Please use these notes for guidance. To protect confidentiality of the information you supply on this questionnaire, please complete the section requiring your details (Q1-14) first, then obtain verification of your identity, (Q15). It is IMPORTANT that the person verifying your identity completes this section before you complete the remainder of the questionnaire. The answers you provide on this questionnaire will be checked against police records and other public records to ensure your suitability to work on Devon & Cornwall Police premises. Should vetting clearance not be granted, you will be advised as soon as possible. Your vetting clearance will last for 3 years from the date of clearance. Please complete the questionnaire clearly and carefully. Check that you have completed ALL the questions by giving an answer in the space provided. Where a question is not applicable or the answer is not known, enter N/A or N/K respectively. Should you require extra space to answer questions, please use a continuation sheet, attach it and return with the questionnaire. You must inform family members and co-residents that their details have been submitted as part of the Vetting Process. The declaration at the end of the vetting form which you are required to sign includes the fact that in some cases a check will be made on your personal finances. This will not be done in the majority of cases for this type of vetting clearance, but is necessary for any role where you are employed as a temporary member of staff or where you will be given access to certain force IT systems. If required the check is merely to ensure that your financial position does not represent a potential vulnerability to corruption. The normal type of debt which many people find themselves in should not affect your vetting clearance although County Court Judgements (CCJ s) most probably will. If you have any concerns about the financial check, should it apply to you, please contact the vetting unit to confidentially discuss the matter. List all first names, as given on birth certificate. Include any names you and your partner are/have been known by.
