior to completion of engineering, construction, or installation; or after completion of engineering, construction and installation, but prior to establishment of service; The termination liability will be the loss on equipment and facilities in the process of building or being installed, cost of installation labor, cost of removal and other expenses not to exceed applicable service connection and installation charges and termination liabilities as specified elsewhere in this document. 4. For the purpose of this document in the application of termination liabilities, the last item (such as station lines, attendant positions, etc.) installed will be considered the first to be removed.

Abuse or Fraudulent Use The Company may, upon at least 5 calendar days written notice to the Customer, without incurring any liability, forthwith deny or disconnect service because of abuse or fraudulent use of the service. Service must be disconnected within 15 days of the date of the notice unless the Customer and the Company agree to extend this time period. In case of a dangerous or emergency condition where the public interest requires immediate action or pursuant to governmental requirements, service may be disconnected without notice for as long as the condition exists. Abuse or fraudulent use of service includes, without specific limitations, the following: a. The use of service or facilities of the Company to transmit a message or to locate a person or otherwise to give or obtain information, without payment of the charge applicable for service; b. The obtaining, or attempting to obtain, or assisting another to obtain or to attempt to obtain Telecommunications Service, by rearranging, tampering with, or making connection with any facilities of the Company, or by any trick, scheme, false representation, or false credit device, or by or through any other fraudulent means or device whatsoever, with intent to avoid the payment, in whole or in part, of the regular charge for each such service; c. The use of service or facilities of the Company for a call or calls, anonymous or otherwise, if in a manner reasonably expected to frighten, abuse, torment or harass another; d. The use of profane or obscene language; e. The use of the service in such a manner as to interfere with the use of the service by one or more other Customers; and f. The impersonation of another with fraudulent intent. 2. Use of Service for Unlawful Purposes Service is furnished with the understanding that it will be used only for lawful purposes. The Company will disconnect or deny service if it has knowledge that the service is being or will be used for unlawful purposes. Service will not be furnished if any law enforcement agency, acting within its jurisdiction, advises that such service is being used or will be used in violation of law, and requests that such service be disconnected or denied.

The Company may, upon at least 5 calendar days written notice to the Customer, without incurring any liability, forthwith deny or disconnect service for any of the reasons shown below. Service must be disconnected within 15 days of the date of the notice unless the Customer and the Company agree to extend this time period. b. For the purposes of these regulations a Company bill is considered a delinquent account one day after issuance. c. The Company may disconnect or refuse service for any of the following reasons: 1) Failure to pay a delinquent account or to comply with the terms of a 2) Failure of a Customer to make suitable deposit or guarantee in lieu of deposit arrangements as required elsewhere in this document. (3) Failure to comply with deposit or guarantee in lieu of deposits arrangements. (4) Where subsequent credit information indicates that the initial application for service was false or incomplete to the extent that a deposit or guarantee would be required or service refused under the applicable regulations or as specified elsewhere in this document. (5) Any other violation of the Company's regulations filed with the Public Service Commission of Wisconsin or the Federal Communications Commission. (6) Charges applicable for restoral of service are found elsewhere in this document. d. The Company will not attempt to collect by threat of disconnection or refusal of service for any of the following reasons: (1) Delinquency in payment for service by a previous occupant of the Premises to be a member of the same household residing at the same Premises; (2) Delinquency in payment for another Class of Service; (3) Failure to pay another Customer's bill as guarantor thereof; (4) Failure to pay for directory advertising or charges for non-utility service billed by the Company; (5) Failure to pay charges arising from any under billing occurring more than one year prior to the current billing and due to any misapplication of rates or due to inaccurate metering; (6) Failure to pay a delinquent account over 6 months old where no collection steps have been taken within 6 months of the date on the delinquent bill or bills, unless the passage of additional time is a result of other provisions described herein or from good faith negotiations or arrangements made with the Customer.

19 Original Sheet 18 GENERAL REGULATIONS 4. TELEPHONE NUMBERS The Customer has no property right in the telephone number which is assigned by the Company nor any right to continuance of service through any particular Central Office, and the Company reserves the right to change the telephone number or the Central Office designation, or both, of the Customer whenever it deems it necessary to do so in the conduct of its business. 5. PAYMENT FOR SERVICE A. GENERAL The Customer is responsible for payment of all charges for facilities and services furnished the Customer including charges for services originated, or charges accepted, at such facilities by the customer or any other person. An administrative charge will apply whenever a customer's check or other draft is returned, dishonored, or requires special handling by the Company. All Exchanges Administrative charge $25.00 B. LATE PAYMENT CHARGE A late payment charge the greater of $5.00 or 1.5% will be applied to any unpaid balance carried over to the next month's bill. 6. CUSTOMER BILLING Bills to customers will be rendered regularly and will contain a clear listing of charges. The Company will comply with reasonable customer requests for itemized statements of charges. The Company will not bill the customer for delinquency in payments by a previous occupant of the Premises other than a member of the same household residing at the same Premises. All recurring monthly charges are billed in advance. All bills are due on the date listed on the customer s bill. Treatment to collect the bill may be started prior to that date in cases where the long distance usage in a month exceeds the previous individual account average. Bills are payable by mail, at any business office of the Company, or at any other location designated by the Company, or by other payment arrangements agreed to by the Company and the customer.

20 Original Sheet 19 GENERAL REGULATIONS 7. ADVANCE PAYMENT AND DEPOSITS A. PAYMENTS IN ADVANCE OF SERVICE Applicants for service who have no account with the Company or whose financial responsibility is not a matter of general knowledge may be required to make an advance payment at the time of application equal to the service connection or installation charges, or both, and the Initial Service Period charges for the service provided. The amount of the advance payment is credited to the Customer's account as applying to any indebtedness under this document. B. DEPOSITS The Company may require an applicant or a Customer to make a suitable deposit to be held by the Company as a guarantee of the payment for charges for Telecommunications Service if circumstances warrant such based on the customer s credit history. The fact that a deposit has been made in no way relieves the applicant or Customer from complying with the Company's regulations as to advance payments and the prompt payment of bills on presentation. The Company may return a customer s deposit after one-year if the customer s payment history is satisfactory. Deposits will be returned no later than when service is terminated if the account has a zero balance. 1. Amount of Deposit The maximum deposit for new service shall not exceed an amount estimated to equal the charges for one month's recurring charges for service and for two months' message toll service. Deposits for existing Customers shall not exceed the bill for one month's recurring charges for service plus the highest actual message toll charges for two consecutive months within the preceding l2 month period. If any service is terminated, the amount of the deposit is credited to the Customer's account and any credit balance which may remain is refunded. 2. Arrearages An arrearage owed by a Customer may be deducted from the Customer's deposit under the following conditions: a. A deposit may be used by the Company to satisfy an arrearage occurring after the deposit was made. b. If the Company deducts an arrearage from a Customer deposit, it may require the Customer to bring the deposit up to its original amount. Failure of the Customer to do so within 20 days of mailing a written request for payment is ground for disconnection. c. When a deposit is refunded to the Customer, the Company may first deduct any arrearage owed by the Customer, whether the arrearage arose prior to or after the date of deposit. 3. Interest on Deposit The Company is not required to pay interest on customer deposits, but may do so at the Company s discretion. Any interest rate will be determined annually.

21 Original Sheet 20 GENERAL REGULATIONS 7. ADVANCE PAYMENT AND DEPOSITS C. GUARANTEE IN LIEU OF DEPOSIT A guarantor, satisfactory to the Company, may be accepted in lieu of a deposit. The guarantor shall guarantee payment of a specific sum not to exceed the cash deposit requirement. The Company may request an existing Customer to furnish a guarantee, an additional guarantee or a deposit upon written notice to the Customer if increased usage warrants such a request or if the Customer's payment record is unsatisfactory. The guarantor's obligation will automatically cease after the Customer discontinues service, upon 30 day's written notice by the guarantor to the Company, or at the end of 2 years. If requested by the Customer, a written explanation of the reason for the deposit or guarantee will be provided by the Company. Where the Customer's business is of a hazardous or temporary nature, the Company may adjust the deposit or guarantee requirement and bill the Customer on other than a monthly basis with a corresponding adjustment in the deposit or guarantor requirements. The guarantor shall receive copies of all disconnect notices sent to the Customer whose account he has guaranteed if the guarantor requests such notice. The notice option shall be part of the guarantee contract. D. DEFERRED PAYMENT AGREEMENT IN LIEU OF DEPOSIT In lieu of a cash deposit or guarantee, an applicant for new Residence Service who has an outstanding account accrued within the last six years with the same utility shall have the right to receive service from that utility under a Deferred Payment Agreement for the outstanding account. A Deferred Payment Agreement is available to Residence Service Customers unable to pay an outstanding bill in full. The terms include a reasonable initial payment and installment payments to pay the outstanding bill within a reasonable time. No finance charge applies. Any payment made by a Customer in compliance with a Deferred Payment Agreement will first be considered in payment of the previous outstanding account with any remainder credited to the current bill. If a Customer has not fulfilled the terms of a Deferred Payment Agreement, the Company shall have the right to disconnect service upon at least five calendar days written notice and under such circumstances the Company shall not be required to offer subsequent negotiation of a Deferred Payment Agreement prior to disconnection.

22 Original Sheet 21 GENERAL REGULATIONS 8. ADJUSTMENT OF CHARGES A. INTERRUPTIONS The Company will, in the event the Customer's Basic Exchange Service is interrupted, make a subsequent appropriate billing adjustment subject to the following conditions: 1. Credit for interruption is not allowed where the outage is caused by a willful act or negligence of the Customer or the failure of facilities provided by the Customer. 2. Credit will be allowed from the time notification is received from the Customer or from the time of discovery by the Company, whichever is earlier, when each interruption comprises at least 24 consecutive hours of outage. 3. A major fraction of the last additional day, consecutive with a period consisting of 24 hours of outage, is considered a full day. 4. Credit for interruption is based upon the number of consecutive days of each outage. 5. For the purpose of determining the allowance for interruption, each month is considered to have 30 days. The refund to the Customer shall be the prorata part of the month's charges for the period of days that the portion of the service is rendered useless or inoperative. 9. LIABILITY OF THE COMPANY The Company s liability, if any, for its willful misconduct is not limited by this document. A. LIABILITY OF THE COMPANY FOR DAMAGES TO CUSTOMER PROPERTY The Company is not liable for any defacement or damage to the Customer's Premises resulting from the existence of the Company's apparatus and associated wiring on such Premises, or from the installation or removal thereof, when such defacement or damage is not the result of the negligence of the Company. B. LIABILITY OF THE COMPANY FOR DAMAGES TO CUSTOMER SERVICES 1. With respect to any claim or suit, by a Customer or by any others, for damages associated with the installation, provision, termination, maintenance, repair, or restoral of service, the Company s liability, if any, shall not exceed an amount equal to the proportionate part of the monthly recurring charge for the service for the period during which the service was affected. This liability shall be in addition to any amounts that may otherwise be due the Customer as specified elsewhere in this document as an allowance for interruptions of service.

The Company shall not be liable for damages arising out of mistakes, omissions, interruptions, delays, errors or defects in transmission, or for other injury, including but not limited to injuries to persons or property from voltages or currents transmitted over the facilities of the Company; a. if caused by Customer-provided equipment (except where a contributing cause is the malfunctioning of a Company-provided connecting arrangement, in which event the liability of the Company shall not exceed an amount equal to a proportional amount of the Company billing exceed an amount equal to a proportional amount of the Company billing for the period of service during which such mistake, omission, interruption, delay, error, defect in transmission or injury occurs); or b. if not prevented by Customer-provided equipment but which would have been prevented had Company-provided equipment been used. C. LIABILITY OF THE COMPANY FOR DAMAGES FROM TRANSMISSION SERVICES OR FACILITIES The Customer indemnifies and saves the Company harmless against claims for libel, slander, or infringement of copyright from the material transmitted over its facilities; against claims for infringement of patents arising from combining with, or using in connection with facilities of the Company, apparatus and systems of the Customer; and against all other claims arising out of any act or omission of the Customer in connection with facilities provided by the Company. D. LIABILITY OF THE COMPANY FOR DAMAGES FROM DIRECTORY LISTINGS The Company's liability for the impairment of the Customer's service arising from errors in or omissions of directory listings shall be limited to an abatement or refund of an amount not exceeding the monthly charges for Exchanges Services, excluding Additional Local Message charges, during the period covered by the directory in which the error or omission occurs. E. OTHER LIABILITIES OF THE COMPANY When the lines of other telephone companies are used in establishing connections to points not reached by the Company's line, the Company is not liable for any act or omission of the other company or companies. F. UNAUTHORIZED ACCESS AND HACKING Unauthorized Access and Hacking - Except for physical damage to Customer s transmission facilities or Customer premise equipment directly caused by the Company s negligence or willful misconduct, the Company is not responsible for unauthorized access to, or alteration, theft, or destruction of, Customer s Equipment, data, programs or other information through accident, wrongful means or any other cause while such information is stored on or transmitted across Company-provided network facilities or Customer premise equipment. Customer is responsible for any Company service or usage charges resulting from any such unauthorized access, unless a Company document, schedule or other written agreement expressly states otherwise.
