MOUNT ROGERS COMMUNITY SERVICES BOARD LOCAL HUMAN RIGHTS COMMITTEE MINUTES May 5, 2015 The Mount Rogers Local Human Rights Committee met on Tuesday, May 5, 2015 in the E. W. Cline, Jr. Building in Wytheville, Virginia. CALL TO ORDER Ms. Judy Burkett, Chairperson, called the meeting to order. II. INTRODUCTIONS Each individual gave an introduction and the organization affiliation III. PUBLIC COMMENT No public comments were presented at the Mount Rogers Local Human Rights Committee meeting. 1

APPROVAL OF THE AGENDA Ms. Wendy Gullion requested to modify the agenda as follows: Add LHRC Reappointment Application; remove Closed Session and to place Group Homes: Review of Video Monitoring Policy on the agenda for the August 4, 2015 LHRC meeting. MOTION: Ms. Danita Moore moved to accept the modifications to the agenda, as presented. The motion was seconded by Ms. Pat McClane, which passed V. APPROVAL OF FEBRUARY 3, 2015 MINUTES Ms. Judy Burkett requested two revisions to the February 3, 2015 minutes. The motions in Closed Session should reflect that Ms. Mary Coulson seconded the motions. MOTION: Ms. Mary Coulson moved to accept the revisions to the February 3, 2015 minutes as presented. The motion was seconded by Ms. Danita Moore, which passed VI. REPORT FROM REGIONAL ADVOCATE In the absence of the Regional Advocate, there was no report. VII. EXTENDED AFFILIATIONS A. Mount Rogers Community Services Board Youth and Family Services Ms. Heather Trail presented a request to extend the current affiliation with the Mount Rogers Local Human Rights Committee to add two locations for Therapeutic Day Treatment Services in Smyth County at the following locations: o Marion Senior High School, 848 Stage Street, Marion, Virginia 24354 o Chilhowie Senior High School, 500 East Lee Highway, POB 2280, Chilhowie, VA 24319 o Northwood High School, 305 Panther Lane, Saltville, VA 24370 MOTION: Ms. Chris Bird moved to accept the request for extended affiliation for the additional locations for Therapeutic Day Treatment, as presented. Ms. Mary Coulson seconded the motion, which passed Sponsored Residential Homes- Addition of Sites Mr. John Lovill presented a request to extend the current affiliation with the Mount Rogers 2
Local Human Rights Committee to add five Sponsored Residential Home Provider locations, once the licenses have been granted by the Licensing Specialist. o Jessica Johnson, 1929 Sylvatus Highway, Hillsville, VA 24343 o Kimberly Trimble, 2010 Lime Kiln Road, Fries, VA 24330 o Heather Hylton, 171 Gunview Road, Fancy Gap, VA 24328 o Rolvin and Cindy Denton, 2746 Pattons Mill Lane, Galax, VA 24333 o William Carico, 61 Rock Hill Lane, Fries, VA 24330 MOTION: Ms. Danita Moore moved to accept the request to extend affiliation for Mount Rogers Community Services Board for six Sponsored Residential Home Providers, as presented. Ms. Pat McClane seconded the motion, which passed ID Addition of Office Space Mr. John Lovill presented a request to extend the current affiliation for additional office space to provide Sponsored Residential Services for children ages 6 to 17 once the license has been granted by the Licensing Specialist. o CEC Woodlawn Office, 3545 Carrollton Pike, Woodlawn, VA 24381 MOTION: Ms. Danita Moore moved to accept the request to extend affiliation for Mount Rogers Community Services Board for the additional office space, as presented. The motion was seconded by Ms. Pat McClane, which passed ID and MH Crisis Stabilization Service Modification - Mr. Shane Ashby presented a request to seek affiliation with the Mount Rogers Local Human Rights Committee in preparation for mobile Intellectual Disability and Mental Health Crisis Stabilization Services, as presented. MOTION: Ms. Mary Coulson moved to accept the request of affiliation for Mount Rogers Community Services Board with the Mount Rogers Local Human Rights Committee, as presented. Ms. Chris Bird seconded the motion, which passed B. Twin County Regional Behavioral Health Policy Review Ms. Michelle Nester presented the Twin County Regional Behavioral Health Policy Review (Restraints). The policy content has not been changed; however, the restraint seclusion room has been removed from the behavioral health unit as the seclusion room is no longer used. Minor 3
language changes were made to several definitions in the policy (page 2) to be the same as the Virginia regulations. MOTION: Ms. Jennifer Haga moved to approve the Twin County Regional Behavioral Health Policy Review pending discussion and review of the policy with the regional and local advocates. The motion was seconded by Ms. Danita Moore, which passed C. Felts Supports for Living Addition of Site Mr. Chris Felts presented a request to extend the current affiliation with Mount Rogers Local Human Rights Committee for one additional Sponsored Residential Home Provider in Carroll County at the following location: o Arnold and Cheryl Grimm, 2450 Hebron Road, Galax, VA 24333 MOTION: Ms. Pat McClane moved to grant the request for extended affiliation for the additional site location in Carroll County by the Felts Supports for Living, as presented. Ms. Danita Moore seconded the motion, which passed VIII. AFFILIATION CONCLUSION A. Felts Supports for Living, LLC Mr. Chris Felts presented a notification to conclude affiliation with the Mount Rogers Local Human Right Committee for the following Sponsored Residential Home Provider: o Kimberly Nichols, 577 Hickory Lane, Galax, VA 24333 MOTION: Ms. Mary Coulson moved to accept the notification for Felts Supports for Living, LLC to conclude affiliation with the Mount Rogers Local Human Rights Committee, as presented. Ms. Chris Bird seconded the motion, which passed B. Mount Rogers Community Services Board Mr. John Lovill presented a notification to conclude affiliation with the Mount Rogers Local Human Rights Committee for the following Sponsored Residential home effective April 19, 2015: o Jane Choate Home, 8022 Pipers Gap Road, Galax, VA 24333 effective April 19, 2015. MOTION: Ms. Mary Coulson moved to accept the notification for Mount Rogers Community Services Board to conclude 4
affiliation with the Mount Rogers Local Human Rights Committee, as presented. The motion was seconded by Ms. Pat McClane, which passed IX. 1st QUARTER REPORTS AND 2014 ANNUAL REPORTS A. Life Center of Galax There was no representative from the Life Center of Galax present. B. Positive Options in Living There was no representative from Positive Options in Living present. C. New River Valley Treatment Center There was no representative from the New River Treatment Center present. MOTION: Ms. Jennifer Haga moved to table the 1 st Quarter Reports and 2014 Annual Reports until the August 4, 2015 LHRC meeting. The motion was seconded by Ms. Danita Moore, which passed D. Mount Rogers Community Services Board Ms. Wendy Gullion presented the 1st Quarter Report for Mount Rogers Community Services Board. E. Twin County Regional Hospital Ms. Michelle Nester presented the 1st Quarter Report for the Twin County Regional Hospital. F. Felts Supports for Living, LLC - Mr. Chris Felts presented the 1 st Quarter Report for Felts Supports for Living, LLC. MOTION: Ms. Mary Coulson moved to accept the 1 st Quarter Reports, as presented. The motion was seconded by Ms. Chris Bird, which passed X. LHRC REAPPOINTMENT APPLICATION Wendy Gullion invited Ms. Judy Burkett to complete the reappointment application. Ms. Gullion will forward the completed application to Ms. Nan Neese who will present the application to the State Human Rights Committee for reappointment. MOTION: Ms. Pat McClane moved that Ms. Judy Burkett seek reappointment to the Local Human Rights Committee. Ms. Mary Coulson seconded the motion, which passed 5
