Please assist me by completing the following questionnaire and returning it to me. The information you provide will be kept in confidence and not shared with parents or teachers. The more information you provide and the more serious you take the task, the better recommendation I can write for you. Feel free to add information not requested if you think it would help me to better understand you and your needs.
