COBRA Termination When processing a COBRA Event in BBP s COBRA system you will use the QB wizard. Scroll over the members tab at the top and click QB Qualified Beneficiaries. Then click the add a new QB There are then 7 steps, but you can finish in 3 steps. Total time should be about 3 5 minutes.
First Screen: You need to complete all information with a red box. Your group name at the top should be defaulted, but then you have to add the First Name, Last Name, SSN, etc. Once you complete please click the next button at the bottom of the screen. See next page for a completed page.
Completed first page: After you click next you will be on step 2 event. As you can see there is not much information to add. You first need to choose if this is an employee event (termination/retirement) or a dependent event (Divorce/Child Reaching Age). You then choose the event and put in the date of the event. Finally, put their date of hire in the HIPAA information. See screen shots on next page:
Completed page: Now you are on step 3. You just have to add in the plan information. You will first click add a plan. Then you will get a plan screen of all plans available. If you are missing a plan please call BBP so we can get any missing plans added at 630 773 2228. Once you select a plan everything will be completed based on set up. So if benefits go to the end of the month the dates fill out correctly based on event and plan set up. All you have to do at this point is choose a coverage level and then click insert.
See completed screen: You then complete this process for each COBRA eligible benefit; Medical, Dental, Vision, EAP, FSA Medical, HRA, etc. Once you complete adding the benefits you can click finish and we will start the process of preparing the form to mail with the proof of mailing. If you would like to continue on with adding dependents, subsidies, letter inserts, please continue on by clicking next. If you do not add dependents our letters are COBRA Court tested as they always include the words to all qualified beneficiaries, if any. So if you do not add or you miss one the letter covers you in a court of law. To add a dependent is a 2 step process. Start by clicking Add a Dependent You will then see the screen below: You need to complete everything in red. Start by choosing the relationship, and complete first and last name. See completed screen below:
Then click save and a new screen pops up below to add all benefits that is based on the plan screen. For example, on the plan screen I choose employee spouse. So by adding a spouse it will show the medical plan that I can add Jane Doe to. To complete I just need to click add plan.

Above is the completed screen so you can see all benefits have shifted from available to add. Then you can either add more dependents by clicking add a dependent or you click next to move onto the next screen.
The next screen is the subsidy screen. So if you have a severance or are paying for part of the COBRA you can add that in under this screen. If not, please click next. The next screen is the letter inserts. So if in IL you want to check Illinois State Continuation. This will cover you on that law as well. Then click next.
The next screen is additional attachments. These attachments would be for retirement plan information or conversion policies. These are additional fees and if you do not have any set up you will see nothing to add on this page. Please note for no additional fee we can have this information posted on the COBRA portal for QB s to see when they log in. Please click next to go to notes. The note screen is just a recap of what was completed. So you will just want to click finish. Once you click finish you will go to the QB home page. You will just repeat these steps for each QB. Please remember you can also complete are termination excel or pdf worksheet and we will do the above for you or we can put your census information into the system so that you can just log in and click terminate.
