This important survey asks thousands of people about the activities they take part in. The Active Lives Survey is run by three government agencies. We work with a company called Ipsos MORI, which does a lot of surveys with young people and their parents. Your parent has filled in a similar questionnaire and they have said it is okay for you to take part, but it s up to you to decide whether you want to or not. We would like you to fill in a short questionnaire on the Internet. The survey will ask you about your hobbies and the activities you do at school and in your free time. We will also ask some questions about you and your lifestyle. Why take part in this survey? 99 It s helpful your answers will help us to make decisions about cultural events, sports and other facilities for young people in [geography]. 99 It s quick it should only take you about 15 minutes. 99 We will send you a 5 gift voucher when you have completed the questionnaire to say thank you. You can use the voucher at lots of different retailers covering music, fashion, restaurants and entertainment.
