AVO Beri nombor CIC kepada klien lama atau dapatkan nombor CIC kepada klien baru. 7.4 Kes baharu? Sekiranya Ya, ikut langkah 7.5 Sekiranya Tidak, ikut langkah 7.6 7.5 Beri nombor kes. Dapatkan nombor kes berdasarkan aturan nombor terkini. Seterusnya ikut langkah 7.23. 7.6 Dapatkan sejarah rawatan. Dapatkan sejarah rawatan dari borang PMR- Patient Medical Record (OPR/FPV/BR004_LAP) bagi mengemukan nombor rujukan kes. AVO Seterusnya ikut langkah 7.23 7.7 Klien baharu? Jika Ya, ikut langkah 7.12 Jika Tidak, ikut langkah 7.15

Dapatkan nombor kes lama haiwan dari CIC klien lama Daftar dalam CLB di bawah nombor kes lama haiwan tetapi di bawah klien baharu menggunakan dakwat merah. 7.10 Pindahkan maklumat haiwan. Pindahkan maklumat haiwan lama ke CIC baharu untuk klien baharu dan masukkan CIC ke CICC. 7.11 Dapatkan PF. Perolehi PF dan tukar maklumat klien. Seterusnya ikut langkah 7.23. 7.12 Haiwan baharu? Jika Ya, ikut langkah 7.13 Jika Tidak, ikut langkah 7.8 7.13 Dapatkan kes. Daftar nombor kes baharu bersiri dalam CLB menggunakan dakwat biru/hitam. (Rujuk: UVH Clinical Practice Manual, FPV/UVH/T001). 7.14 Masukkan data ke dalam program Maklumat Sistem Pengkalan. Sediakan CIC dan masukkan ke CICC. Seterusnya ikut langkah 7.22.

Daftar di bawah nombor kes baharu bersiri dalam CLB. 7.18 Kemaskini KP. Kemaskini CIC jika perlu dan kembalikan ke CICC. Seterusnya ikut langkah 7.22. 7.19 Dapatkan nombor kes. Perolehi nombor kes lama haiwan dari CIC. 7.20 Kemaskini rekod. Kemaskini CIC jika perlu dan kembalikan ke CICC. 7.21 Perolehi PF. Dapatkan fail pesakit dari bilik rekod. 7.22 Sedia rekod. Sediakan PMS dan PF baru. 7.23 PMR baharu. Sediakan PMR baharu untuk lawatan kini.
