2 Welcome to AbbVie CareTM We re here for you, every step of the way Starting a medication like HUMIRA can seem daunting at first, but the AbbVie Care Support Program is here to help. By enrolling with AbbVie Care, you can choose from a wide range of services that are tailored just for you. These services will be there for you every step of the way; not just at the start, but for as long as you need them. The program has been designed around our three principles of support that capture the essence of AbbVie Care:

4 to helping you get started and supporting you with personalized services along the way At AbbVie Care, our support begins before you have your first dose of HUMIRA. You may be wondering how and when you will start your new treatment. But for now, we want to get to know you and what kind of support program you want. To get started, your Personal Care Specialist will give you a call and help you choose some initial services that you think will be a good fit for you. Don t worry; you can always change your mind later. Take a few minutes to read through the list of services on the next page, and think about which options sound right for you. Your Personal Care Specialist will be happy to explain all the details and answer any questions you have about your options. PERSONAL CARE SPECIALIST Name: Hello Get to know your AbbVie Care team Personal Care Specialist Your main point of contact is responsible for handling all your needs associated with your key AbbVie Care services at the start of your treatment journey. Doctors have a support specialist of their own who works with your Personal Care Specialist to help coordinate your enrollment and program support during the initial stage of your HUMIRA treatment. 4
5 Choose the AbbVie Care services you want Reimbursement services help you access your medication Before you have your first dose of HUMIRA, we consider your financial needs, explore all your options for reimbursement and handle the paperwork for you Pharmacy services let you choose how you get your HUMIRA Order your HUMIRA by phone, and pick it up at your local pharmacy Arrange for home delivery, and your medication comes straight to your front door Call to re-order your HUMIRA, so you have it in time for your next dose Injection services help you take your HUMIRA Visit an injection clinic or a specialty pharmacy and a nurse will administer your HUMIRA Opt for one-on-one injection training with a nurse at a location of your choice Sign up for reminder refill calls, where available Educational services offer you the chance to learn as much about your condition and treatment as you want Receive an injection kit and ongoing materials Learn how to take HUMIRA as your doctor has prescribed Discover tips to help you stay on track with your treatment Find out about travelling with HUMIRA and storing your medication properly Get advice on your condition and how to manage your treatment Choose your services today.
