www.lifestyleflooringinc.com CHECK IN PROCEDURES: Our warehouse opens a 7:00 a.m Monday thru Friday and at 8:00 a.m on Saturday when needed. We load crews in the order in which they check in. Any material not used from the previous day s work (example: job gets rescheduled) must be signed in by the warehouse crew. No signature that material was returned means you will be charged for the material in question. You are responsbile for checking the materials on your work order to the materials being loaded on your truck. The warehouse does make mistakes sometimes and we count on you to verify material you are being given. If you install the wrong material because you failed to check, you will be charged. The Sales Office is off-limits to the installers, for the exemption of using the restroom. This is not because we don t like you. We can t give our customers good servie if we can t hear them because of too much noise and distraction in the office. Please wait in the lobby/receiption area if you need to speak to a salesperson. TURNING IN YOUR PAPERWORK Please turn in your paperwork daily! In order for us to bill our customers on time we must have your paperwork. You and the customer should sign all paperwork. Any extra charges must be approved. RETURN OF PAD Unused pad should be returned with tape around it and signed in by the warehouse staff. Failure to get material signed in will mean a back charge to you. RETURN OF USED CARPET, PAD & VINYL All used materials and scraps should be placed in the dumpster, not thrown on the ground. If you are seen throwing material on the ground you will be fined $25.00 for clean up. 1

ON THE JOB PROCEDURES APARTMENTS Always check in with the office at the apartment complex. Remember to smile and be polite; this is your customer, too. Never let the maintenance man let you in. The office will be callingus thinking you didn t show up. Never enter the office without shoes & shirt. LEAVING YOUR HELPER TO DO YOUR JOB IS A TERMINATION OFFENSE. We hired you not your helper. When you go to the unit verify that the unit number is the same as on your work ticket. If you are asked to do a different unit instead you must call Lifestyle and also get the Manager s signature on your work ticket. When you enter the apratment check for marks on the walls, trash in units, doors not hung, etc. If you notice any damage, go to the office immediately and let them know. You don t want to be charged for something you didn t do. List any damage on your work ticket and get a signature form the office. On occupied units, check furniture before you move it. Any flaws should be pointed out to the resident so they won t think you did it. All closets that get carpet should be padded & tacked not loose laid. All units must be vacuumed. All old caulk should be removed from vinyl jobs. If you are skimming the existing floor be sure you do it well. Vinyl prep charges - all extra the office must approve charges and signed for. We do inspect jobs and if you charge for prep you didn t do you will pay to have the job done properly. Before leaving unit check for the following: A/C should be turned off. Check all closets to make sure you didn t miss any. Be sure you left no trash in unit. Make sure bathrooms and sinks are left clean. One of our most common customer complaints is a toilet not flushed. If you removed doors or appplicances be sure you put them back. Check Quality of your work. Trim all seams for threads hanging, etc. Be sure you vacuumed. Remove all old carpet, vinyl & pad, including scraps. Bring back to our dumpster, do not leave at complex. Return key to the office when job is completed. If office is closed, drop key in drop box. NEVER give the key to any one but the office. 2

Get your paperwork signed by the customer. The customer should initial all extra charges. Leave yellow copy for their records. Any extra charges not approved by the customer will be marked off your work order and you will not be paid. You are expected to do the job assigned to you. We cannot always give you the job of your dreams. Refusal of work scheduled for you is unacceptable. Trading jobs or giving your job away is also unacceptable. Jobs short If you feel a job is short, you must call the office and discuss it with the warehouse manager. Jobs rescheduled If the customer wants reschedule a job you must have them call Lifestyle for a reschedule date. If you cannot do your job DO NOT call the customer to reschedule, call Lifestyle and we will handle it. Arrive at the job when scheduled. If you have only one job, we expect you to go there in the morning. If you have more than one job check to see if any are A.M. Or P.M., and go to job at specified time. If you have a bring pad job you must put in the pad or have the Manager s signature saying pad was not needed. Without the signature, you will be charged for the pad at the retail cost. If for any reason you must go back to the previous days job, you must call (904) 332-8005 as early as possible so we can schedule you near the job. PROCEDURE FOR HOME BUILDERS OR NEW CONSTRUCTION Never park on a driveway. Most builders will make you pay for the driveway if you get a mark on it. Verify the address before you start work. Condition of floors. Houses should be given to you in broom clean condition, however you Can expect to scrape up some dry wall mud and paint overspray. If the floor is excessively trashed, get Supervisor to either get it cleaned out or give you a P.O. to clean it. Never throw you trash into another part of the house. If the carpet installer sweeps his trash onto new vinyl areas we may be back charged which we will pass on to you. Also, do not throw trash in the yard. If the vinyl installer left his mess in the carpet area, the carpet installer may bring it back and we will pay him and charge the vinyl installer and visa versa. Always check the house for damage tht my have occurred before you got there. (Example: broken windows). Call us at (904) 332-8005 or find the supervisor if there is any damage. 3

Absolutely no Smoking or Drinking Alcoholic beverages on the job. It goes without saying we will not tolerate any drugs being used. PROCEDURE FOR VINYL INSTALLERS If floor prep is required you must get a P.O. from supervisor. You will not be paid without written approval. Your floor must be free of all debris. Always use the proper lue and seam sealer for the floor you are installing. Always caulk the edges neatly. Wipe black number & letter off floor before you leave. If you did have to prep the floor be sure you allow ample drying time for the patch. On occupied homes where we are replacing vinyl you must install the vinyl under metal. If metal is not in very good condition, you should replace it. Remove all scraps from job and be sure you wipe all glue off baseboards, floors and cabinets. PROCEDURE FOR CARPET INSTALLERS Always verify your material to your work order. Be sure tack strip is no more than 3/8 from the wall. We highly recommed power-stretching. Not only will this prevent most call backs for loose carpet, but by preserving your back and knees from damage, it will allow you to keep doing your job for many years. Z-bar is required at ceramic areas by some of our builders. Walk your job when you are done and check the following: All seams are trimmed & finished. Closets are done and doors are re-hung. All scraps are removed from premises. RULES WITHOUT EXCEPTION (Breaking any of these could cause your termination.) You must maintian a valid driver s license, liability insurance and Workers Compensation (you may file an exemption certificte but your helper must be covered). Drinking alcholic beverages or doing any illegal drugs on the job. 4

Leaving your helper to do your job unsupervised. Being rude to a customer. Turning in work as complete that has not been finished. Giving apartment keys to anyone else (this should include your helper as you are responsbile for the keys, not your helper). Refusing to do the job assigned to you without a valid reason. (The fact that you don t want to do it is not valid). If you are unable to make it work, you must call the office no later than 7:15 a.m. 5
