STUDENT MINDSET SURVEY This is NOT a test! It is an opinion survey. It asks your opinion about things to do with school and being a student. It is very important that you give your own opinion, not what someone else thinks. Read each statement. Decide how much you agree or disagree with the statement and circle your answer. Once completed, go back and fill in your profile numbers and add them up. Do you Agree or Disagree? Disagree a lot Disagree Disagree a little Agree a little Agree Agree a lot Profile Number 1. No matter how much intelligence you have, you can always change it a good amount You can learn new things, but you cannot really change your basic amount of intelligence I like school work best when it makes me think hard I like school work best when I can do it really well without too much trouble I like school work that I ll learn from even if I make a lot of mistakes I like school work best when I can do it perfectly without any mistakes When something is hard, it just makes me want to work more on it, not less To tell the truth, when I work hard at my schoolwork, it makes me feel like I m not very smart MINDSET ASSESSMENT PROFILE NUMBER
CREATING YOUR MINDSET ASSESMENT PROFILE DO YOU HAVE MORE OF A FIXED MINDSET OR A GROWTH MINDSET?
Now, find your group that includes your number in the chart below and circle it. Now read what it says about your MAP group. If your profile number falls into this range: Then your MAP (Mindset Assessment Profile) group is: People in this MAP group usually believe the following things: 8-12 F5 You strongly believe that your intelligence is fixed it F4 doesn t change much. If you can t perform perfectly, you d rather not do something. You think smart kids don t have to work hard F3 You lean toward thinking that your intelligence doesn t F2 change much. You prefer not to make mistakes if you can help it, and you also don t really like to put in a lot of work. You may think that learning may be easy F1 You haven t really decided for sure whether you can G1 change your intelligence. You care about your grades, and you also want to learn, but you don t really want to have to work too hard for it G2 You believe that your intelligence is something that you G3 can increase. You care about learning, and you are willing to work hard. You want to do well, but you think that it is more important to learn than to always score well G4 G5 You really feel sure that you can increase your intelligence by learning, and you like a challenge. You believe that the best way to learn is to work hard, and you don t mind making mistakes while you do it. Do you thing the description under your MAP group matches the way you think and fell about your school work? Which parts are true for you and which are not?

Something I say often (In advisory or class) Example: Great job! You really get this material! A better way to say it Example: Great job I can tell that hard work is paying off. You couldn t do (X) yesterday!
Name: Knowledge Quiz 1 Date: Directions: Answer each question as true or false. Circle your answer. 1. You have a certain amount of intelligence, and you can t really do much to change it. 2. People who achieve great things, like winning a gold medal or a Grammy award, are born with special abilities and rarely have to work as hard as the rest of us. 3. You can learn new things, but you can t really change your basic intelligence. 4. You can grow your brain.
Name: Knowledge Quiz 2 Date: 1. Circle one: Setting goals is a strategy that only works for some people. It s not helpful for everyone. 2. Your 15 year old cousin tells you that he wants to become a marathon runner. He s never run more than a mile without getting tired and quitting. What advice should you give him? Check ALL that apply. Go for it! There s a marathon in a few weeks. You should sign up! Running is boring. Play basketball instead. Go for it! You should start small and push yourself to run one mile. Then try to run two miles, then three miles etc. Talk to the high school track coach for tips on how to run ong distances. Write your goal down in a place where you can see it every day. 3. Your cousin makes running a marathon his goal. He plans to run every day after school, but when he goes home from school he s tired and wants to take a nap instead. After a few weeks, he gives up and decides he wouldn t be a good runner anyway. He feels like he just doesn t have what it takes. What advice should you give him? Check ALL that apply. Don t give up! Change your plan. Instead of going home after school, go straight to the park and go for a run. Running is boring. You should play basketball instead. Nobody is born a marathon runner. People become marathon runners through hard work and dedication. Keep trying! Find some people at school who like to run and see if they would want to run with you to help you train.
Name: Knowledge Quiz 3 Date: 1. Circle one: It is always better to figure something out yourself than to ask a teacher for help. 2. Your friend Alex has been working towards his short-term goal of getting an 80% or above on this English exam. He told you he made a plan of studying every Monday and Wednesday for an hour. He got a 67% on the exam and asks you for advice. What would you tell Alex? Check all that apply. Maybe English just isn t your subject. Focus your time on something else. How has your studying going on Mondays and Wednesdays? Have you found a good environment to study where you are actually getting stuff done? If not, try to make your environment work for you. It seems like your plan isn t working. You should come up with other strategies to help you get an 80% on the next exam. You still passed, don t worry about it. 3. Circle one: Taking notes in class is important. Notes help you study at home by reviewing them and using them to make study tools like flashcards.
