He is a former real estate representative and was a municipal councillor for the Town of Renfrew for 14 years. Mr. Anderson served as a member of the Renfrew Police Services Board, as an appointed councillor. His community involvement includes serving as past president and life member with the Royal Canadian Legion and as an Elder with the Renfrew Presbyterian Church. Police Services Board - Trent Hills (Municipality of) Kathy Ann Dafoe Kathy Ann Dafoe, of Campbellford, works with Correctional Services Canada (CSC) in human resources after serving with CSC for more than 30 years in various roles including correctional officer and in senior administration. Her community involvement includes serving as a member of the Campbellford Memorial Hospital Auxiliary. She is a former CEO with Ottawa Community Housing. Ms. Poirier's community involvement includes serving as Vice Chair of the Board of Health with the City of Ottawa and as founding Co-Chair of Housing Partnership Canada. Mr. Chopra also brings 16 years of experience serving as chair for Human Resources Development Canada Board of Referees and as a member of the Consent and Capacity Board. Recently he has been appointed to the Mississauga Appeal Tribunal. His community involvement includes serving as founding chair of Rainbow Community Services of Peel, coordinator for the fundraising committee for Credit Valley Hospital, Carassauga and YMCA. Mr. Chopra holds a Master's degree in Social Work and has attended management development and strategic planning courses at McMaster University. Health Professions Appeal and Review Board Health Services Appeal and Review Board Angus Grant Angus Grant, of Toronto, was called to the Bar in Ontario in 2002. He articled and practiced with the Public, Regulatory and Administrative Law Group at Borden Ladner Gervais LLP and later commenced a practice focusing on immigration and refugee law with a focus on appellate litigation. Mr. Grant has also worked for Legal Aid Ontario as a staff lawyer. He holds a Juris Doctor degree from Osgoode Hall Law School where he examined intersections between international, administrative and national security law. Local Health Integration Network- Central West Local Health Integration Network Neil Grenville Davis Neil Grenville Davis, of Brampton, is a lawyer and Associate Partner with Davis Webb LLP. His community involvement includes serving as a director with the Canadian College of Naturopathic Medicine and as Vice-Chair of William Osler Health System. Mr. Davis holds an Honours Bachelor of Arts degree in Political Science from the University of Western Ontario and a Bachelor of Laws degree from Queen's University. Local Health Integration Network - Hamilton Niagara Haldimand Brant Paul Armstrong Paul Armstrong, of Binbrook, is Dean of Health Sciences and Community & Urban Studies at Mohawk College of Applied Arts and Technology. Mr. Armstrong's community involvement includes serving as the Vice-President of Brain Injury Services of Hamilton, as a member of the Hamilton Niagara Halton Brantford Community Care Access Centre, and as a committee member with the Hamilton Community Foundation's Community Health, Education & Research Fund. He holds Radiological Technology and Medical Diagnostic Ultrasound diplomas from Mohawk College and a Master of Business Administration degree from Queen's University. Local Health Integration Network- South East Local Health Integration Network Christopher Salt Nominated as Vice-Chair Christopher Salt, of Frankford, is a Chartered Professional Accountant who began his career as the Superintendent of Business and Treasurer for the former Prince Edward County Board of Education. He later became Superintendent of Operations and then Superintendent of Human Resources for the amalgamated Hastings Prince Edward District School Board. Mr. Salt's community involvement includes serving as a member on the Quinte Arts Council. He has been a member of the South East Local Health Integration Network since 2015. Page 5 of 10

Trillium Gift of Life Network Anne Atkinson Anne Atkinson, of St. Catharines, is a former Vice-President of Niagara Health System and a former Executive Lead for St. Catharines Hospitals. She was also a Clinical Manager at Hotel Dieu Hospital from 1987 to 2005. Ms. Atkinson's involvement includes serving as a member of the Hotel Dieu Shaver Hospital Board of Governors and as a member of the board of One Foundation. She is a former member of the Nic;1gara Grant Review Team (2013-2016). Ms. Atkinson holds a Bachelor of Science degree in Nursing from Laurentian University and is a member of the College of Nurses of Ontario. MINISTRY OF LABOUR Workplace Safety and Insurance Appeals Tribunal Beverley Burns Catherine Roberts Nominated as Vice-Chairs Beverley Burns, of Toronto, is a former Director of Employee and Labour Relations with Humber River Hospital and Seneca College of Applied Arts and Technology, and Partner with Watson Burns LLP. She also is a fonner Vice-Chair with the Labour Relations Board of British Columbia. Ms. Burns holds Bachelor of Arts and Laws degrees from the University of Western Ontario and a Master of Laws degree in Alternative Dispute Resolution from Osgoode Hall Law School. Catherine Roberts, of Dundas, is a lawyer and adjudicator. She was called to the bar in 1995, and holds a law degree from Osgoode Hall Law School and a Bachelor of Arts degree (Honours) from Queen's University. She spent 3 years as a law clerk with the Office of the Chief Justice of the Superior Court of Justice, and then practiced civil litigation in the Toronto office of a national law firm. Ms. Roberts now practices law in Dundas, where she is active in the community as Vice-President of the McMaster Children's Centre and a member of several committees of the Hamilton-Wentworth District School Board and the Hamilton Law Association. In 2010 Ms. Roberts was appointed as a member of the Assessment Review Board (Environment and Land Tribunals Ontario). Page 6 of 10

MINISTRY OF MUNICIPAL AFFAIRS ;, Building Code Commission Mazen Alkhaddarn Carolyn Bilson Rick Mori Hanna Regehr Nominated as members Mazen Alkhaddam, of Toronto, is a Principal with Master Building Architects, specializing in condominiums and retail. He is a former Project Manager with Bazis International where he specialized in high-rise residential architecture. Throughout his career, Mr. Alkhaddam studied building code, accessibility, risk and project management, as well as sustainability. Mr. Alkhaddam's community involvement includes serving on committees within the Ontario Association of Architects. He holds a Master of Architecture degree from the Ion Mincu Institute of Architecture and is a member of the Ontario Association of Architects. Carolyn Bilson, of Fergus, is an Architect with James Fryett Architect Incorporated. She is a former Teaching Assistant of Building Science with the University of Waterloo. Ms. Bilson's community involvement includes serving as a site selection committee member with Habitat for Humanity - Wellington, Dufferin and Guelph region. She holds Bachelor of Environmental Studies and Master of Architecture degrees from the University of Waterloo and is a member of the Ontario Association of Architects. Rick Mori, of Markham, is a former Senior Associate with Leber/Rubes Life Safety and Fire Protection Consultants and Building Engineer with the City of North York Building Department. His community involvement has included coaching various boys and girls sports teams. Mr. Mori holds a Bachelor of Science degree in Civil Engineering from the University of Toronto and is a member of the Professional Engineers of Ontario. Hanna Regehr, of Toronto, received her Master of Architecture degree in 1968 in Poland. After gaining professional experience in Poland and Finland, she arrived in Canada in 1970. After working in several large architectural offices, Ms. Regehr started her practice in Ontario as Hanna E. Regehr Architect in 1986, where she is Senior Architect and Principal. She has participated in numerous volunteer assignments at the Ontario Association of Architects, where she is a member. This included involvement with the membership committee and mentoring young architects. Ms. Regehr is a licenced Architect in six provinces. Page 7 of 10

Building Materials Evaluation Committee Gerry Egberts Dale Kerr Jeremy Neven Nominated as members Gerry Egberts, of Toronto, is a professional Engineer with over 30 years' experience and has been President ofegberts Engineering Limited since 1986. He practices structural engineering for buildings, building restoration design, and demolition review. Mr. Egberts was a member of the Building Material Evaluation Commission from 1998 to 2004 and the Building Code Commission from 2004 to 2015. His community involvement includes serving as a member of the property committee with Holy Trinity School and as a member of the planning committee with the Royal Canadian Yacht Club. Mr. Egberts holds Bachelor of Science and Master of Engineering degrees from the University of Toronto and has been a member of the Professional Engineers of Ontario since 1977. Dale Kerr, of Sutton West, is Principal, Chief Operating Officer and a Technical Specialist with GRG Building Consultants. Her community involvement includes serving as an Accessibility Advisory Committee Member with the Town of Georgina. Ms. Kerr holds Bachelor of Science and Master of Engineering degrees from the University of Toronto and is a member of the Professional Engineers of Ontario. Jeremy Neven, of Elgin, began his trade apprenticeship as a plumber after completing college at Sir Sandford Fleming. His trade involvement led to a diverse employment portfolio based on a strong mechanical background and includes self-employment as a plumbing and heating contractor, HVAC designer for a private industry wholesale organization and is currently the Chief Building Official for the Townships of Central and North Frontenac.. Mr. Neven has been involved in other projects such as LEED, Energy Star, has acted as a subject matter expert for Natural Resources Canada as well as assisting with developing, reviewing and delivering courses for the Ontario Building Officials Association. MINISTRY OF NATURAL RESOURCES AND FORESTRY Fish and Wildlife Heritage Commission Kathleen Reid Nominated as Chair Kathleen Reid, of Norwood, is the Communications and Education Coordinator for the Otonabee Region Conservation Authority in Peterborough. Most recently, she held the office of Vice-Chair of the Algonquin Forestry Authority and was a member of the Kawartha Highlands Signature Site Park Management Advisory Board. Ms. Reid is a member of the Ontario Federation of Anglers and Hunters and the Norwood and District Horticultural Society. She holds a Bachelor of Science degree from Trent University. Page 8 of 10

He is a former engineering consultant with Essar Steel Ltd. (India). Mr. Jariwala's community involvement has included serving as President oflndia Canada Association of Windsor & Essex and as a director with the Gujarati Samaj of Windsor. He holds a Bachelor of Chemical Engineering degree from Regional Engineering College (India), a Master of Nuclear Power Engineering degree from the University of Western Ontario and is a member of the Professional Engineers of Ontario. Grant Review Team - Toronto - Ontario Trillium Foundation Phyllis Tanaka Phyllis Tanaka, of Toronto, is a Senior Advisor for Food & Nutrition with Food & Consumer Products of Canada (FCPC). She is former Vice-President of Scientific and Regulatory Affairs with FCPC. Ms. Tanaka's community involvement includes serving as a member of the Food & Expert Advisory Committee and as a member of Equal Voice. She holds a Master of Science degree in Nutritional Sciences from the University of Toronto and is a member of the College of Dietitians of Ontario. Science North (Centre) Jo-Anne Palkovits Nominated as a trustee Jo-Anne Palkovits, of Sudbury, has been the President and CEO of St. Joseph's Health Centre since 2003. In addition, she has been the CEO of St. Joseph's Continuing Care Centre and the Administrator of St. Joseph's Villa and Villa St. Gabriel Villa. Ms. Palkovits is a Certified Health Care Executive and a graduate of Laurentian University with a Master of Arts degree in Human Development and a Bachelor of Science degree of Nursing. She also holds a Bachelor of Arts in Kinesiology from the University of Western Ontario. Ms. Palkovits has served on the Board of the Sudbury Community Foundation as Chair (2007-2009), United Way Fundraising Cabinet as Health Sector Co-Chair (2011-2013), NEON Lights representative on the Canadian College of Health Leaders Board (2006-2010) and is currently a member of the Cambrian College Board of Directors (2012-present). He holds a Bachelor of Alis in Political Science and a Bachelor of Education from the University of Western Ontario. He completed his Bachelor of Laws degree at the University of Windsor. Mr. Paluch articled at Blake Cassels &Graydon LLP. He has served as an Adjudicator of the Toronto Licensing Tribunal and has been a lawyer member of the Consent and Capacity Board of Ontario since 2015. Mr. Paluch is a member of the Society of Ontario Adjudicators and Regulators and completed the Certificate in Adjudication for Administrative Agencies, Boards and Tribunals. Robert Markovits, of Toronto, graduated from the New York Institute of Technology in 1984 with a Bachelor of Architecture degree. He then became a Project Architect/ Associate and later a Sole Practitioner architect for the Robert Allan Markovits Architect firm where he focused on,, small-scale construction. In 2007, Mr. Markovits received his Master of Laws degree from the Osgoode Hall Law School. From 2009-2015, he was appointed as a member to the Immigration and Refugee Board of Canada, Refugee Protection Division.
