Locate Request Information NOTE: DO NOT USE any apostrophes ( )or quotation marks ( ) in Web Ticket Entry. Doing so may hamper the transmission of tickets to the utilities and locators. Limit per Ticket Road Frontage Locates: When digging in an easement or right-of-way, you must use road frontage rules. NOTE: These types of locate requests would be entered as a Street/Address ticket and the Address field would be left blank. NOTE: The instructions must start from a fixed point such as an intersection or address. If an address is used as a reference point, the Address field should be left blank and the address should be referenced in the Locate field. NOTE: When requesting a road frontage ticket, you must have very clear locate instructions stating where the locate begins, what direction you are going, how far it is, and on which side of the road. If you need a specific depth off the road into a field, you will need to state that information as well with the appropriate dimensions. NOTE: When writing your locate instructions in the Locate Where box, always use the word Locate to denote where the actual locate will begin (as opposed to travel directions and such). 1. Within City Limits/Subdivisions: When you are digging inside of city, town, or subdivision limits a request may have up to 1,500 linear feet on one or both sides of the road on one ticket, as long as it stays on the same street/road. You may not turn a corner. Example: Both sides of N East St between Ohio St and E Michigan St inside city limits 1,500 feet

2. Outside City Limits: When you are outside of cities, towns, and subdivisions a request may have up to a half (1/2) mile, which is 2,640 linear feet, on one or both sides of the road on one ticket as long as it stays on the same street/road. You may not turn a corner. So, if you need to dig along both sides of the road for a 1 mile distance, this would become two locate tickets, one for each half mile distance. Example: Both sides of N 900 E from E 200 N heading north for 1 mile outside city limits

NOTE: A ticket is limited to either 1,500 linear feet (inside city limits and subdivisions) or a ½ mile, which is 2,640 linear feet, (outside city limits) when placing street signs and/or utility poles. It should state on the ticket how many signs/poles in the area and the said sign/poles should be properly marked.

Locates for Specific Addresses or Lots: NOTE: These types of locate requests would be entered as a Street/Address ticket and the Address field would be left blank. All addresses or lots must be listed individually on the ticket in the Locate field. As many addresses or lots can be placed on one ticket as long as: 1. All addresses/lots are addressed to the same street 2. All addresses are within the road limitations: *1,500 feet inside the city limits and within subdivisions (whether inside or outside city limits) * A half (1/2) mile, which is 2,640 feet, outside the city limits and outside of subdivisions NOTE: Please state on what side of the road the addresses/lots are located. If they are on both sides of the road, state that as well. Examples: I. Locate the entire rear of the property at the addresses 101-103 - 105-107 - 109 111 113 115 117 Danbury St II. Locate the entire property of lots 23 through 38 on both sides of Apple St III. On Westfield Blvd Locate the south side of 2684 locate the north and east sides of 2685 locate the south and west sides of 2686 and locate the north and east sides of 2687 Westfield Blvd NOTE: Rural Route numbers do not go in the Address field; they should be entered in the Locate field. Add the resident name if known. Make sure to have driving instructions from an intersection to the property in the Locate field. Examples of multiple addresses or lots on one ticket: I. All mobile homes share the address of 1102 E Franklin Rd: This is the address for the Country Woods Estates Mobile Home Park Locate the entire property of lots 1 thru 37 on the north side of E Franklin Rd E Franklin Rd makes a loop around the entire mobile home park this is along the northern stretch of E Franklin Rd on the north side of the mobile home park

II. This is for all the addresses on both sides of Chase Blvd between Hunters Trl and Hunting Creek Blvd Locate the entire front of the properties III. This is Winchester Village Apartments Locate the front of buildings 2108 2110 and 2112 Winchester Dr

Corner Properties/Large Properties: A locate request must be for a single street/road (by name) and cannot turn a corner. If you need an entire property marked (whether it has an address or not) that is touching two different roads and you need the intersecting road located, you must submit two separate tickets. If the property is bordered by four roads and you need all four roads or right-of-ways located, then you will need to submit four tickets. Any work being done on the Near Street requires a second ticket. 1. Properties with an Address: The first ticket for the entire property would use the property address. Each ticket following would be a road frontage ticket starting from an intersection. Example: 6948 N Westfield Blvd on the corner of Page Blvd Ticket 1: Locate the entire property Ticket 2: From the above intersection locate both sides of the road heading east for approx 155 feet for the entire length of 6948 N Westfield Blvd

2. Properties without an Address: Both tickets would be a road frontage ticket starting from an intersection and stating a depth off the road. Example: Empty corner field in the country Ticket 1: From the above intersection locate the west side of the road heading north for 1500 feet extending west into the field for 1000 feet for the entire distance Ticket 2: From the above intersection locate the north side of the road heading west for 1000 feet extending north into the field for 1500 feet for the entire distance

Divided Highways/ Divided Roads: When working on a divided highway, each set of lanes would be considered a separate road. If you need a 1,500 foot distance on both sides of both sets of north and south bound lanes in city limits, this would be separated into 2 tickets. One for both sides of the northbound lanes for 1,500 feet, and a second for both sides of the southbound lanes for 1,500 feet. Outside the city limits the half mile limitation would be in place following the same guidelines. Example: Both sides of both I 65 lanes heading north from Raymond St for 1,500 feet inside city limits

Ramps: When working on ramps at an interchange, each ramp would be considered a separate road. If you need the entire interchange, this would be separated into multiple tickets: One (or more) for each ramp; staying within the allowed road frontage. Inside city limits, a request may have up to 1,500 linear feet on one or both sides of a ramp. Outside the city limits the ½ mile limitation would be in place following the same guidelines. As with other tickets along a road, the instructions should start at the intersection and give directions to the dig site. Example: All 4 outside ramps at the interchange of I-94 and US RT 421.

Ticket 2: FROM THE INTERSECTION TRAVEL EAST FOR 1400 FEET TO THE RAMP FROM NORTHBOUND US RT 421 TO EASTBOUND I 94 LOCATE BOTH SIDES OF THE ROAD HEADING SOUTHWEST FOR 2300 FEET EDING AT US RT 421 Ticket 3: FROM THE INTERSECTION TRAVEL WEST FOR 1650 FEET TO THE RAMP FROM EASTBOUND I 94 TO SOUTHBOUND US RT 421 LOCATE BOTH SIDES OF THE ROAD HEADING SOUTHEAST FOR 2500 FEET ENDING AT US RT 421

Ticket 4: FROM THE INTERSECTION TRAVEL WEST FOR 1500 FEET TO THE RAMP FROM SOUTHBOUND US RT 421 TO WESTBOUND I 94 LOCATE BOTH SIDES OF THE ROAD HEADING NORTHEAST FOR 2640 FEET ENDING AT US RT 421 Example: All 4 cloverleaf ramps at the interchange of I-94 and US RT 421.

Easements: When locating easements that are not along a road, the footage rules also apply. When digging in a rear easement between houses on two different streets you will only need to enter a request for one of the houses stating to locate the rear easement. If you are working on a cul de sac in the easement, you will need to enter the ticket according to the footage requirements. NOTE: While digging in the same continuous easement you can go from the starting point at one address to the ending point at another without listing each individual address/lot in between. NOTE: This would be entered as a Street/Address ticket leaving the Address field blank but still entering the road you are working on/along in the Street Name field and the nearest intersecting road listed second. In the Locate field, you will need to give the locate instructions. Examples: The rear easement behind multiple addresses this does not cover any private property

Alleys: If you are locating in an alley, you need to list the road that it is considered to be off of first, with the nearest intersecting street listed second. In the locate instructions give the information as to where the alley is in relation to the stated intersection, the point at which you are starting, what direction you are heading and the total distance. This must follow the same road frontage guidelines as stated prior. Example: Entire alley east of Caroline Ave between Stewart Ct and 118 th St

Intersections: When requesting an Intersection ticket, it means the excavation is at or between the four corners of the intersection, and you are staying within a 200 or 300 foot confine of the intersection (depending on if you are inside city limits/subdivisions or outside city limits.) NOTE: You must request a specific radius of the entire intersection OR a specific radius around a corner. NOTE: The Intersection selection is not to be used if the dig site is starting at the intersection and then heading down the road for 1,500 feet, or if it is starting at a point that is away from the intersection. All road frontage tickets would fall under the Street/Address selection as this covers both working along a street/road OR at a specific address. Choosing Intersection will automatically map a 200 foot dig site at the intersection. 1. Inside City Limits or Subdivisions: The maximum that can be requested at an intersection is a 200 foot radius. 2. Outside City Limits: The maximum that can be requested at an intersection is a 300 foot radius. Examples: I. Locate the entire intersection with a 200 foot radius II. Locate the northeast corner of the intersection with a 30 foot radius around the white flag

Railroad Tracks/Rivers/Ditches/Creeks, etc: Anything of this nature is bound by road frontage rules. The difference is that you do not have to have a ticket for each side. You may request both sides of one of the above mentioned areas for 1,500 linear feet in city limits/subdivisions or or ½ mile outside of city limits. Make sure to still include directions from a starting intersection. Example: 2,500 feet along both sides of Roberts Ditch outside city limits We do not take tickets for inside or beneath buildings (in crawl spaces). The following will not be accepted (No exceptions will be made): * See crew on site for details * Please respond A.S.A.P. * This is a Short Notice * Call to arrange a meeting on site * Locate entire subdivision/city block * This is the Second Notice or Joint Meet Request (a blanket locate request is not valid)

Long Term or Long Distance Projects If you are working on a large project that could take over 2 weeks or even months to complete, you should submit locates no more than a week in advance of when you will be doing the work. For example; you have a total of 50 jobs to do but your crew can only do 15 jobs in 2 weeks. Enter only 15 tickets to start with and create new requests to replace completed tickets when they are needed. Example: You are working on a 10 mile highway project and you have projected that you will advance 1 mile per week for the first 10 weeks. You would not submit tickets for the entire 10 miles at the beginning of the project; doing so creates unnecessary Remarks and Job Extensions and places a burden on Indiana 811 Members and their locators. A week before the project starts you would request locates for only the first 1 mile stretch of the highway were the work will begin. When you are a week away from starting the next stretch, you would submit locate requests for that area, and so on. This rule applies not only to road projects, but apartment complexes, subdivisions, etc. If you have a ticket that is about to expire, and you have not started excavation (and/or do not plan on starting work in the near future), please wait until a week before you are ready to start, before resubmitting a new ticket. Following this procedure will help eliminate unnecessary Remarks and Job Extensions. Mapping a Street/Address Ticket when there is no address to enter: When the map screen comes up, it should give you the entire road that you are working on as the dig site. If it does not have the road at all, or if it cannot find the road that you have entered, it will select the entire township as your dig site. In either case you will need to map these tickets manually and make the area smaller. 1) The easiest way to get to the intersection that you are starting at is to click on the Search Tool on the tool bar above the map. 2) When this search box opens, select the tab for Search Cross Street. This will then show the primary road information already filled in. 3) Click on the Search button to the right of the road information and it will give you a list of all the roads that intersect with the primary road throughout the township. Scroll until you find the correct one. 4) Select Show on Map and it will take you directly to the intersection and put a green dot on it to show that this is what you selected. You will need to either close or move the search box to see the map. 5) Once there, you will need to map the ticket according to the instructions that you gave on Step 3 in the Locate Where field. Make sure that once you have drawn the correct area, you save it by selecting the Save Tool. Attention: If you are contacted by someone who is NOT an employee of Indiana 811 who tells you that your locate request is not correct and must be resubmitted, please get their name and phone number. Remember, what you are creating is a legal document and your name is on each ticket that is entered, thus each ticket is ultimately your responsibility.

Street and Road Abbreviations The program accepts the following examples when the street name is an interstate, county road, state road or United States highway: Example: Interstate: Enter I applicable # (I 65). Example: County Road: Enter CO RT applicable # (CO RT 100). Example: State Road: Enter IN RT applicable # (IN RT 5). Example: United States Highway: Enter US RT applicable # (US RT 40). Note: If you have a Directional Prefix and/or Directional Suffix enter them in the corresponding fields. DO NOT use a period in this field. If you are digging on a county road that has BOTH a prefix AND a suffix, you will list only the county road number. The Type field further identifies your street name by the type of roadway (e.g. Road, Avenue, Boulevard, Court, etc.). DO NOT use a period in this field. Use the following abbreviations for the street type: Avenue.. Ave Boulevard Blvd Circle.. Cir Court. Ct Cove Cv Crest.... Crst Crossing. Xing Drive Dr Hill.... Hl Hollow Holw Lake Lk Landing..
