1 WIAA ATHLETIC CARDS Green athletic cards are required whenever a student participates in a WIAA athletic sport. They must be signed by the physician. Green cards are good for two years, with the alternate year requiring the beige alternate year card signed by the parent. If the date of the physical is prior to April 1: The green card is good for the remainder of that school year plus the following school year. For example: If the physical was February 15, 1998, it is good for the school year (green card), and the new school year ; but you will need a beige or alternate card for the year. You will need a new physical for the school year If the date of the physical is after April 1: The green card is good for the following two school years. For example: If the physical was May 15, August 15, or September 15, 1998, it is good for the school year (green card signed by doctor) and the school year (you need a beige card signed by the parent) Students will not be allowed to practice or try out without the necessary card on file. Athletic cards are kept in the school office. Athletic fees are $75.00 per sport. ATHLETIC FEES BUS INFORMATION All students are required to ride the provided school transportation to and from an event. We do realize that on occasions it is necessary to take your child from an event. When this case arises, please check with the coach. Jean Nicolet Road Glendale, WI *Grafton High School 1950 Washington Street Grafton, WI I-43 south, left (east) on Good Hope road, right (south) on Port Washington Road for 1 block, right (west) on Green Tree (cross over I- 43) and turn left (south) onto Jean Nicolet which leads to school on the right. East on Mequon Road, north on I-43, left (west) at Grafton exit (Hwy. 60, Washington Street) to school on the right side. *Kletzsch Park Glendale, WI 15- Range Line Road south to Green Bay Avenue. Travel until you hit Fond du Lac Avenue (1 st stoplight after Hwy. 45 turn left and it winds around and is about 1 mile down the road on the right side of the street. Range Line Road Mequon, WI minutes 30 minutes Mequon Road west to 76th Street (Wauwatosa Road). Turn left (south) and follow to Brown Deer Road - turn right (west) and continue on Brown Deer Road - it becomes Main Street and Hwy 74, go left (south) on Pilgrim Road, left (southeast) on Appleton Avenue (Hwy. 175). Go past the Hwy 57 stoplight and the school will be on the left side of the street *Rotary Park Highland Road Mequon, WI 427 W. Jackson Street Port Washington, WI minutes Mequon Road east to Port Washington Road. Turn left (north) onto Port Washington Road. Turn left (west) onto Highland Road. Follow to park on the right side. Port Washington *Thomas Jefferson Middle School 1403 N. Holden Port Washington, WI Mequon Road east to I-43. North on I-43 and exit right (east) on Hwy. 32 which becomes Grand Avenue. Turn left (north) on Webster, left onto VanBuren, and left onto Holden. The track is in the back of the high school. Stairs lead down to it. *Upper Bluff Park Port Washington, WI Mequon Road east to I-43 North. Follow this to Port Washington exit 100. Turn right (south) onto Wisconsin Avenue to Whitefish (right after Pizza Hut). Turn left (east) - veer left onto Hale s Trail. Follow for 2 blocks - parking lot on right. Mequon Road east to I-43. O (Mill Street stop-n-go lights). Turn right (North) on Hwy. O and follow for 3 miles. The park is on the left. Take Mequon Road West to 175 go north about 2 miles until the corner of Holy Hill Road and 175. School is on the corner. River Hills *University School - Milwaukee 2100 W. Fairy Chasm Road River Hills, WI Take Mequon Road east to Range Line Road. Go south on Range Line - travel across County Line Road to Fairy Chasm Road. Turn left (east) onto Fairy Chasm. School entrance is on the left side of the road. Shorewood *Estabrook Park Shorewood, WI Mequon Road east to I-43 south to the Silver Spring East exit. Turn right (south) onto Port Washington Road to Hampton Avenue. Turn left (east) and follow for a few blocks to the park. Turn right (south) into the park. *Kern Park Shorewood, WI Take I-43 South to Capitol Drive go east on Capitol to Humboldt Avenue go right on Humboldt to Kern Park (about 2 blocks). Go west on Hwy. 60 through 1 set of stoplights on the right is a strip mall. The next road is directly after the strip mall turn right and go less than a mile to the school. Take Hwy. 45 south to Silver Spring Drive west. (Just after Silver Spring Country Club on the right), turn right (at the 4 way stop) onto Townline Road. School is on the left (west) side.
