PATERNITY TESTING CORPORATION DNA SPECIMEN COLLECTION PROCEDURE Legal Private Case CAUTION: If an individual appearing for specimen collection is one of your friends or relatives, please call PTC before collecting the specimens. PTC must determine whether there is a conflict of interest, which would prevent you from collecting the specimen. It is very important that you follow these procedures exactly. At the top of the Specimen Collection Form, fill in draw location, the amount of payment received on behalf of PTC (if any), and the name of the person making the payment. 2. Print the names of all parties to the case on the appropriate line, even if you will not be obtaining a specimen for one of the parties to a case. 3. For each client who is present, the sample collector should complete the questions on the form, printing neatly so that the laboratory can record accurate information. For race, circle one of the listed choices, or fill in the race if the correct one is not listed. 4. If any child being tested is an adult, please print the child s name, address, telephone number and ID number on the back of the form. 5. Ask mother and alleged father (and any adult child ) to show you photo ID (such as a driver s license), and make sure the picture matches the individual. a. If the client does not have a picture ID please note that fact on the back of the form, and also list whatever identification the client did have. b. Make a note of any mistakes on the ID such as a changed or misspelled name or address or an incorrect social security number, and write down the mistaken info. c. Take photos of each ID presented. 6. Obtain the right thumbprint of the mother and alleged father in the marked boxes on the form. Page 1 of 4

7. After filling in the form, get all clients to clearly print their Name, Date, and Signature on separate pieces of blank A4 paper. This will be used for the group photograph. Either mother or alleged father may sign the paper of the child. 8. Take a group photograph of mother, alleged father, and child(ren). They should all be each holding the individual A4 paper with their name, date and signature. All clients that come in together for specimen collection should be included in a single group picture, instead of separate pictures. If any party is wearing a hat, hood or sunglasses, please ask them to take off those items before taking the picture. If they refuse, please make a note on the back of the specimen collection form stating the name of the person and that you asked them to remove their hat, hood or sunglasses, and that they refused---and then sign your name at the end of your note. Specimen Collection COMPLETE THE ENTIRE SPECIMEN COLLECTION ON ONE INDIVIDUAL, INCLUDING SEALING THE SPECIMEN(S) IN A SPECIMEN ENVELOPE AND INITIALING THE ENVELOPE, BEFORE BEGINNING THE SPECIMEN COLLECTION ON ANOTHER INDIVIDUAL. Gloves must be worn throughout the specimen collection procedure. Buccal Swab Protocol 9. Open swab kit and remove the specimen envelope for the client whose sample you are obtaining (pink for mother, yellow for child, blue for alleged father, and white for additional children or alleged fathers). [Please save unused specimen envelopes and swabs.] 10. Remove the specimen envelope from its plastic wrapper. Make certain that the desiccant packet(s) remain in the specimen envelope, so that they may dry the sample. THE DESICCANT PACKETS ARE VERY IMPORTANT. 11. Print the client s name on the specimen envelope, along with the date. If you are using a white envelope, also print the relationship of the client (print child or alleged father ). 12. Use four swabs per person. Open the first swab package and remove one sterile swab. DO NOT TOUCH THE SWAB TIP. NOTHING MUST CONTACT THE TIP EXCEPT THE INSIDE OF THE MOUTH OF THE INDIVIDUAL BEING TESTED. 13. Brush the swab firmly up and down against the inside of the client s cheek for at least 15 seconds. Rotate or twirl the swab as you rub it up and down, so that the entire surface of the swab tip comes in contact with the subject s cheek. It is essential to obtain a sufficient amount of DNA to prevent the need for a redraw. Therefore, you should press as firmly as possible WITHOUT causing pain. Generally a better sample Page 2 of 4

can be obtained by allowing an adult or older child to swab his or her own cheek after you have explained how, because the individual can usually press harder than the sample collector would. The sample collector must decide which is best. THE SAMPLE COLLECTOR MUST NEVER LET THE CLIENT OUT OF SIGHT WHILE THE CLIENT IS SWABBING. Never leave clients alone in the room after specimens have been collected but before the specimen envelopes are sealed in the specimen bag. 14. Place the swab directly into the specimen envelope. The subject should be watching as you do this. 15. Take another swab, and repeat the procedure on the client s other cheek. Obtain a total of four swabs on each individual. As much as possible try to apply the four swabs to different areas of the mouth. 16. Make sure that the desiccant packets are in the specimen envelope with the specimens. The desiccant absorbs moisture to prevent bacteria from growing and it is VERY IMPORTANT to make sure that desiccant remains in the specimen envelope. Then fold down the flap of the specimen envelope and press it against the envelope to seal it. 17. Both the sample collector and the client must initial the spaces provided on the front of the specimen envelope witnessing that the client s sample was placed in the specimen envelope which contained the client s name. For minor children s specimens, the legal guardian must initial the specimen envelope. 18. Place the specimen envelope in the rear, sealable pouch of the clear plastic biohazard bag. 19. As you complete the specimen collection on each client, the client must sign the Specimen Collection Form. First have the client read the statement above the signature line. The legal guardian must sign the form for minor children. 20. In the presence of the tested parties, seal the specimen envelope(s) in the longer pouch at the top of the clear plastic biohazard bag by pulling off the adhesive liner at the top of the opening, fold over and press to seal. If you are collecting specimens for the same case at different times, use a separate plastic specimen bag for each collection, and staple them together. Completing Case Processing 21. The sample collector must sign the Specimen Collection Form in the space provided. 22. Fold the Specimen Collection Form in quarters, and place it in the short front pouch of the biohazard bag, along with any other paperwork, and any payment from the client to Paternity Testing Corporation.
