Purchase gift cards to give as gifts, or buy cards for your favorite retailers and use them to purchase gifts, holiday supplies, etc. Completed Order Forms and payment (check or cash) Due to Melissa Medina, FUUCBC Office Administrator OR to Terri Bridgwater NO LATER THAN 3 pm, Monday, November 28, 2016. NOTE: Checks should be made payable to FUUCBC. Your Order will be available for pick-up after our service until 12:30 pm on Sunday, December 11, 2016 and again on Sunday, December 18, 2016. If you are unable to pick up on one of those dates, please contact Terri or Melissa and we will make arrangements to get your order to you. The order form is organized by retailer category. The name of the retailer, the available card denominations, and the percentage FUUCBC receives are listed.
