(They may choose to sponsor you, match or part match your fundraising total, or allow you to promote your fundraising on the work intranet) Your running vest & socks We offer all London Marathon runners a free running vest and socks, along with iron-on letters so you can put your name on your vest in preparation for the big day. These sizes are the actual measurements of the vests. WOMENS MENS Please enter style (womens or mens) and size: Letters for running vest (maximum of 8 letters): About your fundraising How much do you hope to raise? 4 Your fundraising plan Please give us details of how you plan to go about your fundraising. Our Sports Events Team will be on hand with ideas and advice but please outline your own plans below. For example, you may choose to hold a quiz night, cake sale or ask your local supermarket if you can hold a bag packing day. Please state how much you hope to raise from each idea/event. Please provide as much information as possible as these details will affect whether or not your application is successful. Top tips Try to break your target down into achievable blocks and think about how you would raise each amount. Think about work/social contacts you have and any social activities you take part in e.g. are you a member of a club or sports team? Could you organise any events or activities in your local area or at work to boost your fundraising potential? Your story could really help to illustrate the difficulties of arthritis to others. Would you be happy for us to contact you again about potentially using your story in the media? Yes No
5 Terms and conditions Before registering, please read the following carefully and sign below: A 50 commitment fee is required as a Gold Bond place holder. A minimum pledge of 2,000 in sponsorship should be raised from your fundraising. We encourage you to send at least 1000 of your sponsorship money to Arthritis Research UK at least 3 weeks before the marathon date. The remaining sponsorship should be sent to us by the end of June. Should you choose to withdraw, you may return all your sponsor forms and forward monies raised (provided your sponsors agree to this) to Arthritis Research UK or return monies to your sponsors. Your commitment fee of 50 will not be refunded. Arthritis Research UK is obliged to report anyone who fails to raise the minimum sponsorship amount to the London Marathon. This will affect your chances of competing in any other London Marathon Event. You must be a minimum of 18 years old on race day. Of monies raised, 300 will be used to cover the cost of your Gold Bond entry. You participate at your own risk and take full responsibility for your actions prior to, during and after the London Marathon. Please note Gift Aid does not count towards your sponsorship total. Photographs and video footage will be taken at this event and may be used for publicity purposes. Declaration I apply to take part in the London Marathon on behalf of Arthritis Research UK and have read and understood the conditions of entry. I understand that once I have been offered a Golden Bond place with Arthritis Research UK, I will need to pay my commitment fee of 50 within 14 days. My place will only be confirmed once I have paid the commitment fee. The offer of a place will expire 14 days after the date of making the offer if Arthritis Research UK has not received the commitment fee before the date. I pledge to raise a minimum sponsorship of 2000 and understand the terms and conditions of raising this money. I confirm that I am fit enough to complete in the London Marathon and that I do not have any medical conditions which may prevent me from taking part in or completing the London Marathon. I take full responsibility for myself and I confirm that I have sought the advice of my GP before taking part in the London Marathon. I understand that I am responsible for my own actions and omissions and that Arthritis Research UK, its employees and volunteers cannot be held responsible for any personal injury or death caused by my participation in the London Marathon or the loss or damage of my personal effects. I understand that Arthritis Research UK does not hold a policy of insurance which will cover me for the event and that it is my responsibility to purchase any appropriate insurance to cover my own injuries which I may cause to others.
