1 Call for expression of interest: Scoping study to inform the feasibility of developing the seasonal malaria chemoprevention (SMC) /SMC-Calc modelling tool Malaria Consortium is one of the world's leading non-profit organisations dedicated to the comprehensive control of malaria and other communicable diseases in Africa and Southeast Asia. Malaria Consortium works with communities, government and non-government agencies, academic institutions, and local and international organisations, to ensure good evidence supports delivery of effective services, providing technical support for monitoring and evaluation of programmes and activities for evidence-based decision-making and strategic planning. The organization works to improve not only the health of the individual, but also the capacity of national health systems, which helps relieve poverty and support improved economic prosperity. Malaria Consortium is a global leader in the prevention, diagnosis and treatment of malaria, the control of neglected tropical diseases and the promotion of child health. This is a call for expression of interest to qualifying individual consultants with: MSc degree in epidemiology and/or PhD (desirable); a good understanding of malaria transmission in the Sahel Region; demonstrated experience in statistical analysis of public health data and large datasets; demonstrated strong report writing skills; experience in organization of research and expertise in epidemiological modelling; to present motivation letter, proposal for delivery of the assignment as presented in the terms of reference below. 1. About ACCESS-SMC Project ACCESS-SMC is a UNITAID-funded project, led by Malaria Consortium in partnership with Catholic Relief Services, which is supporting National Malaria Control Programs to scale up access to seasonal malaria chemoprevention (SMC) to save children s lives across seven countries in the Sahel. By demonstrating the feasibility and impact of SMC at scale, ACCESS-SMC will promote the intervention s wider adoption. For further information visit and Envisaged Public Health benefits of ACCESS SMC: If ACCESS-SMC reaches its goal of covering 7.5 million eligible children with 30 million treatments annually in seven supported countries, the public health impact would be 5.4 million cases of malaria and 24,500 deaths averted in both 2015 and 2016, totaling 10.8 million cases and 49,000 deaths averted over the life of the project (based on estimates from Cairns et al). Additionally ACCESS-SMC was designed to contribute to: I. Reduce costs of SMC administration and promote its wider adoption by demonstrating its feasibility and impact at scale. II. Demonstrate the impact of SMC on reducing malaria incidence, increase communities support for their children receiving SMC products and in the long-run raise households value on protecting their children with SMC and prioritize the acquisition of SMC products III. Reinforce and expand or help create pharmacovigilance systems, which may be employed for other health interventions. 1
2 Thus ACCESS-SMC has the potential for indirectly redirecting extensive health system and household resources currently directed towards malaria. With parasite-based diagnosis as national policy, SMC will reduce the number of treatments with ACTs in proportion to the number of fever episodes due to malaria compared to non-malarial fever episodes. For example, in a scenario with three fever episodes per annum per child aged three to 59 months, one of which would be due to malaria and two due to other causes. This would also mean fewer numbers of diagnostic tests will be used because malaria will be prevented (i.e. two fever episodes instead of three) compared to a scenario with universal parasitebased diagnosis and no SMC (i.e. three fever episodes). This translates to health system savings The SMC-Calc Concept SMC Calc was conceptualized by Malaria Consortium as an innovative study that generates a tool to be either a management information tool or a pragmatic modeling tool, modeled on Net Calc and like tools, depending on the end user. Additionally, the SMC-Calc will be a tool to disseminate and make use of the enormous data generated from ACCESS-SMC. Envisaged use of SMC-Calc It is envisaged that SMC-Calc shall help (and not limited to): I. Analyze the correlation between coverage and impact, predict savings/cost averted due to SMC; II. Build on baseline morbidity, visa vie cost of treatment averted by SMC III. Consider correlation with other preventive interventions- LLNs, Vector Control IV. Define and describe implications of SMC at large scale visa vie burden on health system due to malaria, and other case management interventions- V. End product needs to be aligned to existing models, and SMC effect on health systems SMC-Calc final product The SMC-Calc concept note envisages that the final product shall be a web-based centrally maintained tool that would enable NMCP / NMEP and partners to estimate the effect of SMC on malaria morbidity and reliably predict the numbers of averted malaria cases in different settings and in response to different scenarios. It will be similar to the Pit Decision-Support Tool 2 and the Malaria Vaccine Model 3, with predictions of malaria morbidity estimated at national, regional levels and district levels. In a way it provides rationale for sustaining investment for SMC, the cost-benefit analysis of the intervention, visa vie other malaria prevention interventions. 3 Basis for SMC-Calc development From the 2012, WHO recommendations and guidelines on SMC, it is known is that using SP+AQ monthly for up to 4 months during the transmission season in children less than 5 years of age: Prevents approximately 75% of all malaria episodes Prevents approximately 75% of severe malaria episodes May result in a decrease in child mortality of around 1 in 1000 Probably reduces the incidence of moderately severe anemia Does not result in an increase in clinical malaria in the following malaria transmission season after one year of administration but the consequences of giving SMC for several years have not yet been evaluated Serious adverse events have not been reported and are probably rare However, the public health impact of SMC at scale is not fully known, number and costs of treatments averted and related costs savings are not documented. 3. Purpose and Objectives of the assignment The scoping study shall be the preliminary phase for developing the SMC-Calc tool. The consultant epidemiologist / scientific modeler shall be expected to conduct a scoping study (baseline) to inform the feasibility and opportunity of developing the SMC-Calc concept. Purpose of the assignment The overarching question to be answered at this phase is the strategic value, the use and target audience of the SMC-Calc modelling tool. Specific objectives / Questions The consultant will help answer the following questions, though this list may not be exclusive and other aspects may be raised as the work progresses: i. What is the strategic value (intrinsic and perceived) of the SMC-Calc modelling tool? ii. Who will use SMC-Calc, and how? iii. Are there any existing tools that could be the base for SMC-Calc or similar in scope, or competing with its stated usefulness? iv. What research questions will be addressed by SMC-Calc? v. What is the gap in information that SMC-Calc will add on? vi. What are the required skills set to enable subsequent phases of the development of the tool? vii. Can the SMC-Calc concept be tested during the life time of the ACCESS-SMC project? Target audience: possible users or contributors to SMC-Calc modelling tool development Managers, Coordinators, M&E focal points in National Malaria Control Programs, and specifically in the Sahel countries that qualify for SMC, as per WHO guidelines Program Managers, Coordinators, M&E focal points from WHO Malaria Control programs, RBM Initiative, WARN and other UN agencies supporting SMC work Other UN agencies, UNICEF, UNDP and others involved in Malaria Control 3
4 Researchers and Academic Institutions, professional societies like ASTMH, contributing to malaria modeling; Imperial college LSHTM and others International and Local NGOs program managers, involved in SMC: MSF; Terres d hommes; CSSI Donors such as USAID(PMI), DFID, the Global Fund, UNITAID and others supporting SMC scaleup Private sector and pharmaceutical companies ACCESS-SMC partners including: Malaria Consortium SMC-Calc reference group, MMV, MSH, LSHTM, CRS, SUA and others 4. Proposed Methodology Methodology The methodology shall include amongst others: a robust document review, including a review and analysis of malaria existing modeling tools; engaging with academic institutions, the Imperial College research group and LSHTM Engaging with managers of NMCP in the Sahel, as well as learn from NMCPs in Africa outside the Sahel. Engaging with prospective target audience and potential contributors. The consultant shall work in consultation with Malaria Consortium SMC-Calc group, from study inception phase, to implementation and report drafting phase. Additionally, the consultant is expected to propose appropriate scientifically sound methodology to generate required results and deliverables, as defined in these TORs. Deliverables 1) A scoping study report of not more than 15 pages (annexes excluded), and a PowerPoint presentation: a) Answering the key strategic questions around the feasibility and opportunity of SMC-Calc model, its use and its prospective target users, resulting from sound research, literature review and scoping surveys. b) Presenting a thorough analysis on use of malaria modeling tools developed in the past (such as Net-Calc) or in development. c) Providing clear recommendations to Malaria Consortium ACCESS-SMC strategic group on the value added of SMC-Calc vis-à-vis available tools, and on whether there are alternative or similar tools upon which SMC-Calc could leverage, build or adapt, or if it would be a new tool entirely. d) If SMC-Calc is deemed a feasible concept, the consultant should outline next steps and timeframe as well as the required technical resources for the development of the SMC-Calc, up to the stage of using available ACCESS-SMC data to pilot the tool in its final phase. 2) The scoping and feasibility study report shall be orally presented and defended by the consultant at the ACCESS-SMC lessons learnt meeting, scheduled for mid-january 2016 in Kampala 4
5 3) All raw data, tools and materials used in relation to this assignment shall be deposited with Malaria Consortium for future reference. Timeline The estimated duration of the assignment is 24 consulting days maximum. Work shall commence by the November 30, 2015, and the 1 st draft report availed by December 20, The pre-final report should be ready by January 7, 2016, informed by feedback on the draft report by the Malaria Consortium SMC- Calc reference group. The Consultant in his bid proposal shall draft a Gantt chart, mapping out the study timeline 5. Management arrangements This assignment shall be managed within ACCESS-SMC project management structures; the consultant for day to day operations shall report and be managed by the Public Health Specialist. The consultant shall in the process of carrying out the assignment engage with the SMC-Calc reference group through face-to-face meetings, or online discussions. Overall project oversight is by the ACCESS-SMC Project Director and with Technical Oversight through the African Technical Director. Location: Home-based assignment, attached to the Malaria Consortium Africa Regional Office, Plot 25 Upper Naguru East Road, Kampala, Uganda Data ownership All data and products generated from this assignment shall be owned by Malaria Consortium and the ACCESS-SMC partners Consultant qualifications The TORs seek for an individual consultant with MSc degree in epidemiology and/or PhD (desirable) Essential skills and experience: Expertise in malaria transmission in the Sahel Region, Advanced skill in Stata programming, Demonstrate experience in statistical analysis of public health data and large datasets; demonstrate strong report writing skills, for similar/related assignments, experience in organization of research. Desirable skills and experience: Expertise in epidemiological modelling (related to malaria preferred), expertise in IPTi/SMC, excellent communication skills (written and oral English, working knowledge of French would be an asset).
