8 Program Descriptions Families and Children: Families and Children is a managed care program that covers health care for the following people who have Medical Assistance: Children under the age of 21 Parents and caretakers of a dependent child Pregnant women Low-income adults without a dependent child Our Families and Children program is called Medical Assistance. MinnesotaCare: MinnesotaCare is a managed care program that covers health care for people who do not have access to affordable health care coverage. Some members may be required to pay a premium to the State. Minnesota Senior Care Plus (MSC+): MSC+ is a managed care program that covers health care and Elderly Waiver services for people who: are age 65 and over and have Medical Assistance. If you are enrolled in MSC+, you may have another option. Check with your county financial worker to see if you can enroll in the Minnesota Senior Health Options (MSHO) program. MSHO is a voluntary program. Special Needs BasicCare (SNBC): SNBC is a voluntary managed care program that covers health care for people with disabilities who: are age 18 through 64, * have Medical Assistance, and have either both Medicare Parts A and B or Medical Assistance only. vi * People in SNBC who turn 65 will be enrolled into a senior managed care program if they continue to meet the eligibility criteria to enroll in managed care. Our SNBC programs are called SingleCare and SharedCare. SingleCare (SNBC) provides Medical Assistance benefits for those not eligible for Medicare. SharedCare (SNBC) provides only Medical Assistance benefits for those that are eligible for Medicare benefits but get them from other health plans. Introduction South Country Health Alliance (also referred to as South Country or SCHA) is a county-owned health plan that serves 12 Minnesota counties. Through South Country, these counties have joined together to help improve how you receive your health care. Clinics, doctors, dentists, eye doctors, chiropractors, hospitals, nursing homes, and home care agencies are working with us to see that you get the best care. The 12 counties in our service area include: Brown, Dodge, Freeborn, Goodhue, Kanabec, Morrison, Sibley, Steele, Todd, Wabasha, Wadena, and Waseca. We work closely with county health care providers, human service agencies, and county public health departments in these counties to support your wellness. South Country supports five different programs to meet our members needs. How to find network providers in your area The providers in this directory are organized by state, county, and city. If you have questions or require assistance in finding a provider, please call our Member Services Department at the phone number listed on the front cover of this book. You must get all of your health care from South Country network providers. There are some exceptions. Examples of exceptions are: emergency or urgent care, open access services, and other previously authorized services. If you have questions about the providers you should use, please contact Member Services at the phone number listed on the front cover of this book. 9 How to access specialty providers You can see any provider in our network without referral, including specialists. This directory has Specialty Care Provider listings organized alphabetically by specialty, by state, county and then by city. Locate the specialty you want. Then locate your state, county, city, or cities nearby, and then look to find the specialist you need. You can get additional information on how to access specialty care by calling Member Services at the phone number listed on the front cover of this book. Or visit How to access emergency care In an emergency that needs treatment right away, either call 911 or go to the nearest emergency room. You do not need to get approval first from your primary care provider. But, please call your primary care clinic as soon as possible. You may get covered emergency medical care whenever you need it, anywhere in the United States or its territories. Our plan covers ambulance services in situations where getting to the emergency room in any other way could endanger your health. How to access urgently needed services Urgently needed services are a non-emergency, unforeseen medical illness, injury, or condition, that requires immediate medical care. Urgently needed services may be furnished by network providers or by out-ofnetwork providers when network providers are temporarily unavailable or inaccessible. You should always try to obtain urgently needed services from network providers. However, if providers are temporarily unavailable or inaccessible and it is not reasonable to wait to obtain care from your network provider when the network becomes available, we will cover urgently needed services that you get from an out-of-network provider. Network urgent care centers are listed in this directory and can also be found on our website: If you cannot find an urgent care center in your area, or have questions, call Member Services at the phone number printed on the front cover of this book. How MSC+ members can get more information about Home and Community Based Service providers Home and Community Based Service providers are not listed in this directory. To find out more about these services, see your Member Handbook. These services may be accessed through your care coordinator as part of your care plan. Call your care coordinator if you need a listing of providers or more information about how to access these services. If you have additional questions, call Member Services at the phone number listed on the front cover of this book. How MSC+ members can get more information about Long Term Services and Supports You may be able to get long-term services and supports (LTSS), such as services provided by the Elderly Waiver (EW) program as a South Country Health Alliance member. Long-term services and supports are help for people who need assistance to do everyday tasks like taking a bath, getting dressed, making food, and taking medicine. Most of these services are provided at your home or in your community but could be provided in a nursing home or hospital. To find out more about these services, see your Member Handbook. Call Member Services at the phone number listed on the front cover of this book to get more information on how to access these services. How to find a network pharmacy Pharmacies are not listed in this directory, but are available in a separate directory. You can look up a pharmacy on our website at by clicking on Find a Pharmacy. If you need help finding a pharmacy or have questions, please call Member Services at the phone number listed on the front cover. Enrollees with Medicare must use a Medicare Part D prescription drug plan for Part-D covered drugs. Contact your Medicare Part D prescription drug plan if you have questions about how to access Part D drugs. This directory list changes often. For the most current network use the search at vii

10 Important Information Care Coordination MSC+, SharedCare, and SingleCare members have a care coordinator available to assist with coordinating needed services. Care coordination is provided for you by local county social workers and nurses or a nurse and social worker within South Country Health Alliance for some members. A care coordinator can give you information about community services, help you find services to stay in your home or community, and help you find services to move out of a nursing home or other facility. They can help you make appointments and coordinate services following hospitalization. If you need assistance to contact a care coordinator, call Member Services at the phone number listed on the front cover of this book. Clinics When you are a member or become a member of South Country, you chose or were assigned to a primary care clinic (PCC). Your PCC can provide most of the health care services you need, and will help coordinate your care. This provider will also advise you if you need to see specialists. You may change your PCC. If you want to switch your primary care clinic, you can call Member Services at the phone number listed on the front cover of this book. Cost Sharing You may be required to pay an amount toward some health care services. This called cost sharing. Some examples of cost sharing are copays or deductibles for certain services. You are responsible to pay your cost sharing amount directly to your pharmacy or healthcare provider. Members get a Member Handbook with more information about cost sharing. Member Handbook To get detailed information about covered health care services, see your Member Handbook. Oral interpretation can be given to you in your language. You may be able to get written information in your language. You must show this card whenever you get health care services, including when you get a prescription filled. Your Member ID cards will be mailed to you in a separate envelope from your member packet. Service Accessibility If you need special access to get services from a provider, you can get more information by calling Member Services at the phone number listed on the front cover of this book if you are concerned about a service being covered or not. You must get our approval before you get certain services or before we pay for them. Services Behavioral Health Behavioral health services include care and treatment for depression, anxiety or other behavioral health issues that interfere with your daily functioning. Elderly Waiver Home and Community Based Services Minnesota Senior Care Plus members If you are age 65 or over and need help to stay in your home, a program called Elderly Waiver (EW) can help. EW offers more services than regular Medical Assistance. Services may include visits by a nurse, home care services, homemaker services, chore services, and more. Your Care Coordinator can arrange these services for you. If you want more information, please call Member Services at the phone number listed on the front cover of this book. Elderly Waiver Obligation Minnesota Senior Care Plus members If you get EW services and your income is over the income limit, you may have to pay a monthly amount. This is called the EW waiver obligation. Your county will tell you about any EW waiver obligation when your eligibility is determined. Home Care Home Care services include the following: Skilled nursing Rehabilitation therapies (for example: speech, physical, respiratory, and occupational) Home health aide Medical equipment and supplies Personal Care Assistant (PCA): MSC+ only Home Care Nursing (HCN) services: MSC+ only If you want more information, call Member Services at the phone number listed on the front cover of this book. Families and Children, SharedCare, and SingleCare members can get information on how to access Personal Care Assistant (PCA) services by calling your county of residence intake for long term care services and supports. Families and Children, SharedCare, and SingleCare members can get information on how to access Home Care Nursing (HCN) services by calling a home care agency for an assessment. Medical and Supplies Medical and Supplies is any medical equipment you can use in the home to improve your quality of living. It can also provide therapeutic benefits to people with certain medical conditions or illnesses. Examples are nebulizers, wheelchairs, crutches and many others. 12 Nursing Home and Skilled Nursing Home Care for MSC+ or non-integrated SNBC SharedCare and SingleCare members. For MSC+ members,we will cover a total of 180 days of nursing home care. If you need nursing home care for more than 180 days, DHS will pay for your care. This applies unless you were already in the nursing home when you joined our plan. For non-integrated SNBC SharedCare and SingleCare members, we will cover 100 days of nursing home care. If you need nursing home care for more than 100 days, DHS will pay for your stay in the nursing home. The health plan will continue to pay for all health care costs. This applies unless you were already in the nursing home when you joined our plan. You can get more information on how to access nursing home services by calling Member Services at the phone number listed on the front cover. Open Access Services For some services, you may choose to get care from any health care provider, clinic, hospital, pharmacy, or family planning agency even if the service/provider is not in our network. These are called open access services and include the following: Family planning Diagnosis of infertility Testing and treatment of sexually transmitted diseases Testing for AIDS or HIV-related conditions Pharmacy Members must use our network of pharmacies for drugs covered by our health plan. Remember to have the following available when you call to refill your prescription: South Country Health Alliance Member Identification (ID) Card Minnesota Health Care Programs card Cards for any other health coverage you have, such as Medicare Part D or private insurance It is important to remember to show both your Member Identification (ID) card and your Minnesota Health Care Programs card when picking up your prescriptions. To see South Country s drug formulary, go to and choose Find A Drug to search the formulary. People with Medicare must enroll in a Medicare Prescription Drug Program (Medicare Part D) to get most of their drugs. Substance Use Disorder Substance use disorder (SUD) services provide care and treatment for substance use disorders. Substance use disorders can include abuse of alcohol, narcotics, or other drugs. Call Member Services for more information on substance use disorder screening options. Transition of Care When you join South Country we will help you continue to get necessary health care services that were authorized before you joined our plan. Some examples are the following: Behavioral health services Orthodontia Prescriptions Services for at-risk pregnancy Substance use disorder services If you are currently getting care from a provider who is not in our provider network, we will help you transition to a participating provider. 13 Transportation for Families and Children, MSC+, SharedCare, and SingleCare members. Medical transportation is available for MinnesotaCare Children under age 19. We are not required to provide transportation to your primary care clinic if it is over 30 miles from your home, if another primary care clinic is available within 30 miles. Exceptions to this rule can be made in certain circumstances. If you need a ride and there isn t a closer clinic in our network please call our Member Services at the phone number listed on the front cover. Members will get a Member Handbook with more information about transportation. Call Member Services to arrange transportation services. Urgent/After Hours/Emergency Care When possible, call your primary care clinic before going to the emergency room. They can help you decide if you need urgent or emergency care. Your primary care clinic s phone number will be on your Member Identification (ID) Card. Also, South Country members have access to the Ask Mayo Clinic nurse advice line. Call Ask Mayo Clinic if you need help deciding if you need urgent or emergency care. Nurses are available 24 hours a day, seven days a week at (toll free), or TTY/TDD users call In an emergency that needs treatment right away, either call 911 or go to the nearest emergency room. South Country Health Alliance covers emergency ambulance services. Rides to urgent care or same day care are based on provider availability.
