Personal Discovery Questionnaire This Personal Discovery Questionnaire is in-depth and personal. You re going to hit the ground running rather than spending weeks, months or even years trying to seek this information out. If you find talking easier than writing, then complete whatever you can and let s get talking, face-to-face or online. How much do you really know yourself? Switched on and ready for life? Chances are it s not as much as you think. Because many people are either hiding from, or unaware of their essence and how to express themselves authentically. If you feel it s time for your big change then let s work together and turn your self-awareness light on. Let s see who you really are and where life wants to lead you. We can read life s messages so you are present and able to attract what you want, instead of perpetually chasing it (away). If you re avoiding your fears and unclear what your core values are, your unexamined beliefs may be restricting your choices without you being aware of it, and in this way limiting your outcomes in life. Rather than choosing how to respond to life, your automatic thoughts may be creating the same undesirable outcomes as you unconsciously react to life. I often refer to this as your survival story, which has been successful in helping you survive life thus far. Are you ready to respond with awareness and allow your dreams to come true? This document will provide you with the perfect mirror to get a sense of yourself right now. You can share it with your nearest and dearest, your therapist, coach, or partner. And if you re truly ready for your big change you can share it with me. So, fasten your seatbelt and loosen-up for your journey of self-discovery. It s like Columbo. We look for clues, I ask the right questions, things fall together, the pennies drop and you become your own authentic man (or woman).
1. Your wish list What s on your Wish List? Life will provide what you ask for, or something even better. Naming what you really want can be challenging, so take a deep breath, get it out of your head, and on paper. Go for gold! You can always edit later.
2. Your family Where have your family come from? What are your roots? Did they come from humble beginnings or wealth and legacy? What s your ancestral history and what (if anything) is expected from you? What was happening in the lives of your parents, in particular your mother, when she was pregnant with you?
Were you an early or a late birth? How was your labour? Did your mother have post-natal depression? Were you breast fed? For how long? Please provide a brief family history, including siblings, relevant adults, ancestors, major influences on the family, status, (in)stability, etc.
3. Your childhood What was the general atmosphere in your home when you were a child and when / how did this change? What was the general atmosphere in your school and how did that change from school to school, to further education and work? What was your personal social standing during childhood, puberty, education, work, sport, religious and any other communities? What is it now?
4. Your mind, body & spirit Did you have any illnesses, accidents, traumas, dramas? Any idea what caused them? What was going on in your life, or the lives of those around you, when these events took place? Do you have any physical or emotional scars? What are their stories?
Do you have issues with your sexual health, sexual expression or sexuality? Are you healthily enjoying, expressing, connecting and growing in this part of your life? When did you gain or lose weight, start to lose your hair, start smoking, over-eating, over-exercising, regularly drinking, taking legal or illegal drugs, etc.?
5. Your defining moments How about defining moments? The good, the bad, happy or sad? The silliest things may have had the most impact, especially if they just happen to cross your mind. When did your behaviour, or outlook on life change? What was taking place, in and around you, at these specific points in your life? Do you remember your main home relocations (if any) or changes in your social structure? Why did
these changes take place? How did they affect you or those around you? Would you call yourself an over-achiever, an under-achiever, neither, or both? Why?
6. Your driving forces In an ideal world, how would you like to be living your life right now? Is there anyone or anything that you feel powerless over? Who or what is running your life? Are you aware of any cycles or patterns in your life, from financial or relationship, to menstrual and lunar?
Do you have a sense of your current life purpose? Do you have clarity around your values or issues that are dear to your heart? What has crossed your mind that I haven't asked you, as you've completed this Personal Discovery Questionnaire? Please jot it down to bring to the table when you are ready.

I will send you a link for us to meet in a virtual Zoom room once I accept your booking. My online sessions are recorded for your reference. You learn to keep your life on track as you ground new goodness, grow into your creative new life, and grow out of your old life of limiting survival. Most people happily move on in their lives and don't need to work with me anymore. Some reappear as challenging opportunities arise or if they simply wish to check in. Warning! If we do work together, I need you to know that life will never be the same again. You cannot undo awareness. You cannot unknow what has been revealed.
