1 Guyana Shield Meeting on the Implementation of the Framework for Artemisinin Resistance Prevention, Containment, and Elimination in South America Paramaribo, Suriname July 2016 Background Artemisinin-based combination therapies (ACTs) are the recommended treatments for Plasmodium falciparum malaria in all malaria endemic areas of South America. Resistance of P. falciparum to the artemisinin drugs has already been detected in the Greater Mekong subregion of Southeast Asia and would represent a major setback to malaria control efforts if it were to develop in or spread to South America. Although artemisinin resistance has not been confirmed in the Americas, the interior of Guyana, Suriname, and French Guiana and bordering areas of Brazil and Venezuela (together known as the Guiana Shield) share many characteristics with the Greater Mekong subregion that increase the risk for selection of resistant parasites. These characteristics include higher levels of transmission of P. falciparum than in the rest of the Amazon Basin, highly mobile populations, ready availability and widespread use of a variety of antimalarial drugs of questionable quality, including artemisinin monotherapies, and lack of access to and use of formal malaria diagnostic and treatment facilities. The Framework for Artemisinin Resistance Prevention, Containment, and Elimination in South America was consolidated in November 2014 to prevent the development of artemisinin resistance in South America, and to contain and eliminate resistance if it should be confirmed. A longer-term objective of the framework is to eliminate P. falciparum malaria, as this will be the only sure way to avoid the selection of resistant parasites. While some activities geared towards responding to the recommendations in the framework were implemented through the work of the Amazon Malaria Initiative/Amazon Network for the Surveillance of Antimalarial Drug Resistance (AMI/RAVREDA) since 2014, targeted and intensified efforts are deemed essential especially in the light of the goals of the Global Technical Strategy (GTS) for Malaria , the commitments outlined in the Plan of Action for Malaria Elimination in the Americas , and the targets expressed by countries in their respective national malaria plans. s This meeting is a concrete step in intensifying efforts in the Guyana Shield towards the implementation of the Framework for Artemisinin Resistance Prevention, Containment, and Elimination in South America. Specifically, the meeting seeks to: Review the malaria strategies of Guyana Shield countries, the Framework for Artemisinin Resistance Prevention, Containment, and Elimination in South America and corresponding malaria elimination targets (as in the GTS ). Discuss challenges and gaps experienced by countries in the Guyana Shield in the delivery of the WHO recommended strategies. Review key interventions, relevant to the Guyana Shield, in scaling up malaria programmes towards Universal Access. Provide an overview of elimination procedures and the available resources, tools and opportunities for scaling up malaria programmes towards elimination. Facilitate joint planning of activities towards the implementation of the Framework for Artemisinin Resistance Prevention, Containment, and Elimination in South America and in support of malaria strategic plans of participating countries. 1

Challenges and gaps in the implementation of the WHO recommended strategies identified. The susceptibility of the Guyana Shield countries to artemisinin-based combination therapies resistance discussed. Key interventions in reaching migrant/mobile populations in the Guyana Shield reviewed. The way forward on the proposed self-diagnosis and treatment intervention discussed and agreed on. Elimination procedures, available resources, tools and opportunities to scale up malaria programmes reviewed and discussed. Joint planning opportunities, a coordination and networking structure between countries created. Country plans with identified priorities including joint and aligned activities between countries developed.
