This workbook has been created to support you to prepare for your planning conversation with the NDIA Planner. It is not your NDIS plan but can help you to have the right conversation when you meet to discuss what reasonable and necessary supports you need. We have created this section at the side of each page to give some extra support. After completing this workbook, you can simply cut this strip off. Thinking about your future can be difficult Today s preparation determines tomorrow s achievements (author unknown) Getting NDIS Ready: Pre-planning Workbook By working through this workbook you will look at what you life looks like now and it will encourage you to think about what your future could look like. You can use this workbook independently, in our pre-planning workshops and/or with the support of family and friends. There is also a section at the back of the workbook for you to write questions that you may want to ask the NDIA Planner in your meeting. You will already have a lot of information which can be used to complete this workbook. If you have more information that you would like to include, you can attach extra pages or documents that you think are relevant.
All about me A one page profile captures all the important information about you on a simple sheet of paper, under three single headings. 1. What people like and admire about me 2. What s important to me 3. How best to support me It provides others with a way to support you to build better relationships by knowing what really matters to you. A well created one page profile can be used with any of your supports. Your one page profile can be updated whenever it needs to be to reflect your current situation. If you already have a one page profile then stick it on this page. If you do not have a one page profile, choose one of the templates and stick it in here to complete.
