1 AI and Healthcare in Boston IA Boston, November 2017 Walter de Wit, Christian Cohen, Tyrone Pater, Isabel Barnhoorn, Tonatiuh Belderbos Introduction Over the past few years Artificial Intelligence (AI) has exploded; it s hard to think of a single technology that will shape our world more in the next 5 years than artificial intelligence. Proponents strongly believe that AI is able to help solve complex social challenges that our current and future society are facing. One of the industries that is betting big on AI is healthcare. The combination of data and AI has the potential to improve outcomes and reduce costs within healthcare by applying machine learning algorithms and predictive analytics to reduce drug discovery times, providing virtual assistance to patients, and diagnosing ailments by processing medical images. Healthcare entrepreneurs are focused on the win-win-win prospect of lowering the cost of care while making it better and available to more people. In 2014, worldwide revenues from healthcare-related AI products were an estimated $630 million. It is expected that with an annual growth of 40 percent the market will expand to $6.6 billion by A noteworthy growth pace, and with an expected global healthcare spending of $8.7 trillion in 2020 there is still a huge market potential. i Boston is because of its many top ranked universities and research institutes, active startup scene, venture capital availability and major players like IBM, Google, Boston Dynamics and GE, well-equipped to be a strategic hub for the new wave of Artificial Intelligence. In combination with Boston s leading position in the healthcare and life sciences industry, Boston seems ready to be the frontrunner of AI applications in healthcare. AI applications in healthcare ii Electronic Health Record (EHR) systems helped digitizing health records within most major hospitals and practices today. Additionally, data is gathered through mobile health apps and wearables. AI can help to use the enormous amount of data to improve diagnoses and to reduce drug discovery time. iii In the near future, AI in healthcare will likely have the most impact in data management and in the area of image analysis. The expectation is that a general purpose AI, an AI program that would be able to talk to patients, assesses their symptoms and comes up with a full diagnosis and treatment, is at least a decade away. iv Data management

2 The number one application of AI in healthcare is data management. AI helps in collecting the data, storing it, normalizing it and tracing its lineage. Google s Deepmind Health project is mining data records to improve the quality and the speed of health services. Image interpretation AI is especially likely to make a difference in any job that involves image analysis, for example in the areas of radiology and dermatology. In these fields, a doctor s job related to imaging is expected to radically change: instead of reading images all day, doctors are predicted to exclusively focus on cases that are hard to assess. For example, Medical Sieve, launched by IBM, is able to detect problems in radiology images faster and more reliable than humans. Figure 1 Heatmap of AI venture capital deals in Healthcare Assisting in treatment plans An AI algorithm can analyze medical records and can detect negative effects of a planned treatment. AI algorithms are also able to detect certain patterns in the clinical data which indicate a significant change in the condition of the patient. For example, AI can spot patterns in data sets of genetic information and medical record, resulting in early diagnosing cancer and vascular diseases. IBM s Watson for Oncology is able to analyze the patient s medical record and to combine it with clinical expertise, external research and data from reports, to design treatment plans for a patient. Another example is Babylon Health (BH), an app launched

3 by Babylon, which is able to make a first diagnosis and to suggest the right course of action based on common medical knowledge and symptoms reported by the users of the app. In addition, BH reminds users to take their medication and follows up to see how they are feeling. Precision medicine Artificial intelligence will have an impact on genetics and genomics as well, and therefore on precision medicine, which takes into account variability in genes for each person. Deep Genomics, based in Toronto, aims at identifying patterns in huge data sets of genetic information and medical records, looking for mutations and linkages to disease. Deep Genomics is inventing a new generation of computational technologies that can inform doctors what will happen within a cell when DNA is altered by genetic variation, whether natural or therapeutic. Drug development In addition to using all the knowledge available, AI systems have the ability to learn and see patterns humans won t detect. This could be helpful in drug creation. In healthcare drugs are created through clinical trials, which costs billions of dollars and can take up to a decade. AI can develop new pharmaceuticals by going through enormous sets of data. For example, Boston-based company Berg Health analyzes datasets to find out why some patients survive diseases, while others don t. Customer care Virtual assistants can significantly reduce costs by replacing humans in customer care. An intelligent virtual assistant is 24/7 available and has the benefits of speed and selfservice. Improvements in natural language processing are vital since humans explain their issues in their own spoken or typed words. v Boston ecosystem of AI and Healthcare This section provides an overview of the most relevant universities, research programs, hospitals, companies and startups operating in the greater Boston area. Finally, it also provides a list of the most important AI and Healthcare events that will be organized in 2017 and Universities and research programs MIT-IBM Watson AI Lab In September 2017 IBM and MIT announced that IBM is investing $240 million to create an AI lab in partnership with MIT. This new lab will be mobilizing over 100 AI scientists, professors and students to carry out joint research at IBM s research lab in Cambridge. Researchers at the lab will conduct research in fundamental AI which includes the research areas of AI algorithms, AI hardware and software. It also aims to increase the impact of AI on industries such as healthcare and cybersecurity. The MIT-IBM AI lab is a new addition to a multiyear collaboration between MIT and IBM. In 2016 IBM announced a long-term collaboration with MIT s Department of Brain and Cognitive Sciences to make progress in the field of machine vision. Moreover, IBM invested $50 million in a five-year research collaboration on AI and genomics with the Broad Institute of MIT and Harvard. vi

4 MIT CSAIL - Clinical Decision Making Group The Clinical Decision Making Group at the MIT Laboratory for Computer Science is a research group dedicated to exploring and furthering the application of technology and artificial intelligence to clinical situations. Because of the vital and crucial nature of medical practice, and the need for accurate and timely information to support clinical decisions, the group also focuses on the gathering, availability, security and use of medical information throughout the human "life cycle" and beyond. Harvard University The mission of the Biomedical Informatics department of the Harvard Medical School is to develop methods, tools and infrastructure required for a new generation of biomedical investigators and care providers to move biomedicine rapidly forward by taking full advantage of the insight and precision offered by the enormous data resources in healthcare. vii Moreover, the Harvard-MIT Health Sciences and Technology (HST) program brings together MIT, the Harvard Medical School, Harvard University and the Boston area teaching hospitals in a collaboration that integrates science, medicine, and engineering to solve problems in human health. Part of the HST is the Medical Engineering and Medical Physics (MEMP) program, which is a five to seven-year program that prepares students to be medical innovators. viii Northeastern University NEU offers the Master of Science programs Health Informatics and Health Data Analytics, two interdisciplinary programs taught by the College of Computer and Information Science (CCIS) and the Bouvé College of Health Sciences (BCHS). ix Additionally, NEU offers a PHD-program in Personal Health Informatics. The program the first of its kind in the nation prepares students from both the health and computer sciences fields to lead research and development of new technologies to transform healthcare delivery around the globe. Worcester Polytechnic Institute HDI The Healthcare Delivery Institute (HDI) at the WPI is a university-wide multidisciplinary research and innovation institute to drive collaborative research, innovation, and education. HDI focuses on applying digital technologies, IoT, data intelligence and machine learning x with the objective to impact healthcare delivery and consumer/patient health. HDI leverages WPI's interdisciplinary expertise to address healthcare challenges. Hospitals Beth Israel Deaconess Medical Center The Beth Israel Deaconess Medical Center (BIMC) is one of Harvard s teaching hospitals. One of BIMC s research projects is Medical Artificial Intelligence. BIMC is currently working on an intelligent sepsis monitoring system to continuously identify and risk stratify patients at increased risk of sepsis. Together with machine learning experts at the Courant Institute of Mathematical Sciences they are developing machine learning theory to integrate all available demographic, laboratory, radiographic, and continuous hemodynamic data. These algorithms are used to derive next generation clinical alerts, contextual information retrieval, and targeted decision support. MGH & BWH Center for Clinical Data Science

5 The MGH & BWH Center for Clinical Data Science is a collaboration between Mass General and Brigham and Women's Hospital to create, promote and commercialize artificial intelligence for healthcare. By using AI techniques such as machine learning and artificial neural networks, the center is building systems to improve the detection, diagnosis, treatment and management of diseases. One of the founding technology partners of the Clinical Data Science Center is Nvidia, a computer technology company that has pioneered GPU-accelerated computing. xi Boston Children s Hospital In November 2016, GE Healthcare and Boston Children s Hospital announced a joint venture to combine the expertise of the clinicians at the Hospital and the AI technology of GE. They aim to produce software that will help doctors to more accurately interpret brain scans of young patients. Their goal is to develop the system in 18 months and then market it to hospitals around the world. xii Partners Healthcare & General Electric In May 2017, GE and Partners Healthcare launched an initiative to employ artificial intelligence to improve medical care. The 10-yearlong effort includes clinical and technology experts at the Partners-owned Massachusetts General and Brigham and Women s hospitals working alongside engineers and developers at GE. The partnership is initially focused on developing deep learning algorithms that analyze medical images to help identify patients that need follow-up or intervention by doctors. Over time, they also want to create applications for genomics, population health, and other areas of medicine. Executives from GE, one of the nation s largest corporations, and Partners Healthcare, Massachusetts biggest nonprofit hospital network, argue that such technology has the potential to help care providers do their jobs more efficiently so that patients receive more accurate diagnoses and better treatments. Without disclosing specifics, both companies said they will spend a significant amount on the initiative. Additionally, both stand to gain revenue if they are successful in creating useful software programs that can be sold to hospitals around the globe. xiii Companies and startups IBM IBM is an early leader with its Cambridge-based Watson Health business. xiv IBM spent over $4 billion in the past two years in acquiring healthcare-computing companies and building out its AI capabilities. IBM acquired AI-healthcare companies Ann Harbor, Merge Healthcare, Phytel and Explores in the last two years. IBM claims that its Watson software is used by a dozen of the largest life sciences companies and that so far more than 55 hospitals and healthcare organizations implemented its oncology tools. Watson says that it can quickly sift through sets of data to perform drug research and to make diagnoses. For example, Watson software analyzed 1,000 cancer patient cases and suggested potential treatments that doctors weren t able to identify in about 30 percent of the cases. Watson for Oncology assists physicians by quickly identifying key information in medical records from patients and generating a list of potential treatments based on their medical records. xv As mentioned, IBM announced that it will spend $240 million to open an AI-lab in collaboration with MIT. One of the focus areas of the lab will be healthcare. xvi

6 Barrow Neurological Institute used Watson technology to help identify five genes associated with ALS that had not previously been linked to the disease. Moreover, a pilot study with Watson technology helped shortening the time needed to screen patients for clinical trial eligibility from 1 hour and 50 minutes to 24 minutes. xvii General Electric As stated above, GE and Partners Healthcare launched an initiative to employ artificial intelligence to improve medical care. GE Healthcare is a leading seller of medical devices, such as X-ray machines, patient monitoring equipment, and ultrasound systems. The fact that GE s machines are already widely used by hospitals worldwide gives it an advantage over competitors in healthcare AI software that don t sell medical devices. Philips Sixty percent of Philips R&D is dedicated to software, which for a large part includes AI as well. Philips expects that in the future most of their products are supported by AI. Today, Philips is using advanced big data analytics to bring genomics data together with information on patients family history, lab results, medical scans, other diseases and results from previous treatments. They are supporting doctors to set up precision medicine programs in local and regional community hospitals, outside of the context of the large academic centers. xviii Philips Research in North America aims to create AI technologies to improve healthcare decision making and interventions across the care continuum. Their research activities cover a wide range of areas in AI and Healthcare, including Clinical Question Answering and Precision Medicine. Nvidia Nvidia, a computer technology company with an office in Westford, is building a common platform for AI across healthcare. Nvidia aims to facilitate the transformation of the healthcare industry by providing an end-to-end AI computing platform designed for the healthcare community. Their GPU-accelerated solutions aim to foster collaboration, while at the same time keeping each institution s information secure. xix Luminoso The MIT Media Lab spin-out Luminoso partnered with the Centers for Disease Control (CDC) to launch a far-reaching project to integrate data to more accurately predict trends in the spread of the flu. The overall goal is to prove the value of unstructured data in understanding the spread and severity of a wide range of health challenges. With the help from Luminoso, CDC improved their prediction model. During the Ebola outbreak the CDC used the help of Luminoso to identify misinformation on social media and publicly address it. xx Startups Boston has a rich ecosystem of AI startups, focusing on a wide range of applications of AI in healthcare. An example is Welltok, located in Burlington, MA, and with total received funding of $191 million it is one of the five most well-funded AI companies in Healthcare. xxi Its mission is to empower consumers to achieve and sustain their optimal health. Their CaféWell Health Optimization Platform connects consumers with available and relevant resources, benefits and rewards using a data-driven, personalized approach. Additionally, their technology-enabled services leverage both advanced

7 analytics and multi-channel communications to identify, target and reach consumers. Below some other examples of AI and healthcare startups in the Boston area. Startup PathAI Interactions OM1 Jibo Short description The Cambridge-based PathAI developed a platform that provides automated and data-driven pathology analysis. Interactions provides virtual assistant software that combines speech recognition and human intelligence. Interactions is based in Frankling, MA and raised $167 million. OM1, located in Cambridge, uses machine learning algorithms and healthcare data to create better standards for procedures and treatment. Jibo is an example of a robot companion that serves as a social partner in order to alleviate loneliness and treat mental health issues. Jibo is Boston-based and received over $70 million in funding. Legionarius NeuroSleeve Bergh Health Boston-based Legionarius developed a wearable system integrated into shirts and pants that stops the bleeding from gunshot, stab, and blast wounds, and provides remote triage capabilities. Within seconds, it applies pressure to the wound to stop bleeding. Autonomously and automatically. NeuroSleeve developed a low-cost and automated nerve conduction sleeve for the diagnosis of Carpal Tunnel Syndrome. Berg Health developed a platform that combines patient biology and artificial intelligence-based analytics to engage the differences between healthy and disease environments. The patient s own biology drives the platform s results and guides them in the discovery and development of drugs, diagnostics and healthcare applications. Venture Capital In 2016, venture capitalists in the US invested in 658 AI-related startups, with a total invested amount of $5 billion. This is an increase of 61 percent compared to The growing interest of venture capital firms in artificial intelligence is reflected in Boston. Boston-based VC firm Glasswing emerged to exclusively invest in AI-related firms. In June 2016, it announced that it targets to raise $150 million to invest in AI startups. xxii Other AI investors based in the Boston area include Hyperplane investors, Procyon Ventures and GE Ventures. xxiii Events Upcoming AI and Healthcare events in the Boston region: - Healthcare + A.I. - November 2, 2017 Xconomy is gathering top leaders from New England and beyond to discuss strategies that companies are using to bring A.I. to healthcare, the business challenges around those efforts, and how all of this could impact patients, doctors, and the healthcare industry overall.
