Hello, my name is. I work with the RQIA. RQIA stands for Regulation & Quality Improvement Authority. RQIA visit Day Centres. This is called an inspection. During the visit I talk to service users, staff and families and carers. I ask them to tell me about your centre. Then I write a report about the centre for RQIA. Anyone can read the report.

I want to know what you think about your centre. It helps me to write the report. Can you help me by answering some questions about your centre? You can fill it in yourself or ask for help to fill it in. You could ask staff, a friend or family member to help. names RQIA don t have your name or address. I gave this questionnaire to the manager to ask you to fill it in. You can give the questionnaire back to me during the inspection. Or you can put it in the envelope and post it to me. You need to post it before and you don t need a stamp. I might put the information you told us in my report. I won t ever use anyone s name in the report. I might tell the manager of the centre what you have told me. This is so they know what works well in your centre or what needs to be better.

What you think about your Centre? Safe Does your care protect you from harm? Safe Do you feel safe when you are at the Centre? Are the rooms in the Centre good for doing your chosen activities? Is care compassionate is your care given kindly with dignity and respect? Are staff kind, thoughtful and caring to you? Do staff make sure you have privacy? Do staff ask you about what is important to you? Would you like to tell us anything else?
