Minutes The Occupational Therapy Advisory Committee of the Board of Medical Licensure and Supervision met on April 7, 2016, in accordance with the Open Meeting Act. The meeting was held at the office of the Board, 101 NE 51 st Street, Oklahoma City, Oklahoma. Advance notice of this regularly scheduled meeting was transmitted to the Oklahoma Secretary of State on October 1, 2015, and posted on the Board's website on April 5, 2016. The notice and agenda were posted in prominent public view on the front doors of the Oklahoma Board of Medical Licensure and Supervision building located at 101 NE 51st Street, Oklahoma City, OK on April 5, 2016 at 3:00 p.m. Members Present: Kari Garza, OT, Chair Mary White, OT Kim Hancock, OTA Members Absent: Troy Lee, OT Ms. Wilma Cooper, Consumer Members Others Present: Reji Varghese, Deputy Director Barbara Smith, Executive Secretary Jason Seay, Assistant Attorney General, Alternate Committee Advisor Teresa Mitchell, Licensing Director Kenna Shaw, Licensing Administrative Technician Having noted a quorum, the meeting was called to order by Kari Garza at 9:03 a.m. After Committee review, Ms. White moved to accept the minutes of January 21, 2016 as written. Ms. Hancock seconded the motion and the vote was unanimous in the affirmative. JULIE B. MOORE, Occupational Therapist, appeared in support of her request for permission to continue supervising a sixth Occupational Therapy Assistant. Ms. Moore provided information and documentation for committee review regarding the supervision of the six Occupational Therapy Assistants ("OTAs") she currently supervises. The Committee requested that the sixth OTA be named rather than left open as Ms. Moore requested. Following further discussion, Ms. Garza moved to deny Ms. Moore's request to continue to supervise additional OTAs without an annual appearance before the Committee. Ms. Garza withdrew the motion. Following further discussion of Ms. Moore's Request No. 1 under "Requests for the Future" in her letter dated March 1, 2016 (See Attachment #1), Ms. White moved to deny Ms. Moore's request to not be required to make a yearly appearance regarding her supervision of OTAs, and to require an annual review with personal appearance and, further, require Ms. Moore Page 1 of 5

to complete the revised supervisory/request to supervise an additional OTA online form with continued documentation of supervision of each OTA listed on her license. Ms. Hancock seconded the motion and the vote was unanimous in the affirmative. Ms. White moved to amend the previous motion to add the requirement to complete the revised supervisory/request to supervise an additional OTA online form within fifteen (15) days of today's meeting. Ms. Hancock seconded the amendment and the vote was unanimous in the affirmative. Ms. Garza then moved to deny the request to not be required to make a yearly appearance regarding supervision and to require an annual review with personal appearance, and require the revised supervisory/request to supervise an additional OTA online form to be completed within fifteen (15) day of this meeting, with continued documentation of supervision of each OTA listed on Ms. Moore's license. Ms. Hancock seconded the motion and the vote was unanimous in the affirmative. Upon discussion regarding the status of the online supervisory/request to supervise an additional OTA form, Mr. Varghese advised the Committee that the revised form is complete and would be uploaded on this date. After further discussion and consideration of Ms. Moore's Request No. 2 under "Requests for the Future" in her letter dated March 1, 2016 (See Attachment #1), Ms. White moved to approve continued coverage to include a sixth therapist not to exceed a total of 128 supervised hours per week and provide written documentation of supervision provided every sixty days of all six (6) OTAs listed on Ms. Moore's license. Ms. Hancock seconded the motion and the vote was unanimous in the affirmative. After further discussion and consideration of Ms. Moore's Request No. 3 under "Requests for the Future" in her letter dated March 1, 2016 (See Attachment #1), Ms. White moved to deny the request that the sixth OTA be unnamed and the permission to supervise a sixth OTA extended for one calendar year commencing on July 7, 2016. Ms. Hancock seconded the motion and the vote was unanimous in the affirmative. Next the Committee reviewed applications for licensure. Ms. Garza moved to recommend approval of the application of KAYLA M GUILLIAMS for Occupational Therapist licensure. Ms. White seconded the motion and the vote was unanimous in the affirmative. Ms. White moved to recommend approval of the application of ASHLEY D MCALARY for Occupational Therapist licensure. Ms. Garza seconded the motion and the vote was unanimous in the affirmative. Ms. Hancock moved to recommend approval of JASON C ROSS for Occupational Therapist licensure. Ms. White seconded the motion and the vote was unanimous in the affirmative. Ms. White moved to recommend approval of the following incomplete applications for Occupational Therapist licensure pending completion of the files. Ms. Hancock seconded the motion and the vote was unanimous in the affirmative. Ms. Hancock moved to recommend approval of the applications of KATHRYN R TIPTON and DAELYN M. TUCKER for Occupational Therapy Assistant licensure. Ms. Garza seconded the motion and the vote was unanimous in the affirmative. Ms. Garza moved to recommend approval of the applications of PAMELA M DOWDY and SHANNON MARIE FOSTER for Occupational Therapy Assistant licensure. Ms. White seconded the motion and the vote was unanimous in the affirmative. Ms. Garza seconded the motion and the vote was unanimous in the affirmative. Ms. White moved to recommend approval of the incomplete application of DEBBIE SUE MUSUMECI for re-entry of Occupational Therapy Assistant licensure pending completion of the file. Ms. Garza seconded the motion and the vote was unanimous in the affirmative. The Committee then reviewed a list setting forth previously reviewed and pre-approved continued education units ("CEU"). Ms. Garza moved to approved the continuing education units requests on the list as presented. Ms. Hancock seconded the motion and the vote was unanimous in the affirmative. (See Attachment #2). Next the Committee reviewed requests for continuing education units which were not previously reviewed or pre-approved. Ms. Garza moved to approve the CEUs as presented. Ms. White seconded the motion and the vote was unanimous in the affirmative. (See Attachment #3). Finally, the Committee welcomed the Oklahoma City Community College visiting class of Occupational Therapy Assistant students. The Committee gave a brief overview of their responsibilities and the floor was opened to the students who asked several questions of the Committee. The students thanked the Committee for granting this learning opportunity. There being no new business, Ms. Garza moved to adjourn the meeting. Ms. White seconded the motion and the vote was unanimous in the affirmative. The meeting was adjourned at 12:08 p.m. Page 5 of 5
