What does information about me mean? Anything that is about you. For example, your photo, or talking about you, or things that have been written about you. Page 3 of 21
House with No Steps must keep information about you private. We have to ask you before we share information about you. Sharing information about you means showing something about you talking to someone about you. If you say yes it is called giving your consent. This toolkit is to make sure you are in control. What are the steps that staff must follow? Staff have to go through these steps with you. Someone else needs to watch you go through these steps. This person is called a witness. You can choose other people to help you decide if this is what you want. Page 5 of 21
Staff have to help you understand what information about you they want to share. For example: Your photo Your file Something you have done How you are feeling Page 6 of 21
Something that happened to you. Staff have to help you understand what they want to do with that information. For example: Tell the doctor Put it on a website Page 7 of 21
Tell Centrelink. Tell a House with No Steps staff member. They have to ask about each different thing they want to do with your information. For example: Can I tell the doctor your back is hurting? Page 8 of 21
Can I tell your mother your back is hurting? They cannot ask just one question about sharing your information with lots of different people or in lots of different ways. For example they cannot ask Can I talk to everyone in the government about you? Can I show your file to everyone? Page 9 of 21
Staff have to help you understand how sharing your information will help. For example: It will help the doctor give you the best care It will help more people to learn about House with No Steps It will help staff at House with No Steps know how to give you the best support. Page 10 of 21
Staff have to help you understand any risks or bad things that might happen if they share information about you. For example: More people will know about things that you might think are very private. When a photo is put on the internet other people can make their own copies of it to keep. Page 11 of 21
Your answer You can say yes or no or something else. You can change your mind at any time. We will respect your choice. Page 12 of 21
When does HWNS have to ask me again? 1 year. Tell us if you want us to ask you again before 1 year. Page 13 of 21
What if I have a guardian? Guardians can legally make some choices for you. Staff need to check which choices your guardian can make for you. Staff will always talk to you about your guardian and which choices they make for you. Page 14 of 21
