In order to formulate additional polygraph questions which you can comfortably answer, it is necessary to know a great deal about your background. This questionnaire will assist the examiner in formulating questions for your test. Some of the polygraph questions you will be asked are enclosed in this worksheet. It is important that you discuss and clarify any questions you may have with the polygraph examiner prior to the polygraph examination. You will be given that opportunity. 2
