Contractor must assume that measurements contained in this request are approximations; it is the contractor s responsibility to assure accurate measurements when quoting. As of January 1, 2010, contractors/maintenance workers performing renovation, repair and painting projects that disturb lead-based paint in homes, child care facilities, and schools, built before 1978 must be certified and must follow specific work practices to prevent lead contamination in most situations. Firms performing renovation, repair and painting projects for compensation that disturb lead-based paint in pre-1978 homes, child care facilities and schools must be certified by the EPA and must use certified renovators who are trained by EPAapproved training providers to follow lead-safe work practices. Contractor will be responsible for the cost of any retesting of failed Clearance test(s) and will be responsible for any additional labor and material costs until a passed Clearance test is obtained. Due to program funding restraints and requirements we reserve the right to remove less priority Item numbers to achieve the maximum allowable work to be performed. Not looking to level, minimal jacking as needed, to add additional support and stabilization. Insure existing 2x8 floor joist are properly fastened to center girder. Install 1 drop beam spanning approximately 6 floor joists with proper sized beam, placed approximately 3 from exterior wall. Install cement footers 18 x18 x12 deep, for each jack post spaced no wider then 6. Repair/replace up to 3 floor joist as needed. Repair floor at bedroom door. Install 5.0MM 4'x8' utility PR (luan). Fill all joints and fasteners divots to insure smooth continuous flooring. Install new linoleum (Vinyl Sheet Flooring:.125 mm). Install new transition strip at door locations. Install new base moulding and base shoe molding as needed painted to match existing. Remove and install new shelve under sink. 1

Do not remove tub Remove and dispose of ALL linoleum and ALL deteriorated wood flooring. Install floor joist blocking as needed (the same sized dimensional lumber as floor joist) to provide a fastening surface for sub floor patch. Insure patch is the same height as the surrounding floor. Install 5.0MM 4'x8' utility PR (luan). Fill all joints and fasteners divots to insure smooth continuous flooring. Install new linoleum (Vinyl Sheet Flooring: a minimum of 2MM thick). Insure water tight seal around tub. Install new transition strip under door. Install new base moulding and base shoe molding as needed painted to match existing. Windows must be double hung, half screen, E-star rated, gas filled. Replace rotted exterior trim and sill on windows as needed.
