Gordon, Deputy Secretary Office of Medical Assistance Programs PURPOSE: The purpose of this Medical Assistance (MA) Bulletin (bulletin) is to announce to providers that the Pennsylvania Department of Public Welfare (Department) is updating procedures for the reporting of National Identifiers (NPI) when submitting Medicaid health care claims as mandated by the Patient Protection and Affordable Care Act (ACA) of 2010. SCOPE: This bulletin applies to all providers enrolled in the MA Program who render services to recipients enrolled in the MA Fee-for-Service (FFS) delivery system. s rendering services to recipients in the MA managed care delivery system should address any billing questions to the appropriate managed care organization. BACKGROUND: As a result of the Health Insurance Portability and Accountability Act of 1996 (HIPAA), the federal Department of Health and Human Services (HHS) mandated the use of the National Identifier (NPI) number, which is a standard unique identifier for health care providers. The federal mandate required that as of May 23, 2008, all HIPAA-covered health care providers, whether they are individuals or organizations, must obtain an NPI number for use to identify themselves in HIPAA standard compliant transactions. During the initial implementation of NPI, the Department published a number of bulletins regarding the use of NPIs when submitting Medicaid health care claims, including: - MAB 99-05-13, titled Announcement of the National Identifier Number, stated the Department s intent to implement the use of the NPI number. - MAB 99-06-14, titled Instructions For Registering Your National Identifier (NPI) Number to the Department of Public Welfare (DPW), provided information on how to obtain an NPI number and how to register it with the Department, along with COMMENTS AND QUESTIONS REGARDING THIS BULLETIN SHOULD BE DIRECTED TO: The appropriate toll free number for your provider type Visit the Office of Medical Assistance Programs Web site at http://www.dpw.state.pa.us/provider/healthcaremedicalassistance/index.htm

- 2 - recommendations related to obtaining NPI numbers for organizations with multiple locations or subparts. - MAB 99-07-07, titled Instructions For Using Your National Identifier (NPI) Number to Bill the Department of Public Welfare (DPW), provided information on how to properly report a provider NPI on different health care claim submission media. DISCUSSION: The federal final rule Changes in and Supplier Enrollment, Ordering and Referring, and Documentation Requirements; and Changes in Agreements issued on April 27, 2012, implements several provisions set forth in the ACA. Most notable of these provisions is that all providers of medical or other items or services and suppliers under the titles XVIII and XIX programs who qualify for an NPI must report their NPI on enrollment applications and on all claims for payment including paper and internet claims. This differs from the NPI final rule which required NPIs to only be reported on HIPAA covered electronic claims. Beginning with claims received on January 1, 2013, the Department will require the following updates for properly reporting NPIs on paper health care claim forms, through the PROMISe Internet Portal, and on X12 version v5010 837 electronic claim transactions. In order to ensure no disruption in claims payment, providers must have an NPI registered on all billing and rendering provider service locations submitted on FFS claims. Additionally, providers will need to report the NPI for all referring and ancillary providers as required by MA billing instructions. Below are the updates the Department is making to the various claim submission types. Paper Claims: The Department currently accepts the following health care paper claim forms listed below. Beginning with claims received on January 1, 2013, the submission of an NPI will be required in certain fields. CMS-1500 The following fields are to be reported as indicated: o Field 17b NPI of Referring (if applicable*) o Field 24J (b) NPI of Rendering (if applicable*) o Field 32a NPI of Service Facility (if applicable*) o Field 33a NPI of Billing (required) UB-04 The following fields are to be reported as indicated: o Field 56 NPI of Billing (required) o Field 76 NPI of Attending Physician (required) o Field 77 NPI of Operating Physician (if applicable*) o Field 78 & 79 NPI of Other Physician (if applicable*) ADA 2006 The following fields are to be reported as indicated:

Billing Guides can be found at the following link: http://www.dpw.state.pa.us/publications/forproviders/promiseproviderhandbooksandbillingguides/index.htm Internet Claims: s who are registered with the Department can use the PROMISe Internet Portal to perform a number of functions and transactions including submitting health care claims. Beginning with claims received on January 1, 2013, the following reporting and processing changes will be in effect for certain provider fields. Billing providers who submit health care claims using the PROMISe Internet Portal will continue to log into the portal using their 13-digit Medicaid Identification Number (MAID#). When the billing provider attempts to submit a health care claim, the PROMISe Internet Portal will check to see if the billing provider has an NPI that is registered with the Department. If the billing provider s NPI is registered with the Department, the claim submission option will be available. If the provider does not have an NPI that is registered with the Department, the provider will not be able to submit the health care claim. The provider will receive a message with instructions on how to obtain an NPI and register it with the Department. Rendering providers 13 digit MAID#s and NPIs are required to be registered with the Department and are to be reported on all professional health care claims. If the billing provider does not record the rendering provider s NPI, the billing provider will not be able to submit the claim and a message will be displayed stating that it is missing. If the rendering provider s NPI is not registered with the Department, the billing provider will receive a message explaining that the NPI is not registered. If an Ancillary provider s (referring, ordering, attending, operating, facility or other) MAID# or medical license number is being reported on health care claims via the PROMISe Internet Portal, the billing provider will now also be required to enter the ancillary provider s NPI in the appropriate provider NPI field. (Please note, at this time it is not necessary for Ancillary provider NPI information to be registered with the Department.) Atypical providers are defined as providers who do not provide health care services. Taxi services, home and vehicle modifications, and respite services are examples of non-healthcare or atypical services. Atypical providers may submit HIPAA transactions, but since they do not meet the HIPAA definition of a health care provider, they do not receive an NPI number. More information regarding atypical providers can be found in MAB 99-07-07 titled Instructions For Using Your National Identifier (NPI) Number to Bill the Department of Public Welfare (DPW).
