1 DENISE M. MIRANDA, ESQ. NYS Justice Center for the Protection of People with Special Needs Denise M. Miranda, Esq., Executive Director of the NYS Justice Center for the Protection of People with Special Needs (Justice Center) has been actively engaged in the practice of law for more than 20 years focusing her work on protecting the rights of vulnerable individuals. Ms. Miranda graduated from New York University (B.A. 1993) and from St. John s University School of Law (J.D. 1996). She is a member of the Bar of the State of New York as well as admitted to the Southern and Eastern Federal District Courts. Prior to her appointment as Executive Director of the Justice Center, Ms. Miranda served for six years as Managing Director of the Urban Justice Center s Safety Net Project (UJC) in New York City. UJC provides direct legal services, litigation, research and policy making to achieve economic justice for low and no income New Yorkers. Ms. Miranda began her legal career as an Assistant District Attorney in the Domestic Violence and Sex Crimes Bureau of the Bronx County District Attorney s Office. Her extensive experience working with domestic violence survivors served as a foundation for her transition to civil practice working as a senior attorney with the Network for Women s Services and later when she established her own private practice. Ms. Miranda is a member of the New York State Bar Association, the Puerto Rican Bar Association, the Women s City Club and 100 Hispanic Women of Westchester. She is an active volunteer and board member of several non-profit organizations. She lives with her family in Westchester County.

2 Jo ANN LAMPHERE, DrPH NYS Office for People With Developmental Disabilities JoAnn Lamphere serves as Deputy Commissioner for the Division of Person Centered Supports within the Office for People With Developmental Disabilities. She and her team are responsible for designing and implementing policy solutions that address significant transformation goals established for OPWDD. These include: design and implementation of Person First Care Coordination, Medicaid redesign and Waivers, implementation of the CAS person-centered assessment tool, strengthen Self-Direction to support program s exponential growth, assure compliance with home and community-based services (HCBS) settings, build new options for home and community living and employment, and cultivate workforce culture change. Dr. Lamphere has served as a health care consultant in the Washington, D.C. and Colorado offices of Health Management Associates. She consulted with private sector organizations and assisted community-based disability and aging organizations in designing and delivering managed long-term services and supports. Dr. Lamphere served as AARP s State Government Affairs and Advocacy Director for the Health & Long-term Care team in Washington, DC. She began her career with the NYS Department of Health. JoAnn earned her Doctorate in Public Health, specializing in Health Policy & Management, at Columbia University. SHAWN M. DES ROCHES, MPA NYS Office for People With Developmental Disabilities Shawn Des Roches serves as Deputy Director for the Division of Quality Improvement and Performance Management within the Office for People With Developmental Disabilities. Shawn came to OPWDD in September of Prior to OPWDD, he was with NYS OMH for 11 years and served in various roles which included Licensing and Clinical Risk Management. He also worked within a County LGU for 7 years which included CBR/CFR reporting, program development under reinvestment and oversight of clinic operations under OMH and OASAS. He has his Masters in Public Administration from Marist College. Outside of work Shawn has been a Volunteer Firefighter for 27 years and is an Assistant Fire Chief. He also serves on his Town Planning Board.

3 RUSSEL BELL Oneida-Lewis Chapter, The Arc New York Russel Bell, CIO at The Arc, Oneida-Lewis has 20 years of experience in designing, developing, and implementing technology projects. Russel started his career in the computer industry in 1989 while serving in the US Air Force. In 1995, he began his second career working for The Arc, Oneida-Lewis on a variety of IT projects from planning and design to implementation. Beginning in 2011, Russel worked closely with the CEO and stakeholders on a multi-year strategic plan to build a more robust IT infrastructure to prepare for IT advancements in the health and human services field. His accomplishments have been implementing an EHR, building data management and collection systems, and more recently directing the implementation of the agency s NVITE telehealth project which stands for Non-Virtualized & Individualized Telehealth Experience. Russel and his experience lead to successfully being awarded telehealth grants from BIP, NICIP, and USDA- DLT/RUS totaling close to 1.5 million dollars for telehealth equipment and infrastructure throughout all of their residential and day habilitation sites across Lewis and Oneida counties. Russel will be speaking today about the agency s NVITE project, the reasons why they chose to do telehealth, and things to keep in mind when deciding to engage in telehealth across your agency. JACLYN McKENNA The Center for Discovery Jaclyn McKenna (Jackie) has been supporting people with disabilities in various ways for 24 years. Jackie began her career in the field as a musical/theater director for a camp for kids with disabilities on Long Island, NY. In college, Jackie became a Direct Support Professional at a Dutchess County agency. From there, Jackie s career path took her to an agency in Orange County where she worked in the mental health field, and then transitioned to a Medicaid Service Coordinator, advocating and acquiring the necessary supports and services for people living with disabilities. Jackie found her true passion when she began working in the Quality Improvement and Corporate Compliance Department, maintaining certified investigator status, as well as assuring that regulations were being met and that the highest quality of life was being obtained for the people supported at the agency. Jackie has been working at the Center for Discovery as the Chief of Quality Improvement, Corporate Compliance and Adult Continuous Program Development for over five years and maintains her passion for quality and compliance while ensuring that the people supported at the Center for Discovery have their rights protected and live life to the fullest.

4 TIM O BRIEN & JOHN GLODE Montgomery County Chapter, The Arc New York Tim O Brien is the Director of Information Technology and the Security Officer for the Montgomery County Chapter, The ARC NY. In his 5 years at Liberty ARC, he has worked to bring IT from behind the scenes keeping the lights on to having a meaningful impact in people s lives on a daily basis. He is passionate about building relationships with direct care staff and people that receive support services to see how technology can help achieve a greater quality of life. Tim spent over a decade in the profit sector traveling internationally to support many companies where he specialized in networking and security. He has achieved over a dozen certifications in the IT field and holds a bachelor s in Computer Science but he can t stand writing code. He found a home at Liberty ARC which provides a way to merge his passion for people and technology. Tim loves his family which includes two beautiful daughters, impacting people with technology, golf and breathing, in that order. John Glode has been in the disabilities field for over 20 years, and is currently Operations Director for Liberty ARC in Montgomery County NY. He leads a very committed team in providing residential and day program options to 450 persons. His passion is for advocacy for persons with disabilities, and to achieve creative ways for people to work together to support that cause. He has a variety of career experiences that provide him with a well-rounded and balanced approach. John achieved a BA at the University of Albany, and holds a PHR Certification in Human Resources. He leads through a combination of expertise, teamwork, and humor that translates into quality services. He is a husband and very proud father of three. PHILLIP PROCTOR AHRC New York City Philip Proctor is Director for Enterprise Innovation at AHRC New York City. He leads mission focused technology deployments across all programs at AHRC New York City. Philip aligns people, processes and technology platforms in support of mission driven technology deployments. Philip is currently working on developing a client facing network enabling higher levels of independent living for people with intellectual and developmental disabilities (I/DD). In previous roles in AHRC Philip led AHRC New York City s Balanced Incentive Program Innovation Project, supporting people with I/DD to access and integrate consumer technologies into personal goal pursuit. Philip led the creation and delivery of micro-learning knowledge base to support staff development. Prior to coming to the United States Philip worked in the U.K. education system and support teachers to harness the benefits of technology in their classrooms.

5 MELISSA M. ZAMBRI Barclay Damon, LLP Melissa M. Zambri is a partner with the law firm of Barclay Damon, LLP in its Albany, New York office. She focuses her practice on enterprise development and regulatory guidance for the health care industry, providing counsel to numerous types of providers regarding HIPAA, compliance, Board of Director responsibilities, fraud and abuse, licensing, certificate of need and other transactional, regulatory and business matters. She is the Chair of the Firm s Health Care Practice Group and Co-Chair of the Firm s Health Care and Human Services Practice Area. Ms. Zambri is a member of the American Bar Association and New York State Bar Association Sections on Health Law and the American Health Lawyers Association. She was the former Chair of the New York State Bar Association Health Law Section s Committee on Fraud, Abuse and Compliance. She has published numerous articles and health law advisories, and has been a lecturer on various health law topics such as compliance programs, conflict of interest, board of director responsibilities, fraud and abuse issues, audits and investigations of health care providers and HIPAA privacy and security compliance. She is an adjunct professor of management at Clarkson University s Capital Regional Campus, teaching Legal Aspects of Health Care. Ms. Zambri is a graduate of Albany Law School and Union College, where she concurrently received her JD, cum laude, and MBA in health systems administration. Ms. Zambri is also a graduate of Siena College, where she graduated summa cum laude in finance. She was named as one of The Best Lawyers in America in the practice area of Health Care Law from
