2 All Applications must be submitted with the items listed below. Please CHECK all items submitted. Then sign and date this Form. Pages 1 and 2 of the Home Improvement Application Neighbor Notification Form: Obtain the names, addresses, signatures and contact information (optional) of affected neighbors acknowledging they received notice of the improvement. Owners must make an attempt to obtain signatures from owners of adjacent homes that are unoccupied or rented out to meet submittal requirements. Please note neighbor awareness does not constitute neighbor approval or disapproval. The intent of the Neighbor Notification Form serves to make neighbors aware of the proposed improvement. Neighbor Location Diagram Form: Use this diagram to determine which neighbors need to be notified. Fill in the corresponding lot letters using the Neighbor Notification Form. Color Photo of Home (showing entire section to be modified such as front yard, front of home, backyard, etc.) Color photos showing examples of all materials to be used (plant materials, ground cover, roofing, planter border, rocks, fountains, plant pots, etc.) and/or finished project examples (walkway, lattice, etc.) House Paint Information (Paint Applications Only): Indicate which colors will be used on the home by filling in the color names next to the corresponding sections (i.e. stucco, trim, shutters, etc.). Actual paint samples (no smaller than 3 x 5 ) must be included. Concrete Setback Form (Concrete Additions Only): If submitting an application for concrete additions including walkways, patios and driveway expansions, fill in the required dimensions of your lot noted on the form. Plans: Detailed plot plan drawn to scale and/or CAD drawing showing location, size, dimensions, setbacks, identified scale, and description of proposed improvements. Plans and specifications for improvements must be prepared with sufficient clarity and completeness to enable the ACC to make an informed decision on the request. Said plans shall include a plot plan drawn to scale showing the following: All proposed Improvements and relevant elevations, together with the desired location of such Improvements as they relate to the Residence and adjacent fence/lot lines (setbacks). Complete dimensions of the proposed Improvements. Drainage plans (if applicable) where the established drainage pattern might be altered by the proposed Improvement. If drainage is to be connected to the existing system as built, a clear notation must be made on the plans. Any plans larger than 11 x 17 must be accompanied by a digital copy. Application Condition for Approval It is the Owner s responsibility to supply any and all details required, as stated in the Home Improvement Application. This gives the Architectural Control Committee (ACC) the needed information to determine compliance with the CC&Rs and the Architectural Control Guidelines regarding this proposed Home Improvement Application. The ACC reserves the right to request additional information when necessary. The Owner is responsible for the accuracy of the information supplied within this Home Improvement Application and its compliance with any and all CC&R and Architectural Guidelines. If the construction or installation of any home improvement item(s) is (are) deemed non-compliant to the CC&Rs or Architectural Guidelines after being constructed or installed, even with an approved Home Improve Application. The Owner will be required to correct the noncompliance or de-construct the non-compliant item. SIGNATURE DATE

3 Neighbor Notification Form Using the Neighborhood Location Form, please show and explain your plans to all affected neighbors. Please have them sign this form to verify they have been notified of your improvement. If surrounding properties are non-owner occupied, please contact the management office for owners contact information to notify them of proposed modification. By signing this form, your neighbors are NOT approving your proposed improvement, but rather acknowledging they have been informed of the proposed project. This form is just a notification tool. 5 House Paint Information Please fill in paint color (name and number) in the below diagram Items not mentioned (e.g., Roof Vents, Electrical Panel, Railings, Bannisters, Columns, Accessory Frames etc.) must be painted to blend with and or match the surrounding areas. Paint Brand Name Indicate which colors will be used on the home by filling in the color names next to the corresponding sections (i.e. stucco, trim, shutters, etc.). Actual paint samples (no smaller than 3 x 5 ) must be included. 1

6 Concrete Setback Form Please use this form for all front driveway extensions/concrete additions -- The below information is required and your application will be deemed incomplete if missing. Not all parcels are the same size, therefore the below information is necessary for the ACC to make an accurate decision on your request for a concrete extension/addition. - Fill in the questioned dimensions below to find your setback information. Please note any additional information on a separate sheet. - In addition to this form you will also need a detailed drawing, with dimensions, of all existing front concrete. You must include all concreted porch areas and walkways.

11 Notice of Completion Form This form needs to be submitted no more than 10 days following completion of the improvement. Per the Natomas Park Association Architectural Control Guidelines, owners shall notify the Association that the improvement has been completed no more than 10 days following completion. This form must be used to notify the association that the improvement listed below has been completed as approved.
