2 PHEN Summit Program Thursday MR. EUNICE (ROBBIE) ROBINSON Mr. Robinson is the PHEN Support Group Coordinator and a four-and-a-half year prostate cancer survivor. In his current role with PHEN, he provides education awareness and outreach to men diagnosed with prostate cancer at the Organization s monthly support group sessions. He also recently participated in several PHEN dinner forums to bring education and awareness to men about the disease. In 2011, he participated in the Department Of Defense Consumer Review Process held annually to award funding to Doctors and Scientist in the area of prostate cancer research. Mr. Robinson is the building Service Manager for the Boston Public Schools. He received his bachelor of science in business from Norfolk State University, Norfolk, VA in MR. THOMAS A. FARRINGTON Thomas A. Farrington was diagnosed with prostate cancer in 2000, just four months following the death of his father from the disease. In 2001, he released "Battling The Killer Within." The book provided an insightful look at men facing prostate cancer and the prostate care system in the United States. It highlighted the harsh impact of this system on Black men. In 2003, Mr. Farrington founded the Prostate Health Education Network. Inc. (PHEN) a Boston, Massachusetts based non-profit organization to focus on the urgent and unmet prostate education and awareness needs of African-American men. In 2005, Mr. Farrington released his second book; Battling The Killer Within and Winning. Under Mr. Farrington s leadership, PHEN has taken innovative approaches to addressing the African American prostate cancer crisis which has proven effective in raising the visibility of the issue and gaining support and resources. PHEN efforts include the following: In 2005, PHEN organized and hosted the first ever African American Prostate Cancer Disparity Summit. This historic event was held in collaboration with U. S. Senator John Kerry (MA) and U. S. Congressman Gregory Meeks (NY), and hosted in the Rayburn House Office Building on Capitol Hill. PHEN is now recognized nationally as a leading prostate cancer advocate. PHEN was the catalyst for legislation introduced in the U. S. Congress by Senator Kerry and Representative Meeks for the federal government to recognize the prostate cancer crisis and provide additional resources to eliminate it. PHEN was also a leading voice advocating for congressional hearings on prostate cancer to bring clarity to government efforts addressing the disease. The US House of Representatives Oversight Committee, chaired by Representative Ed Towns (NY), held a hearing in March 2010 where Mr. Farrington testified. PHEN has mobilized a prostate cancer survivor network nationwide to outreach and help other men successfully face the disease. This survivor network is the foundation for much of PHEN s education and awareness outreach at the grassroots level. 2

3 In 2008, PHEN launched its national Rally Against Prostate Cancer (RAP Cancer). This initiative utilizes the PHEN survivor network; PHEN produced television programs available online, on DVDs and broadcast nationally over community television stations. PHEN also broadcasts regular radio programs in Boston and nationwide over the internet. On Father s Day 2010 PHEN implemented its first annual Father s Day Rally Against Prostate Cancer in partnership with churches across the country. The rally takes place within each church, and receives a high level of visibility through the media with a theme of Joining Hands in Prayer and Action to Save Lives. The church partnerships are used to reach, educate and support men and their families within the churches and their communities. Under Mr. Farrington s leadership, PHEN is succeeding in raising the visibility of the African American prostate cancer crisis, and has developed extensive education and awareness resources to be deployed with its survivor network and partners to support the educational needs of the men most at risk for prostate cancer. Mr. Farrington is a retired entrepreneur and business executive with more than 30 years experience in the Information Technology industry. In 1969, he founded Input Output Computer Services, Inc. where he served as president and chairman. This company was a pioneer in the evolution of IT professional services and the first technology company on the Black Enterprise top 100 listings. Mr. Farrington is a member of the board of trustees at the Dana-Farber Cancer Institute and serves as a member of the prostate cancer treatment guidelines panel of the National Comprehensive Cancer Network. Mr. Farrington was born and raised in Chapel Hill, NC. He attended North Carolina A&T State University and received a BS degree in electrical Engineering. Mr. Farrington has been married to Juarez Farrington for more than forty years, and they are the parents of three adult children and two grandsons. SENATOR JOHN KERRY (MA) John Kerry was born on December 11, 1943 at Fitzsimons Army Hospital in Aurora, Colorado. Not long after Kerry was born, his family returned home to Massachusetts, where his parents, Richard and Rosemary, taught him the values of service and responsibility and the blessings of his Catholic faith, lessons he carries with him to this day. As he was graduating from Yale University, John Kerry volunteered to serve in the United States Navy, because, as he later said, "it was the right thing to do." Lt. Kerry served two tours of duty in Vietnam. On his second tour, he volunteered to serve on a Swift Boat in the river deltas, one of the most dangerous assignments of the war. He was decorated with a Silver Star, a Bronze Star with Combat V, and three Purple Hearts. When he came home to the United States, John Kerry spoke out against a policy he felt gave politicians political cover while soldiers bore the real burden. He also began a lifelong fight for his fellow veterans joining with other vets to found the Vietnam Veterans of America to fight for veterans benefits, for extension of the G.I. Bill for Higher Education, and for treatment of PTSD. 3

4 Later, John Kerry accepted another tour of duty - to serve in America's communities. After graduating from Boston College Law School in 1976, he went to work as a top prosecutor in Middlesex County, Massachusetts. He took on organized crime, fought for victims' rights and created programs for rape counseling. John Kerry was elected Lieutenant Governor in Two years later, he was elected to the United States Senate and he has won reelection four-times since. He is now serving his fifth term. John Kerry entered the Senate with a reputation as a man of conviction. He helped provide health insurance for millions of low-income children. He has fought to improve public education, protect our natural environment, and strengthen our economy. From his ground-breaking work on the Iran-Contra scandal to his leadership on global AIDS, John Kerry has distinguished himself as one of our nation's most respected voices on national security and international affairs. As chairman of the Senate Select Committee on POW/MIA Affairs, he worked to learn the truth about American soldiers missing in Vietnam and to normalize relations with that country. As the ranking Democrat on the East Asian and Pacific Affairs Subcommittee, he is a leading expert on that region, including North Korea. He worked on a bipartisan basis to craft the American response to September 11th and has been a leading voice on American policy in Iraq and Afghanistan, the war on terrorism, the Middle East peace process and Israel's security. In 2002, John Kerry announced that he would be a candidate for president of the United States -- and he went on to mount a come from behind campaign that won the Democratic nomination, and was nearly elected President in a close contest against a wartime president. Kerry returned to the Senate, where today he continues fighting for what motivated him to enter public life in the first place: love of country. He is the Chairman of the Senate Foreign Relations Committee, the very committee he testified before in 1971 as a veteran pushing for an end to the Vietnam War. Under Kerry s leadership, the committee is addressing the key foreign policy and national security issues facing the United States including Afghanistan and Pakistan, nuclear nonproliferation, and global climate change. Today, Senator Kerry is the tenth most senior Senator and the second longest serving Senator in his seat. He also holds senior positions on the Finance, Commerce, and Small Business Committees. In his life of public service, John Kerry is sustained by his loving family. He is married to Teresa Heinz Kerry, and they have a blended family that includes two daughters, three sons, and two grandchildren. 4

5 SENATOR BEN CARDIN (MD) Ben Cardin believes government has a duty to improve the lives of all Americans. From expanding access to health care to boosting job growth to making home ownership affordable to improving the environment, Senator Cardin is a recognized leader with a record of legislative success. First elected to the U.S. Senate in 2006, he currently serves on the Finance, Budget, Foreign Relations, Environment and Public Works, and Small Business committees. The Washington Post has said Ben Cardin has a command of issues, proven integrity, formidable intellect and an unstinting work ethic. Senator Cardin believes access to quality, affordable health care is a right. That s why he supported the Affordable Care Act, which will provide more than 30 million uninsured Americans with health coverage. In the wake of the death of a 12-year-old Marylander from an untreated tooth infection, he successfully fought to get a guaranteed dental benefit included in the Children s Health Insurance Program. Senator Cardin also made sure the American Recovery and Reinvestment Act included his provision to provide first-time home buyers with an $8,000 tax credit along with his provision to help small businesses obtain larger contracts by raising the cap on surety bonds. He also is a strong champion of protecting our air and water. He has introduced legislation to restore the health of America s great water bodies including the Chesapeake Bay and, he has fought for funding to improve America s aging water infrastructure system, which would create thousands of new jobs. In the Senate, Ben Cardin brings the expertise he developed while serving in the House of Representatives. Known for his command of pension and health care legislation, many of his financial proposals have been enacted into law, including: increasing the amount Americans can save for retirement and expanding Medicare to include preventive benefits. CONGRESSMAN ELIJAH E. CUMMINGS (MD) Congressman Elijah E. Cummings was born and raised in Baltimore, Maryland, where he still resides today. He obtained his Bachelor s Degree in Political Science from Howard University, serving as Student Government President and graduating Phi Beta Kappa, and then graduated from the University of Maryland School of Law. Congressman Cummings has also received seven honorary doctoral degrees from Universities throughout the nation. Congressman Cummings has dedicated his life of service to uplifting and empowering the people he is sworn to represent. He began his career of public service in the Maryland House of Delegates, where he served for 14 years and became the first African American in Maryland history to be named Speaker Pro Tem. Since 1996, Congressman Cummings has proudly represented Maryland s 7th Congressional District in the U.S. House of Representatives. 5

6 Congressman Cummings often says that our children are the living messages that we send to a future we will never see. In that vein, he is committed to ensuring that our next generation has access to quality healthcare and education, clean air and water, and a strong economy defined by fiscal responsibility. Congressman Cummings currently serves as the Ranking Member of the Committee on Oversight and Government Reform. As the main investigative committee in the House of Representatives, Oversight and Government Reform has jurisdiction to investigate any federal program and any matter with federal policy implications. As the Committee s Ranking Member, Congressman Cummings fights to hold the Presidential Administration to a high standard of excellence and to ensure efficiency and effectiveness in the actions of the government of the United States. He also seeks to identify appropriate reforms that prevent waste, fraud and abuse and that ensure government programs meet the needs of the American people. Congressman Cummings is also a senior member of the House Committee on Transportation and Infrastructure, serving on both the Subcommittee on Coast Guard and Maritime Transport and the Subcommittee on Highways and Transit. Congressman Cummings has consistently been an advocate for the rights of those facing foreclosure and holds regular Foreclosure Prevention Seminars for people who are at risk of being foreclosed upon. The homeowners are matched with lenders to work out loan modifications on site to prevent foreclosure. He began the 112th Congress by requesting that the first hearing in the House Committee on Oversight and Government Reform investigate the causes of, and potential solutions to, the foreclosure crisis. Congressman Cummings serves on numerous boards and commissions. He is spearheading an effort to strengthen the Maritime curriculum at the Maritime Academy in Baltimore. He is the Chairman of the Maritime for Primary and Secondary Education Coalition (MPSEC). He also serves on the U.S. Naval Academy Board of Visitors, the Morgan State University Board of Regents, the Baltimore Aquarium Board of Trustees, and the Baltimore Area Council of the Boy Scouts of America Board of Directors. He is an honorary Board member of the SEED School of Maryland, KIPP Baltimore Schools, and the Baltimore School for the Arts. He has also received seven honorary doctoral degrees. Congressman Cummings is an active member of New Psalmist Baptist Church and is married to Dr. Maya Rockeymoore Cummings. J. JACQUES CARTER, MD, MPH Dr. Carter currently serves as a senior physician at the Beth Israel Deaconess Medical Center in Boston and is an assistant professor of medicine at Harvard Medical School. He also serves as the Medical Director of the Prostate Cancer Screening and Education Program at the Dana- Farber Cancer Institute in Boston. He has earned degrees from Howard and George Washington Universities and received his medical degree from Georgetown University, all in Washington, D.C. Dr. Carter completed his residency training in Internal Medicine at Beth Israel Deaconess Medical Center in Boston, followed by a graduate program at the Harvard School of Public 6

7 Health, where he received his Master of Public Health (MPH) degree. He then completed a clinical fellowship in Primary Care Medicine at the Massachusetts General Hospital. Since completing his training, Dr. Carter has held a number of clinical and administrative positions, including Medical Directorships of several local and national health care organizations. A former director of one of the major clinical clerkships, he now serves as a teacher, advisor and mentor for students at Harvard Medical School and the Harvard School of Public Health. Dr. Carter has been active in a number of civic and community organizations, including past president of the board of Family Service of Greater Boston and past chairman of the Brookline Advisory Council on Public Health. He regularly gives talks on medical and health related issues to community groups and students throughout the greater Boston area. He also lectures nationally and internationally on medical and public health topics. Dr. Carter serves as a medical consultant and resource for members of the print and electronic media. He is a past President of the Harvard School of Public Health Alumni Association and a current director of the Harvard Alumni Association (HAA). Dr. Carter is a founding member of the Georgetown University African American Advisory Board. He is the recipient of the 2010 Harvard Medical School/Harvard School of Dental Medicine Community Service Lifetime Achievement Award. His biography has been included in Who s Who in the East, "Who's Who in Medicine and Healthcare", Who s Who in Science and Engineering, and "Who's Who in America". JAMES L. MOHLER, MD James L. Mohler, MD, is Associate Director and Senior Vice President for Translational Research, Chair of the Department of Urology, Founder of the Prostate Program, and Professor of Oncology, at Roswell Park Cancer Institute, Professor of Urology at the University at Buffalo (UB) School of Medicine and Biomedical Sciences, and Adjunct Professor of Surgery and Member, UNC-Lineberger Comprehensive Cancer Center at University of North Carolina. Dr. Mohler earned his medical degree from the Medical College of Georgia and competed residency training in Surgery and Urology at the University of Kentucky Medical Center and a research fellowship in Urologic Oncology at The Johns Hopkins University School of Medicine. Dr. Mohler is licensed by New York and North Carolina, a Diplomat of the National Board of Medical Examiners and the American Board of Urology, and a Fellow of the American College of Surgeons. Dr. Mohler is Chair of the NCCN Guidelines Panel for Prostate Cancer, Vice-Chair of the Urology Committee of the Alliance for Clinical Trials in Oncology (formerly CALGB), and Past-President of the Society for Basic Urologic Research and is a member of the American Medical Association, American Association for the Advancement of Science, American Association for Cancer Research, American Urological Association and American College of Surgeons. 7

8 Dr. Mohler s clinical practice focuses upon prostate cancer and robot-assisted laparoscopic surgery. His laboratory focuses upon the role of the androgen receptor in racial differences in prostate cancer aggressiveness and prostate cancer recurrence during androgen deprivation therapy. He has authored or co-authored more than 200 publications and book chapters and a book Androgen Action in Prostate Cancer. He serves on the editorial boards of The Prostate, Journal of the National Comprehensive Cancer Network, and Therapeutic Advances in Urology, and reviews for several journals including Cancer, Cancer Research, Clinical Cancer Research, Journal of Molecular Endocrinology, Journal of Urology, Science Translational Medicine, Oncogene and Urology. HAROLD C. SOX, MD Harold C. Sox, M.D. graduated from Stanford University (B.S. physics) and Harvard Medical School (cum laude). After serving as a medical intern and resident at Massachusetts General Hospital, he spent two years doing research in immunology at the National Institutes of Health and three years at Dartmouth Medical School, where he served as chief medical resident and began his studies of medical decision making. He then spent fifteen years on the faculty of Stanford University School of Medicine, where he was the chief of the division of general internal medicine and served as director of ambulatory care at the Palo Alto VA Medical Center. In 1988 he returned to Dartmouth as chair of the department of medicine. He was the Editor of Annals of Internal Medicine from , and is now Professor of Medicine and of The Dartmouth Institute at Dartmouth Medical School. Dr. Sox was the President of the American College of Physicians during He chaired the U.S. Preventive Services Task Force from 1990 to 1995, the Institute of Medicine Committee to Study HIV Transmission through Blood Products, and the Institute of Medicine Committee on Health Effects Associated with Exposures Experienced in the Gulf War. In 2009, he co-chaired the Institute of Medicine committee to set national priorities for Comparative Effectiveness Research. He chaired the Medicare Coverage Advisory Committee of the Center for Medicare Services from 1999 to 2003 and served on the Report Review Committee of the National Research Council from 2000 to He also chairs the National Advisory Committee for the Robert Wood Johnson Foundation Physician Faculty Scholars Program. He serves on the Board of Directors of the Foundation for Informed Medical Decision Making and the Stakeholders Advisory Committee for the AHRQ Effective Health Program. He was elected to the Institute of Medicine of the National Academies in 1993 and to fellowship in the American Association for the Advancement of Science in His books include Medical Decision Making, Common Diagnostic Tests: Selection and Interpretation, and HIV and the Blood Supply: an Analysis of Crisis Decision-making. PATRICK WALSH, MD Dr. Walsh is best known for his 30 years as the Professor and Director of the Brady Urological Institute ( ) and for his pioneering work in the development of the anatomic approach to radical prostatectomy, which involves nerve-sparing techniques that have reduced the probability of impotence and incontinence. He has also made major contributions to the basic understanding of benign and malignant neoplasms of the prostate. Along with co-workers, 8

9 he was the first to describe the 5 alpha-reductase enzyme deficiency, to develop an experimental technique for the induction of benign prostatic hyperplasia, to demonstrate the influence of reversible androgen deprivation on BPH, and to characterize hereditary prostatic cancer. He is on the editorial board of the New England Journal of Medicine and is a member of the Institute of Medicine of the National Academy of Sciences. For 25 years he was the editor-inchief of Campbell s Textbook of Urology, which has been renamed Campbell Walsh in his honor. In 1996, Dr. Walsh received the Charles F. Kettering Medal from the General Motors Cancer Research Foundation for the most outstanding recent contributions to the diagnosis and treatment of cancer. Dr. Walsh was honored as the 2007 National Physician of the Year for Clinical Excellence by America's Top Doctors, and was the co-recipient of the 2007 King Faisal International Prize in Medicine. Together with Janet F. Worthington, he authored the best-selling books for lay people The Prostate: A Guide for Men and the Women Who Love Them, which is published by Johns Hopkins University Press (1995) and Warner Books (1997) and more recently, Dr. Patrick Walsh s Guide to Surviving Prostate Cancer, Warner Books (2001 and 2007). Dr. Walsh served as the president of both the American Association of Genitourinary Surgeons and the Clinical Society of Genitourinary Surgeons. MR. SCOTT T. WILLIAMS Scott Williams is currently Vice President of Men s Health Network (MHN), a Washington, DC based national non-profit organization whose mission is to reach men and their families where they live, work, play, and pray with health prevention messages and tools, screening programs, educational materials, advocacy opportunities, and patient navigation. Scott is actively sought out as a speaker and resource on men s health issues by the media, policymakers, public health professionals, physician key thought leaders, and other patient advocacy leaders. He has been featured as an expert in the New York Times, CNN, MSNBC, Washington Post, Baltimore Times, Medicare Reimbursement Weekly, Chicago Tribune, National Public Radio, St. Petersburg Times, Inside CMS, Dallas Morning News, Seattle Post-Intelligencer, among others. Scott has presented at the Food and Drug Administration (FDA), Massachusetts Medical Society, World Congress on Men s Health in Vienna, Austria, State Medicaid Pharmacy & Therapeutics Committees, and the United States Senate. He attended and spoke at health policy briefings and receptions at both the 2008 Democratic and Republican National Conventions. Scott is a contributing author to the American Journal of Men s Health, a SAGE Publication. He has recently been nominated to serve as the Membership Chair for the American Public Health Association s Community Health Planning and Policy Development Section. He is a representative to the Partnership to Fight Chronic Disease, MAPRx, Medicare Rx Education Network, and currently holds positions within the One Voice Against Cancer (OVAC) Budget and Communications Workgroups. He is President of the Maryland Men s Health Network 9

10 Board of Directors, member and former President of the Sigma Phi Epsilon Alumni Volunteer Corporation, and current member of the Moravian College Alumni Association Board. Prior to joining Men s Health Network, Scott was a Senior Analyst of Strategic Services at PharmaStrat, Inc. based in Flemington, NJ. He has also worked as Public Relations Manager for sanofipasteur in Swiftwater, PA. Scott received his BA in Political Science with Honors and a minor in Business Management from Moravian College in Bethlehem, PA. CHILEDUM AHAGHOTU, MD, PhD Dr. Chiledum Ahaghotu is a tenured professor of surgery and chief of urological services at Howard University Hospital. He is published widely on the genetics and natural history of prostate cancer including the African-American Hereditary prostate cancer study network. He has served in regional and national leadership roles including President of the Washington Urologic Society, the urology section of the National Medical Association, and CMS's Practicing Physicians Advisory Council. Dr. Ahaghotu currently serves as a consultant to the American Urologic Association's Health Policy Council. JASON ALTER, PhD Dr. Jason Alter joined Iris Personalized Medicine in December 2011 as Vice President of Marketing. He is responsible for marketing NADiAProsVue, an in vitro diagnostic assay for determining very precisely how fast prostate specific antigen (PSA) rises over time postprostatectomy. After robotic or open-radical prostate cancer surgery, NADiAProsVue measures PSA concentrations at three time points at levels a thousand times lower (picograms) than standard tests (nanograms) and provides a rate of rise or slope. In conjunction with clinical evaluation, NADiAProsVue is indicated as a prognostic marker for identifying men at reduced risk for recurrence of prostate cancer (slope 2 pg/ml/month) for eight years after prostatectomy. Dr. Alter has had a wide-ranging career; prior to joining Iris, he was Vice President of Marketing at Aureon Biosciences and was involved in the commercialization of personalized prognostic tests for prostate cancer. In 2001, he joined IBM s newly-formed Life Sciences Division as a Worldwide Marketing Campaign Lead and was responsible for many of the initial outbound marketing activities of the business unit. Subsequently, as Program Marketing Manager and then the Manager of Program Marketing, he and his team were responsible for all outbound marketing for the IBM Life Sciences team. Dr. Alter holds a dual B.A. in Biology and History from Alfred University, an M.S. degree in Microbiology from Texas A&M University and a Ph.D. in Molecular Biology from Binghamton University. Dr. Alter did a postdoctoral fellowship at Schering Plough Pharmaceuticals. 10

11 H. JEFFERY LAWERENCE, MD H. Jeffrey Lawrence, MD received his undergraduate degree in Molecular Biophysics and Biochemistry at Yale University and his medical degree at Stanford University School of Medicine. He is board certified in Internal Medicine, Hematology and Medical Oncology. He was an academic clinician/investigator for over 25 years, most recently as a Professor of Medicine at the University of California, San Francisco (UCSF) from His clinical oncology practice at the San Francisco VA Medical Center included a large number of patients with advanced prostate cancer, and he was involved in early clinical studies of anti-hormonal therapies including goserelin (Zoladex) and bicalutamide (Casodex). During that time, Dr. Lawrence also conducted basic research focused on the molecular biology of leukemia and solid tumors, with research funding from the NIH, the VA and other sources, and has published over 80 peer-reviewed articles. He has served as a member of numerous NIH and VA grant review study sections, and on the editorial boards of several medical journals. He also served as Chairman of the Board of the Northern California Institute for Research & Education, the largest VA-affiliated research institute in the United States. Dr. Lawrence joined Roche Molecular Systems in Pleasanton, California where he was the senior director in Genomics and Oncology, where he oversaw the clinical development studies for molecular companion diagnostic assays used to select patients for novel targeted cancer treatments, including the first-in-class BRAF inhibitor vemurafenib (Zelboraf) for the treatment of metastatic melanoma. In 2012, Dr. Lawrence joined Genomic Health, Inc. as Senior Director in the Oncology Department, where he now leads clinical development studies for the prostate program. PAMELA J. REED, MS Pamela Reed is currently the Product Manager for Prostate Disease and Tumor Markers at Beckman Coulter, Inc. Beckman Coulter develops, manufactures and markets diagnostic systems that automate complex biomedical testing. Pamela has more than 18 years of experience leading the successful innovation, patenting, commercialization, and market success of technologies for the medical device and diagnostics industries. Pam co-invented new antimicrobial technologies while working at Ecolab, Inc. in St. Paul, MN, managed laboratories for biosafety testing, both nationally and internationally, and most recently, has managed the market development of new medical diagnostic products for cancer detection at Beckman Coulter. Pam holds a Bachelor of Science in Molecular Biology from the University of Wisconsin-Madison and a Master of Science in Cellular and Integrative Physiology from the University of Minnesota. CONGRESSMAN GREGORY W. MEEKS (NY) Congressman Gregory W. Meeks represents New York's Sixth Congressional District in the United States House of Representatives. Now serving in his sixth full-term, Meeks was elected to the House on February 3, He is a member of the powerful House Committee on Financial Services and the much sought after Committee on Foreign Affairs. Meeks is a Member of the Congressional Black Caucus (CBC), New Democrats Caucus and the Democratic Leadership Council (DLC). He serves as the Co-chair of the Malaysia Caucus, Services Caucus, Dialogue Caucus and the Middle East Economic Partnership Caucus. 11

12 As a Member of Congress, Meeks has championed causes and sponsored legislation that: addresses increased educational opportunities for all children, ensures that minority businesses and contractors significantly participate on all projects that receive federal funding, trade expansion, protection of civil and human rights, and promotes healthy livable communities. Representing a district of mostly middle-class working people, Meeks has been a strong advocate for working families and the poor. He has been praised for his advocacy to protect worker s rights to organize, strengthen workplace protection laws, and making laws that create jobs that produce a livable wage. For his strong leadership, Congressman Gregory W. Meeks (06-NY) was honored by the New York Roadway Coalition as its 2006 Legislator of the Year. This annual prestigious award is given to a New York Democrat and Republican for their work on infrastructure issues important to the state. This is the second time in Meeks congressional tenure that he was bestowed this award, becoming the first Member of the congressional delegation to receive the award twice in less than 10 years. In 1978, he received his Juris Doctorate from Howard University School of Law. After law school, he joined the Queens County District Attorney's Office. He next worked at the Office of the Special Narcotics Prosecutor for the City of New York. Meeks then became Assistant Council for the State Investigation Commission, where his responsibilities were to direct investigations. He was then appointed Supervising Judge of the New York State Worker's Compensation System. He held this position until 1992 when he was elected to the New York State Assembly, where he served for five years. He is the former Chair of the Council of Black Elected Democrats (COBED) and has successfully worked with a bipartisan group of his colleagues to dialogue about issues of importance to the United States and the international community. Meeks is a founding member of the Thurgood Marshall Regular Democratic Club and remains active in grassroots community organizing. Meeks is also a member of several civic and professional organizations including: the National Bar Association, the Coalition of 100 Black Men, the National Association for the Advancement of Colored People (NAACP), United Black Men of Queens, the National Black Caucus of State Legislators Inc., the Alpha Phi Alpha fraternity and United Black Men of Queens. He is also a member of the Allen AME Church. Congressman Meeks currently resides in St. Albans, Queens with his wife Simone-Marie and has three daughters Ebony, Aja, and Nia-Aiyana. DEEPAK KAPOOR, MD Dr. Kapoor is one of the youngest physicians to have been certified by the American Board of Urology and comes to IMP with over 20 years of clinical and business experience. He received his B.S. magna cum laude from the Pennsylvania State University and his M.D. from Jefferson Medical College, in the Accelerated Five Year Honors Program. He completed his residency at Geisinger Medical Center, after which he served as Assistant Professor of Urologic Surgery at 12

13 the University of Minnesota Medical Center. His medical background is diverse with both laboratory and clinical experience, both in the academic and private sectors. Dr. Kapoor s expertise includes basic science research in molecular biology as well as extensive experience in oncologic and reconstructive surgery. Presently, Dr. Kapoor is President of the Large Urology Group Practice Association, is Chairman of Access to Integrated Cancer Care (an informal advocacy group representing the rights of patients to access integrated services of the highest quality), Chairman of SCRUBS RRG (the only national Urology specific medical malpractice carrier), is a member of the Board of Directors of Allied Urological Services, ( the largest lithotripsy partnership in the United States, where he also functions as Chairman of the Finance Committee) and is Founder and Past- President of the Integrated Medical Foundation. Prior to his tenure with IMP, Dr. Kapoor functioned as managing partner of Long Island Urological Associates, during which time LIUA enjoyed double digit growth in each year of his leadership despite declining medical reimbursements nationally. Dr. Kapoor has published and lectured extensively on both clinical and business medical issues, and serves on a number of medical advisory boards, including the New York State Governor s Prostate Cancer Advisory Panel. He continues to enjoy an active clinical practice along with his administrative duties with IMP. 13

14 PHEN Summit Program Thursday Afternoon BISHOP MICHAEL V. KELSEY, SR. Michael Vernon Kelsey, Sr. is a native Washingtonian. He accepted Jesus Christ as Lord and Savior on March 31, 1977 and was licensed to preach the Gospel shortly thereafter. Bishop Kelsey received his Bachelor of Arts Degree from the University of Maryland in College Park and went on to receive his formal theological training at Virginia Union University School of Theology in Richmond, Virginia. There he earned the Master of Divinity Degree, Magna Cum Laude. Bishop Kelsey has also received and earned a Doctor of Ministry Degree from The United Theological Seminary in Dayton, Ohio. He later continued his postgraduate studies at Oxford University in Oxfordshire, England. During the past three decades, he has pastored two churches in the Richmond, Virginia area; and for the past seventeen years served as the Senior Pastor of the New Samaritan Baptist Church, one of the leading churches in the Metropolitan Washington area. Bishop Kelsey is committed to making a difference in the community and is an active participant in a variety of faith and community based organizations. He was appointed by Mayor Anthony Williams to the Mayor s Interfaith Council of Washington, DC where he served as the Chair of the Council. He currently serves as the Chair of the Board of Directors of SamariCorp Community Development Corporation and is a member of the Gallaudet Community Relations Council. He works closely with city officials on issues of crime prevention and economic development in Ward 5 and has been invited by President Obama s Administration to participate in many significant White House briefings. Known for his integrity and emphasis on order, Bishop Kelsey currently serves on the Executive Council of the Full Gospel Baptist Church Fellowship, International, as the International Executive Treasurer. As the former State Bishop of Washington DC (2000 to 2011), he was responsible for overseeing the work of the Fellowship among the pastors and churches in the District of Columbia, as well as Prince George s and Montgomery Counties, Maryland. He is blessed to serve under Bishop Paul S. Morton, Sr., International Presiding Bishop, who is his spiritual father in the ministry. Bishop Kelsey is the founder and CEO of Kelsey Ministries. His ministry is international in scope. He has ministered the Word of God in many foreign countries, including: Ghana and Liberia in West Africa, the Ukraine in Eastern Europe; the Sea of Galilee in Israel; Havana, Cuba; London, England; and Ramstein, Germany. 15 MR. MICHAEL HAYNES As a spokesperson for the Know Your Stats About Prostate Cancer Campaign, Michael Haynes is committed to sharing the message of early prostate cancer detection. Haynes discovered he had prostate cancer in a 2008 after attending an event conducted by the AUA Foundation and supported by the National Football League (NFL) Player Care Foundation. Now a prostate cancer survivor, he speaks on behalf of the Foundation, encouraging men to talk with their doctors about getting tested for this deadly disease. Haynes professional football career began when he was the fifth player selected in the 1976 draft. A first-round draft choice of the New England Patriots, Haynes was voted the NFL defensive rookie of the year. He was selected by his peers to represent the AFC in nine Pro Bowls during his 14-year NFL career with the Patriots ( ) and Los Angeles Raiders ( ) and earned a Super Bowl ring as a member of the 1983 Raiders. Haynes is widely recognized as one of the NFL s greatest defensive backs. He was inducted into the Patriots Hall of Fame in 1994, named to the NFL s 75th Anniversary All-Time Team in 1995 and inducted into the Pro Football Hall of Fame in In 2009, Haynes and his former Raiders teammate, Lester Hayes, were named the best cornerback tandem in NFL history by the NFL Network program NFL s Top 10. During his playing career, Haynes recognized the need to prepare for life after football and began to establish the groundwork for his career off the field. He spent a distinguished seven years at Callaway Golf, a global leader in the development and manufacture of golf equipment, with six years as their Global Licensing Manager prior to being promoted to Vice President of Recreational Golf Development. He returned to the NFL in 2002 where he led the Player Development Department, which created and managed programs to help players transition into and out of the NFL. Haynes also held the position of Special Advisor to the Commissioner of the NFL, responsible for providing counsel on player issues. He is now the President and founder of Mike Haynes & Associates (MH&A), a consulting company with more than 35 years of experience that takes a holistic approach to problem solving. MH&A works with clients that demonstrate a commitment to a winning culture, seek a competitive edge and value making a difference in our shared community. Giving back to the community plays a large role in Haynes philosophy. He is a passionate advocate for issues that often times break barriers and bring about meaningful improvements and change. He has received numerous honors for following his desire to make a positive contribution in the world. Recently, Haynes received the Gilder Radner Courage Award from the Roswell Park Alliance Foundation for his willingness to use his personal experience with prostate cancer to encourage men to learn about prostate health. More recently, he was inducted into the John McLendon Minority Athletics Administrators Hall of Fame, which was founded by the National Association of Collegiate Directors of Athletics to recognize outstanding accomplishments in sports administration. Haynes was honored for his personal decorum while becoming a role model to others and rising to the highest level of excellence in his chosen career. 15

16 ARTIE L. SHELTON, MD Dr. Shelton is a nine-year prostate cancer survivor and advocate for the Prostate Health Education Network. Dr. Shelton was appointed by Governor O Malley to serve a three -year term as a member of the Maryland Council on Cancer Control and co-chaired writing the prostate chapter for the Maryland Comprehensive Cancer Control Plan Executive Summary. Dr. Shelton has served on the Department of Defense (DoD) Congressional Directed Medical Research Program (CDMRP) as a consumer reviewer/advocate and on the consumer work group. Dr. Shelton states that during his personal venture, with prostate cancer, he became aware of the magnitude of this disease that is impacting all men, but especially on African American men. Prostate cancer, particularly among African Americans, is a disgraceful tragedy that needs immediate and drastic action. As PHEN, all health organizations must take the lead and lead by example in the effort to combat this disease that can be cured if diagnosis early. Faith-based and other organizations need to take advantage of their leadership roles and position in society to raise public awareness in the fight against prostate cancer. Dr. Shelton received his BS, MS, and MD degrees from Howard University. He did his internship and residency training at Walter Reed Army Medical Center. He received his fellowship training in Adult/Pediatric Allergy and Immunology at National Institute of Health, Children s Hospital and Walter Reed Army Medical Center. Colonel (Ret) Shelton served 21 year s in the U.S. Army with various assignments in CONUS and overseas.. He served as Commander, Pentagon Army Health Clinic; Chief Department of Primary Care and Community Medicine. Appointments as the foreign travel medical escort physician to the Congressional and Senate leadership and personal physician to the Army s Chief of Staff have distinguished Dr. Shelton s military career In addition, he served ad PROFIS Commander 42nd Field Hospital, Fort Knox KY. In 1993, he deployed to Mogadishu Somalia where he served as the 42nd Medical Task Force Commander. COL Shelton is the recipient of numerous military awards to include the Distinguished Service Medal, Legion of Merit (2 Oak Leaf) and Meritorious Service Medal (3 Oak Lea). Dr. Shelton is President /CEO Shelton Professional Services. MR. DALE BELL Dale Bell directs activities whereby African American men are provided education and awareness for proper prostate care. He is certified by the American Cancer Society as a facilitator in prostate health and a leader of the Brother to Brother Prostate Support & Information Group in Westchester County, New York. He is associated with national and local prostate health organizations which prepares him to lead discussions on these issues within various men and women groups, while providing support to 16

17 individuals and their families. Dale understands many of challenges accompanying prostate cancer; he is a prostate cancer survivor of thirteen years. After graduating from Albany State University in Albany, Georgia with a Bachelor of Business Administration degree, he enjoyed a thirty-four year career at the International Business Machines Corporation (IBM) before retiring in While a member of the U.S. Army, Dale completed tours in the United States and South Viet Nam. Dale serves as a member of the Deacon s Ministry at Grace Baptist Church in Mount Vernon, New York. Dale lives with his wife of forty-three years, the former Henrimae Henderson in New Rochelle, New York. REVEREND DR. FRANK D. TUCKER The Reverend Frank D. Tucker was born in Blackstone, Virginia. He attended the Virginia Public Schools and graduated from Forster High School in He received his BA Degree in Sociology/Philosophy in June 1961 from Howard University. In June 1964, he received his Bachelor of Divinity Degree from Howard University s School of Religion. Rev. Tucker was College Minister and Assistant Professor of Theology at Florida Memorial College from 1965 to He was then called to Pastor the Jones Memorial Baptist Church in Philadelphia, Pennsylvania from 1966 to On February 1, 1976, Rev. Tucker became Pastor of the First Baptist Church, located at New Hampshire Avenue and Randolph Street Northwest, Washington, D. C. In 1986, he joined the faculty at the Howard University School of Divinity as adjunct professor. Rev. Tucker is affiliated with many community and religious groups. He is a member of the Baptist Ministers Conference of Washington, DC; Executive Committee of the Foreign Mission Board of the National Baptist Convention, USA, Inc.; and is the Facilitator for the Wednesday Clergy Fellowship. He formerly served on the Board of Directors of the Washington Urban League; the Interfaith Conference of Metropolitan and as President of the Baptist Convention of DC & Vicinity; former Chairman of the Board and CEO for the Church Association for Community Services; the Board of Directors of the United Planning Organization, and Former Chairman of the Mayor s Faith Advisory Council. Rev. Tucker is the recipient of many recognitions and awards, some of which are: The Man of the Year Award for Humanitarian Service (1992); the Martin Luther King, Jr. Humanitarian Service Award (1993); the New England Youth Workshop Century Award (1999); the CACS Faith Award, Tenth Anniversary of CACS (1999); the Judy Halpern Award for making a difference in the World by Religious Coalition for Reproductive Choice (1999); and, the Prophet for a New Millennium Award-Honoring the Integrity and the Power of Prophetic Preaching (1999) presented by the Institute of Great Preaching. Howard is an advocate & supporter of men's health. SONJA WILSON, EdD Sonja M. Wilson, EdD, is an Assistant Professor at North Carolina Agricultural and Technical State University, School of Nursing. Dr. Wilson serves on PHEN's Medical Advisory Board, and has been a member of PHEN since She has been a past presenter at PHEN's Annual Prostate Cancer Disparity Summit. Dr. Wilson is a member of Mitchell Chapel AME Zion Church in Pittsboro, NC. She and her sister, Ms. Lillian Alston, have successfully led prostate cancer education efforts within the church, their town, and within their AME Zion church district. MR. JERRY L. WOMACK Mr. Womack is an Advocate & Prostate Cancer Survivor for the Prostate Health Education Network (PHEN). He is completing his eighth year as a Prostate Cancer Survivor. He is a 10 year veteran of the U.S. Air Force where he served as a Jet Engine Technician and spent a two year tour in England where he played football in the U.S.A.F. E. (U.S. Air Force Europe) league. After leaving the military, he served as V.P. of Sales and Operations for one of New England's leading service companies and is now a Regional Sales Executive for America's largest manufacturer of commercial cleaning products. Combining his strong belief in God and his roots in faith based services, he has committed himself to working within the church circuit as an extension of the ministry of help to men and their families. Given an opportunity to extend his life through early detection of prostate cancer, he set goals of being part of the educating of the community on Prostate Cancer awareness. Mr. Womack is one of four intermediate family members that were diagnosed with prostate cancer. Knowing the risk of Prostate Cancer, he has extended his family to all those who will listen and receive the message of early detection to include many of the men and their families with whom he joins in weekly worship.he wears the blue ribbon proudly and gladly tells his story of life after prostate cancer detection, a message of healing and hope with and through Christ. 18

GILLIAN MULLINS Gillian Mullins is the Managing Director of Alliance Development at the Biotechnology Industry Organization (BIO), a national trade association representing approximately 1,100 biotechnology companies, located in Washington D.C. In this capacity she oversees BIO's external relationships with allied organizations, including: patient advocacy organizations, physician and payer groups, universities, venture capital organizations and the state-based bioscience trade groups that comprise the Council of State Biotechnology Association (CSBA). Gillian received her B.A. in Political Science from Miami University located in Oxford, OH. She is a native of Washington, D.C. and resides there with her husband John Mullins, CPA, and their daughter Keaveny Mullins, age 2. JANICE KING, PhD Dr. Janice King joined Medivation, Inc as a Clinical Scientist in April 2008, where she worked on the development of Dimebon for Alzheimer s disease. In her current role, she supports Medical Affairs and Clinical Development programs for Enzalutamide in the treatment of advanced prostate cancer. Prior to joining Medivation, Dr. King held positions of increasing responsibility at Johnson & Johnson where she ultimately led a field based team supporting Procrit across multiple indications. Her industry career was preceded by several years of research and regulatory experience obtained from positions held at both the National Institutes of Health and the Food and Drug Administration. Dr. King received her BS degree in Pharmacy, summa cum laude, from Howard University and PhD in Pharmaceutics from the State University of New York, Buffalo. MR. THOMAS SELLERS Tom Sellers is a 13-year cancer survivor whose life mission is to ensure the best possible quality of life for all cancer survivors. Since his mother s death from lung cancer in 1980, he has spent years relentlessly pursuing that mission in both his private and professional life. Since September 2011 Mr. Sellers has been Senior Director, Patient Advocacy and Corporate Philanthropy, at Millennium: The Takeda Oncology Company. He is responsible for leadership, strategy, and management of sustainable relationships with the patient community. He also oversees philanthropy and programs ensuring patients, families and caregivers have accurate, up-to-date information and access to appropriate treatments. Previously, Mr. Sellers was President and Chief Executive Officer of the National Coalition for Cancer Survivorship. There he advanced the cause of quality cancer care for all people touched by the disease through strategic partnerships with national standard setting bodies and collaboration with other advocates, policymakers, and key opinion leaders. 19

20 Prior to NCCS, Mr. Sellers capped a 14-year career as CFO of the American Cancer Society s New England Division by leading fundraising, community relations, design, and construction for a $30 million ACS Hope Lodge in Boston to provide free lodging and services to more than 1,000 cancer patients in treatment annually. Mr. Sellers came to the ACS with more than 20 years of experience in public service and non-profit management. He is also active in the Massachusetts cancer community, serving on the board member of the Schwartz Center for Compassionate Healthcare and as a Trustee of Dana-Farber Cancer Institute. He is an Affiliate Member of the American Society of Clinical Oncology. Mr. Sellers attended Amherst College and has a master's degree in public administration from the Harvard Kennedy School. Mr. Sellers has been personally touched by cancer on many levels. In addition to his mother, his father succumbed to an inoperable brain cancer. Mr. Sellers was a caregiver for both his parents and was diagnosed with and successfully treated for prostate cancer in Mr. Sellers is married with two grown daughters, a grandson, and a passion for golf. OLGA BANDMAN, MD Dr. Olga Bandman held positions of increasing responsibility at ARYx Pharmaceuticals, most recently serving as Associate Director, Medical Affairs. During this time, she designed and was a medical monitor on several phase 1, 2 and 3 trials in four different therapeutic indications. She was a Scientist in a team working on defining the use of transcript imaging in drug discovery focusing on immune system at Incyte Pharmaceutical. She conducted doctoral research on vaccine development at Mechnikov Research institute of Vaccines and Serums in Moscow, Russia. She received her M.D. from Sechenov Moscow Medical Academy and specialized in epidemiology and public health. MR. JOE I. DEPINTO Mr. DePinto currently serves as executive vice president of global commercial operations. Mr. DePinto most recently was vice president and product champion for Imclone Systems, Inc., a wholly-owned subsidiary of Eli Lilly & Company and previously served as vice president of U.S. sales and marketing at Abraxis Bioscience. Prior to that, Mr. DePinto served as vice president of commercial operations at ImClone Systems, Inc.; global Marketing Leader, Oncology Therapeutics for Johnson & Johnson Pharmaceutical Services, Inc.; and vice president, oncology sales at Ortho Biotech Products. Mr. DePinto earned his Bachelor of Science in Marketing and MBA in Pharmaceutical Chemical Studies from Fairleigh Dickinson University. 20
