The open and international standard Integrating the Healthcare Enterprise (IHE) is the right approach if you plan to establish a sustainable Health Information Exchange which is not limited by any boundaries. With its standards-based profiling of interoperability use-cases and vendor-neutrality it s the most promising approach to achieve sustainable Healthcare Connectivity. Large scale ehealth infrastructures have been already implemented in several countries as ground work and the most basic use-cases are already operational, leveraging the advantages of IHE. But the next generation of use-cases are already ahead, waiting to be implemented using new technologies like FHIR and blockchain. Today's challenge is not to do it - but to do it cost-effective and sustainable! This session summarizes the experiences of the national Health Information Exchange in Austria (ELGA) which successfully set up an IHE based ehealth Infrastructure and explains their roadmap to tackle new use-

cases and technologies. Learn about the current state-of-the-art blueprints and architectures for meaningful, sustainable and large-scale HIE to understand where the different standards and buzzwords (IHE, XDS, FHIR, Blockchain,...) have their place in the big picture, including the new project "Gemini", a Joint Venture of IHE and HL7 to Advance Use of FHIR for Interoperability. 14:30-15:00 Session CT2 - Driving Digital Transformation in Healthcare Delivery Amidst a Fragmented Health IT Landscape George Conklin, Senior Vice President & Chief Information Officer, CHRISTUS Health, USA Varun Anand, Co-Founder, MphRx, USA John McDaniel, Executive Vice President, Innovation & Technology Solutions, The HCI Group, USA Session CP2 - Reserved for Industry 14:00-15:00 Health 2.0 Showcase Panel: Chronic Disease Management Moderator: Karolina Korth, Founder, Health 2.0 Chapter - Kuala Lumpur, Malaysia The population is Asia Pacific (and particularly in Asian countries) is fast on track to becoming one of the oldest in the next few decades. Digital health technologies offer a promising role as an enabler in the delivery of evidence based care provision, however, in primary care, there are several challenges around leveraging digital health tools by healthcare providers, as health related information are collected, stored and used in compartmentalised, specialty focused health delivery models. The presentation aims to showcase an example an example of a digital health ecosystem that puts the consumer in the centre of healthcare delivery and this model would be a novel approach towards the WHOs approach on Integrated People Centred Healthcare Delivery Model aimed at achieving Health For All. Session CP4 - Consumer Innovations well and providing groundbreaking results that will advance healthcare now and in the future.
