You may use this form to make an amendment or surrender a UK hip Radio Licence or a UK hips Portable Radio Licence. A separate form should be used for each type of licence. Only one licence can be applied for on each form. Please refer to the hip Radio guidance notes Of168a when filling in this form. Page 1

Application guidance Conditions Under the Wireless Telegraphy Act 2006, you must have a licence issued by Ofcom before you establish or use any Business Radio equipment. Registering Please note that Business Radio Licence applicants are encouraged to register and apply for and print their radio licences via the Ofcom website www.ofcom.org.uk. Communication preference Where possible, we send correspondence to licensees electronically. This is quicker for you and cheaper for us. By applying online and providing your email address, we assume that you wish to receive correspondence and notifications relating to Wireless Telegraphy Act licences electronically and not on paper. If you do not wish to receive documents electronically, you must opt out by ticking the Letter box. If you do not provide a telephone number and email, it may delay the processing of this form. Important Maritime radio transmitting radio equipment may be operated only or under the direct supervision of the holder of the appropriate Maritime Radio Operator s Certificate or Competence and Authority to Operate. Details of these are available from the Maritime and Coastguard Agency (MCA). The radio equipment is covered by this licence must meet certain performance requirements as described in ection 4 of the guidance notes. Payment A fee of 20 will be charged for each hip Radio Licence and each hip Portable Licence. Registering and applying online using the Amateur Radio and hips Radio Licensing portal at www. ofcom.org.uk is completely free of charge. Payments must include all bank charges. If Ofcom does not receive full payment, it will return any partial payments received. Data Protection Act: The information you provide in this form will be collected, held and handled in accordance with our Data Protection tatement. This is published on our website at: https://www.ofcom.org.uk/about-ofcom/foi-dp/dataprotection The information held by Ofcom will be updated as appropriate by relevant numbering records provided by Inmarsat, Accounting Authority Identification Code records by the (Maritime Radio) Accounting Authorities and EPIRP/PLB records provided by the MCA. The information is provided by Ofcom to the International Telecommunications Union (ITU) and the Maritime and Coastguard Agency (MCA) to assist in earch and Rescue operations and to help ensure that the information held by the ITU and MCA is accurate and up to date. This information may also be provided by Ofcom to law enforcement agencies (such as the police) for crime and taxation purposes as well as the Maritime Administrations MCA Channel Islands and the Isle of Man. Hand held VHF radiotelephones If you do not supply an MMI for a hand-held VHF radiotelephone (e.g. because it is new or you do not use DC) we shall issue one. You must ensure that the MMI used in the apparatus is the one that we assign on the licence. MOB and AI If you indicate that you will be carrying an MOB DC, MOB AI, AI AR Transmitter or EPIRB AI but fail to provide an MMI for each item carried, we shall contact you for the missing MMI(s).
