ADPH website: ADPH.org Click on ADPH Portal Login, which is located on the bottom left side of the ADPH home page. Click on Security Portal. You will be taken to the Security Portal Login screen.
Enter your username and password, then click Login
Click Select next to the decedent s record to begin working on a death certificate.
Click on the red Cause of Death tab to go to the Medical Certification Section.
The decedent s name and date of death move over to this page at the time the record is created by the medical facility. Both items can be corrected here if needed. The time of death should be entered by the medical facility.
By indicating Natural Cause, the Trauma question will be answered, and the Injury information will be grayed out.
Other Significant Conditions is limited to 120 characters. Click Add or Remove to make changes to the Other Significant Conditions. 1. For Lines A, B, C, and D, as you enter a cause of death, entries that begin with what you typed will appear in a drop down box. You do not have to use the sample entries. 2. Lines A, B, C, and D are limited to 100 characters. 3. Please do not type the cause of death in all capital letters, unless it is an accepted abbreviation for which there is only one meaning such as COPD. 4. Enter the interval for Lines A, B, C, and D, or select Unknown.
The tobacco question must be answered for all decedents. The pregnancy question must be answered for female decedents age 5-75. By indicating Accident, Homicide, or Suicide, or by answering yes to the Trauma question, the Injury information will open and must be completed.
If Transportation Accident is yes, the second question must be answered. If Unknown, select Other, and enter Unknown in the box provided. The Place of Injury is chosen from a drop down box
1. The State, County, and City are chosen from drop down boxes. When the county is chosen, a list of cities in that county is provided. 2. The address is the specific location where the injury occurred.
1. After competing the Medical Certification Section, click on Save Record/Changes. 2. After saving, click on Sign Record.

If the Cause of Death is Pending, check this box. When the Pending box is checked, the Immediate Cause and Manner of Death are filled in and all other Manner/Autopsy questions are grayed out.
When the cause of death is Pending, verification is required. You will be sent to this page automatically after the first signature attempt.
1. Select Edit Status. 3. Select Update. 2. Select Verified As Is.
2. Select Death Record to return to the decedent s record. 1. Record has been verified. 3. Return to the Cause of Death screen.
After verifying that the cause of death is Pending, click on Sign Record.

When the death certificate has been signed, the home page is updated.
1. If there is an X in the column, Under CHS Review, the record cannot be filed and an issue must be resolved. If additional action is required on your part, you will be contacted by phone or email. 2. Examples of corrections that might be needed are a) the date of death entered by the funeral home does not match the date of death entered by the medical facility, or b) one or more lines of the cause of death are too long and will not fit on the death certificate. 3. A record that goes under CHS Review is reviewed by a member of the EVERS Help Desk Team. The record is then released for issuance, or sent back to the funeral home or medical certifier for correction. 1. If the record must be released back to you for correction, the record will be unlocked and moved back to the top of your home page. 2. When a record is sent back for correction, it will appear on your home page in a red bar, and the medical certifier s signature will be removed.
Select the record from your home page, and go to the Status page of the record. Instructions for correcting the error will be sent when the record is released. In this case, the date of death must be corrected before the record can be submitted again.
The Status page shows all activity on a death record.
1. Type Notes here. 2. Save the Notes. Once saved, the Notes move here. Notes can be added to a record and saved for future reference. The Notes do not appear on the death certificate.
The Search function provides several ways to look for a decedent s record. You can only search for records which you certified.
