6 There are no surgical robots! (yet) A robot is any electro-mechanical device that is programmed to perform a set of tasks autonomously, once it is started. A robot does not require constant human control. A surgical assist device is always under the direct control of a surgeon. The surgeon may be local (in the operating room) The surgeon may be remote; next door or around the world (telesurgery) A combination of the two (local control & remote supervision or remote control & local supervision.
7 Mediating Systems Between Input and Output: typewriter evolved through the recording typewriter to word processor piano evolved through player piano and MIDI to music synthesizer machine shop processes evolved through NC to CNC machining stage performance evolved through VTR & DVD to recorded video each evolution allows: storage of input, independent of output device correction and revision of input after recording one-to-many replication of output choices of output device review and optimization of input to create perfect output output can be physically and temporally remote from input When will that all happen to surgery?
8 Intuitive Surgical Corp s da Vinci Surgical System Strongest in MIS: minimally invasive surgery First FDA approval July 2000 for laparoscopic surgeries 3803 units in use worldwide, 2501 in the US (Sept 2016) The da Vinci is a telesurgical system. Surgeon controls remotely. Primary advantages: decreased surgical personnel (maybe) decreased physical fatigue for the surgeon better-than-human control of instrumentation filtering of muscle tremor ability to manipulate instruments with greater degree of freedom than human hands minimal surgical trauma for the patient Primary disadvantage: cost (~$2M currently, plus expendables)
