ASBESTOS PROGRAM PART 1 INITIAL MEDICAL QUESTIONNAIRE This mandatory form contains the medical questionnaire that must be administered to personnel who are exposed to asbestos above the permissible exposure limit, and who will therefore be included in the UW medical surveillance program for asbestos. Part 1 is the Initial Medical Questionnaire, which must be obtained from all new hires who will be covered by the medical surveillance requirements (Wyoming OSHA 1926.1101).
