1 D.R I have a good sense of humour and like to have a joke with people. I enjoy my food! And I can eat for England! I Love trains and can tell you where the trains are going when they pass our home! I love going on day trips and I am very social able! I am cheeky and like to get away with not doing my chores around the house! I enjoy lots of company. I enjoy winding people up and having a joke with people. I like to go on day trips especially to new places. I love a shower and the odd bath! I like to collect leaflets and booklets and keep them in my walker. I enjoy helping out with household chores. I enjoy going out for meals. I like to be involved with all my decisions even if I just answer I don t know I do sometimes have a difficult relationship with MG and I need staff to support me with this. I can sometimes get very upset over sometimes something quite small but I do cheer up quite quickly if staff are positive with me. I enjoy doing my exercises and helping out with household chores but I do need encouragement to get moving and sometimes a bit of support with the hoover. Please read my support plans! They are very detailed and will help support me how I like to be supported and the safest way.

I am softly spoken and easy to talk to Getting my paper on a Thursday Going to Bradford on Avon with Staff and showing them around where I used to live Going on trains Cup of tea and a biscuit or cake in the evening Chocolate! Being included with day to day things Being included in conversations Baking Cakes Going out for a meal Going to TMC (day service) Doing puzzles I am very independent and just need prompting to do most things, I like to be given plenty of time to get ready for things and do not like to be rushed. I enjoy one to one time with staff and sometimes ask staff to do things that I can do for myself and just need a gentle reminder that I can do it and you will help if I struggle. I enjoy spending time with my house mate DST but I can sometimes feel a little left out as he needs more support than I do this can make me upset so please try to include me. I am very quiet until I get to know you but when I feel comfortable I am quite chatty. Please read my support plans! I am quick witted What makes me happy I enjoy banter with people I enjoy seeing my sister on a Monday and spending time with her. I enjoy a beer now and then I enjoy going to the pub for food and to watch football or rugby I like to make my own choices I enjoy going to Riverbank and hydrotherapy A wish for the future... To go to the pub more often with support How I like to be supported I need a lot of time to make my choices and sometimes when I am tired I am very slow to answer. I am shy around new staff and sometimes may not answer them. I get tired if I have had a busy day or not slept well at night. I like staff to acknowledge me when they come in the room I am in and say hello to me. I need quite a lot of support but I like staff to take their time and help me be as independent as possible. I like to be involved. Please read my support plans! They are very detailed and will help support me how I like to be supported and in the safest way.

4 M.G People say I am Bubbly and have a great sense of humour! I am very sociable I love to banter with staff and people I know. I enjoy my food especially steak! I love singing and I like people to sing with me. I love my family and enjoy meeting with them in town. I can cheer people up with my smile and my laugh! Singing with people! Looking through magazines and books I enjoy a bath and shower and often splash staff! Meeting with family and friends! I love Jewellery and often ask to wear staff s watches! I enjoy gardening and sitting in the sun I love going to my day centre! I like to buy Balloon s regularly I really like clothes and choosing what to wear each day! I like to look good! I like to be talked through the process of personal care and be involved. I can communicate very well and let you know what I would l I like staff to say hello to me when they walk into the room I am sat in and I like to be involved in conversations. I do sometimes have a difficult relationship with DR and I need staff to help me manage this. I can become very tired and I need to be encouraged to have bedrest. I also need to be encouraged to have an early night if I do decline bedrest as I can become over tired and decline to go to bed and get quite upset with staff over this. 5 S.J I am quite a chatty person and like to have a giggle with others I love sunbathing and go a lovely colour when I ve been out in the garden I am very good at completing puzzles and can do the harder ones with staff help! I love my coffee and I like to help make one with support I am very independent I enjoy coffee and like to have several as a part of my daily routine I enjoy going to TMC (day service) I love going to the beach and love sunny days! I like to be involved in all my decisions I like a lot of reassurance from staff about who is going to be on shift and what is planned for the day I like my sleep I like to look nice and wear lovely clothes that I have chosen. I like to be given lots of choice and plenty of time to make my decisions. I enjoy lots of coffee but I do need to be reminded to drink other drinks also. I enjoy my sleep and can become very tire and need encouragement to go to bed at a reasonable time during the week but I can stay up late on the weekends. I can sometimes get upset or annoyed with people if I feel they are talking without involving me and I need support with managing this. I enjoy being independent but I can sometimes need support with transferring if I am tired. If I ask you if you would like a coffee this means I would like a coffee and I cannot make this by myself.
