2 Student 7-9 Survey of Internet and At-Risk Behaviors 1) 7-9th GRADE ASSENT TO PARTICIPATE IN THE RIT SURVEY OF INTERNET AND AT-RISK BEHAVIORS You are being asked to take part in a research study to help teachers learn more about how students use computers. Researchers want to ask you some questions about how you use computers, other types of electronic devices and the Internet. You will not directly benefit from answering these questions, and it will take you about ten minutes to answer all the questions. Some of the questions may cause you to think about things that make you feel uncomfortable. Your parents have been notified about this research and they have not objected to you participating or answering questions. But your participation is completely voluntary. You can decide not to answer any questions. You can also change your mind and to stop answering questions at any time for any reason. And no one will be upset if you do not want to answer any questions or if you decide to stop answering questions. Your answers will remain complete confidential. No one will know your answers to any questions. And nothing you say will be identified with your family or friends. You can ask your teacher any questions that you may have about the study. You may also ask your teacher about any questions that you do not understand while you are answering them. If you choose to stop answering questions at any time during the survey, just raise your hand and let your teacher or administrator know. Do you want to participate in the survey?
