Introduction For the CATT Follow-up Study, digital retinal images will be submitted online through a web browser. Only.jpg images may be submitted online. Color photographs and the fluorescein angiogram for a study patient should be stored in a directory that identifies the images by study id, alpha code, and visit date. Only photographers who are registered on the CATT Follow-up Study Certification Website may submit images. The certification website address is: http://www.cpob.org/studies/cattfup/certification/. IMPORTANT NOTE: Before images may be submitted for photographer certification or for the study, the cameras used for imaging must first be certified. A copy of this form is available on page 14 of this guide. The form is also accessible from the CATT Landing Page, http://rt5.cceb.med.upenn.edu/public/catt_fus_byname_crf.html. Click on the link Digital Retinal Images Web Submission Application. The image submission is also accessible from the photographer certification checklist.. 1
Login Screen For the CATT Scheie Image Reading Center Web Submission Application, the Username will be the same as the CATT Follow-up Certification Website Username. The Password will be the photographer s CATT Follow-up Certification Number, which is different from the certication number originally used in CATT. If you do not know or remember your certification number, it can be found by logging into your account on the CATT FS certification website. If you need assistance with your Username or password, please contact Revell Martin (ermartin@upenn.edu) or Candace Parker Ostroff. (cparker@upenn.edu). To log in, type in your Username and password and click on Submit. Application Links After successfully logging in, a selection of links will appear on the left side of the screen. These include Home, Load Image, Show All Images, and Logout. Clicking on Home will return to the below screen. 2
Loading Image To upload images to the Scheie Image Reading Center, click on Load Image. A data entry screen will appear. The following are instructions by field for completing the required data entry. Clinic o Use the pull down list to select the CATT clinic. Site o Use the pull down list to select the CATT site (1, 2, or 3). Patient ID o Enter the CATT study id without a dash. This will be a 5 digit code. This id is available from the CATT FS Clinic Coordinator. o Alpha Code o Enter the CATT study alpha code. This will be 4 alphabetic characters. This alpha code will be checked by the system to validate the Patient ID. This alpha code is available from the Study Clinic Coordinator. Color photographs o When submitting color photographs, click on Yes for question 1 and complete the color photographer certification number and date the color photographs were taken. The slashes must be typed in between the 2 digit codes for month and day and between the 2 digit code for day and the 4 digit code for year (i.e., mm/dd/yyyy). o If the color photographs are not being submitted, click on No for question 1 and skip the color photographer certification number and date of color photographs. Fluorescein angiogram (FA) o When submitting a fluorescein angiogram, click on Yes for question 2 and complete the FA photographer certification number and date the FA was taken. The slashes must be typed in between the 2 digit codes for month and day and between the 2 digit code for day and the 4 digit code for year. o If a fluorescein angiogram is not being submitted, click on No for question 2 and skip the FA photographer certification number and date of FA. 3
Below is an example of a completed data entry screen. Selecting files for submittal Click on the Choose Files button. This will bring up a screen that resembles a Windows Explorer screen. 4
Navigate to the file folder where the CATT patient s color photographs and FA are located.. Select all the images that are to be submitted and click on the open button. Scroll down and you will see the list of images that were selected. 5
Click on the upload button and wait until the upload process is complete. In the lower left corner will be an indicator of % complete while uploading. If the upload is successful, a list of the images uploaded will be presented on the screen. 6
After the upload you will be able to submit another CATT patient s images by completing the data entry screen, selecting images, and uploading. Pressing the home link will return you to the main screen. From this screen you may select Load Image, Show All Images, or Logout. 7
Show all images If you click on the link Show All Images, this will take you to a page that will list all images submitted by your clinic to the Scheie Image Reading Center. You may scroll to find a set of images submitted. In the example below, the black lines are server folder names; the blue lines are clickable links to individual images. 8
When you click on one of the image s links, this will allow the image to be browsed. A click on the web browser back button will return to the image set list. 9
Error Message Examples If there is an error during the upload process, there may be a pop-up message. Click on OK and correct the error before attempting to upload the image set. Below are examples of some of the error messages. Clinic and Site Missing Invalid combination of ID and Alpha Code 10

Invalid File Type/Image Exceeds Size Limit Only.jpg files may be uploaded. Validation Post Submission After images have been submitted to the Scheie Image Reading Center, the data recorded in the online form and the images will undergo further validation and review. If there are any discrepancies in the submission, the Reading Center will contact the photographer. In some cases, a resubmittal of images may be required. Please retain study images so that these may be retrieved by the study id and remember to back up the study images frequently. 13
