2 September 12, 2001 PART TIME APPOINTMENTS MINISTRY OF HEALTH AND LONG-TERM CARE Cancer Care Ontario Tony Dagnone Nominated as a member Tony Dagnone, of London, was President and Chief Executive Officer of University Hospital since In October, 1995, he was named CEO of the amalgamated Victoria/ University Hospital, now London Health Sciences Centre. Prior to joining University Hospital, Mr. Dagnone spent 25 years at Royal University Hospital in Saskatoon, including 15 years as President and CEO and Director of the Hospital Foundation. Mr. Dagnone is a fellow in both the American and Canadian College of Health Services Executives. MINISTRY OF MUNICIPAL AFFAIRS AND HOUSING Ontario Municipal Employees Retirement Board Normand Wilfrid Fortin Nominated as a member Normand Wilfrid Fortin, of Brooklin, is the former Superintendent of Business and Finance with the York Catholic District School Board. He also served as Superintendent of Operations and Treasurer with the Kirkland Lake District Roman Catholic Separate School Board. Mr. Fortin served as a member of the Ontario Association of School Business Officials and as President of the Ontario Separate School Business Officials Association. MANAGEMENT BOARD OF CABINET Ontario Realty Corporation Board of Directors Malcolm L. Heins Richard Ling Malcolm L. Heins, of Oakville, is CEO of the Law Society of Upper Canada. He is the former President and Chief Executive Officer of the Lawyers' Professional Indemnity Company. Mr. Heins is also past President & Chief Operating Officer of Simcoe Erie Group/GAN Canada and a former partner in the law firm of Thompson Rogers. Page 1 of 4

He has been a Director of the Chinese Cultural Centre of Greater Toronto, Director and Legal Counsel of the Richmond Hill & Markham Chinese Business Association, Director of the Mon Sheong Foundation and Director of the St. Michael's Hospital Foundation Board. MINISTRY OF NORTHERN DEVELOPMENT AND MINES Northern Ontario Heritai:e Fund Corporation Board of Directors Jack Siemon Margaret Moody Marguerite Hoey Robert Manseau Christopher McKibbon Jack Siemon, of Kirkland Lake, is owner and General Manager of Goldbelt Tire and Supply Co. Ltd. He is Treasurer of the Kirkland Lake Rotary Club and a Director of the Kirkland Lake Physicians Recruitment and Retention Fund. Mr. Siemon is past Director and Chair of the Kirkland Lake Economic Development Corporation and past Director and President of the Temiskaming Development Fund. Margaret Moody, of Astorville, is Station Manager for Air Ontario in North Bay. She is also a member of the North Bay Rotary Club and the Nipissing and Area Breast Health Coalition Committee. Ms. Moody is a former member of the Economic Development Commission and a former member of the CNIB Steering Committee. Marguerite Hoey, of Sioux Lookout, is General Manger and President of Sioux Travel/Carson Wagonlit Travel. She is President of the Sioux Lookout District Health Foundation and Co Chair of the Sioux Lookout Anti-Racism Management Board. Ms Hoey is a member of the Sioux Lookout Chamber of Commerce Board of Directors and the Sioux Lookout District Health Centre Board of Governors. She is also Chair of the New Hospital Fundraising Campaign and a Board member of the New Hospital Planning Committee. Robert Manseau, of Timmins, is a Management Consultant and principal of Commerce Management Group. He is a former Vice-Chair of the Central Temiskaming Economic Development Corporation and a former member of the Ontario Board of Parole. Page 2 of 4

He is Co-Chair of the OMA's Physician's Services Committee and a member of the Committee on Hospitals. Dr. McKibbon also serves as a member of the Board of Directors, Huntington College, Laurentian University and Medical Director of the Northern Ontario Comprehensive Arthritis Initiative. He is Past President of the Sudbury and District Medical Society. MINISTRY OF TRAINING, COLLEGES AND UNIVERSITIES Ontario Educational Communications Authority (TVO) Ravi Seethapathy Mitzie J. Hunter Ravi Seethapathy, of North York, is an Audit Associate (Manager) with Hydro One Networks Inc. He is Director of Engineers without Borders and former President of the Indo Canada Chamber of Commerce. Mr. Seethapathy is currently the Vice-President and Corporate Director of the Canadian Club of Toronto. Mitzie H. Hunter, of Toronto, is President and C.E.O. of SMART Toronto Advanced Communications. She is a former Manager of Communications with Bell Canada and was also responsible for Bell Canada's municipal government relations. Ms Hunter is a member of Canadian Women in Communications and Co-Chair of the New Media Village Board of Directors. Selection Board (Ontario Graduate Scholarship Pro2ram) Dr. Lorraine Clark Dr. Martin Kreiswirth Dr. John Goyder Dr. Pascal Van Lieshout Dr. Lorraine Clark, of Peterborough, is an Associate Professor, English Literature at Trent University. She has previous experience as a member of an OGS Selection Panel. Professor Clark has been teaching for 15 years. She has received numerous research awards, published in many refereed journals and has served on a variety of committees, internally and externally. Page 3 of 4

He has previous experience as a member of an OGS Selection Panel and served as Chair from 1993 to Dr. Kreiswirth is Associate Dean, Faculty of Graduate Studies and has served on numerous committees, internally and externally. Dr. John Goyder, of Waterloo, is a Professor in the Department of Sociology at the University of Waterloo. He has previous experience as a member of an OGS Selection Panel and served as Chair from 1994 to Professor Goyder is Interim Co-Director of the University of Waterloo Survey Research Centre. Dr. Pascal Van Leishout, of Toronto, is an Assistant Professor, Department of Speech Language Pathology, Faculty of Medicine, University of Toronto. Dr. Van Leishout is a member of the International Phonetic Association and a member of the Faculty Council, Faculty of Medicine, University of Toronto. Page 4 of 4
