EXHIBIT G: SUPPLEMENTAL ARCHITECT/ENGINEER AGREEMENT REQUIREMENTS WHALEN GYMNASIUM EXPANSION AND RENOVATION The Architect/Engineer Agreement shall be amended as follows: ARTICLE 1. BASIC PROFESSIONAL SERVICES Add the following paragraph 1.1.8: The Architect / Engineer shall participate fully in the code review process including submitting the required project documents and supporting information at the completion of each design phase, developing specific written responses for each issue identified by the code consultant and correcting the identified deficiencies in the construction documents. Provide all information required by the code review consultant(s) for this project. Approval must be obtained on a phase-by-phase basis. Specific requirements are identified in EXHIBIT D: Code Compliance Plan Review Procedures. It is the Architect's responsibility to obtain full compliance with all requirements identified in EXHIBIT C, Approved State Building Codes (Revised 7/18) and Exhibit G, Supplemental Architect / Engineer Agreement Requirements. ARTICLE 3. BASIS OF COMPENSATION 3.1. PAYMENT 3.1.2 Payment to the Architect/Engineer. Add the following sentence: The final (10%) of the Architect/Engineer s compensation for each phase of services as identified in Article 3.1.1 will be withheld until all required services for each individual phase have been provided. 1.6 CONSTRUCTION DOCUMENTS PHASE Add paragraph 1.6.7 Documentation (Reports, Specifications and Drawings) as follows: (a) Design Reports: Update the previous Design Development Report and clearly identify revised and/or additional information. The Construction Document phase submittal shall include a total of four (4) Design Reports and shall be formatted to match the previous Design Development Document Report. Include the following supplemental material..1 Structural Analysis: Provide two complete copies of the structural analysis for the project bearing the seal of the licensed Engineer in responsible charge. The structural analysis shall be provided at the completion of the Construction Documents Phase. One copy shall be provided to the code review consultant for the project. The other copy will be retained in the project file at the college..2 Asbestos Containing Materials: At the completion of the updated Construction Documents phase, provide a letter certifying that no materials or portions of materials specified for installation or use on this project contain asbestos. The Architect/Engineer will be responsible for (1) recording all modifications and publishing a report of the items and the corrective actions taken and for (2) providing a written response to each design review comment and shall specifically identify the corrective action and where that information will be found in the construction specifications and/or drawings. All design review comments and modifications will need to be addressed to the satisfaction of the College. Add paragraph 1.6.8 Required Documents as follows: Provide the following documents at the completion of the updated Construction Document Phase..1 A minimum of two sets of project specifications and drawings sealed by the appropriate registered professionals. These documents will be utilized for the construction contract documents..2 Four full size and two half size sets of project drawings and six sets of specifications. These sets will be utilized by the College staff during the Bidding Phase and Construction Administration Phase of the project..3 Single line floor plans with room numbers on 8 1/2" x 11" page size..4 Comprehensive building survey that includes all information required for (1) real property management including the area and volume of each room, the gross, actual, and assignable square footage of the building as computed to Colorado Commission on Higher Education Standards; and (2) custodial staffing and operations including the areas of all floor, wall and ceiling surfaces by room identifying the type of surface material, the Project 2007-130P18 Whalen Gymnasium Expansion and Renovation Page 5 of 7
number of light fixtures, plumbing fixtures, grills, and all special cleaning requirements. 1.8 CONTRACT ADMINISTRATION PHASE FOR MULTIPLE SEPARATE BID PACKAGES Add paragraph 1.8. 39 Errors and Omissions as follows: As part of the process to record design variations or departures, a means to clearly identify the source of the required change shall be identified. Errors and/or omissions in the design or project administration shall be clearly attributed to the Architect/Engineer. Changes in scope shall be attributed to the Principal Representative. The Architect/Engineer shall be responsible to compensate the College for the cost of additional work required to correct the work resulting from negligent acts, errors or omissions attributed to the Architect/ Engineer during design or project administration. Add paragraph 1.8.40 Documentation as follows:.1 Accessibility: At the completion of the project provide a letter certifying that the project has been inspected for compliance with accessibility requirements and that the project was found to be in full and complete compliance..2 Structural Analysis: At the completion of the project provide two copies of the revised structural report that includes all information regarding any changes made to the structural design during construction..3 Required Documents: Provide the following documents at the completion of the project..1 As-Built drawings in PDF format. MISCELLENEOUS PROVISIONS Add paragraph 12.25 High Performance Certification Program as follows: In recognition that a portion of the funding for the project has been provided to incorporate the requirements of the Office of the State Architect High Performance Certification Program (HPCP), the Architect / Engineer shall fully coordinate with the CMGC, assist Fort Lewis College and lead the effort and as necessary to achieve compliance. The Office of State Architect High Performance Certification Program (updated July 2018) is incorporated by Reference as part of Exhibit E.
