Digital Health, Past, Present and Future In the beginning Paper! c. 1960Telemedicine the use of telecommunication and information technologies to provide clinical health care at a distance (year of my birth!) One size fits all solutions Professional associations; International Medical Informatics Association, the American Telemedicine Association, and the UK Telecare Services Association WebMD Medline Plus
Digital Health, Past, Present and Future
Digital Health, Past, Present and Future This is $h!t
Digital Health, Past, Present and Future
Digital Health, Past, Present and Future Let s think bigger! National Program for IT (NPfIT) 2002 Led by Tony Blair Budget 6billion 2011 Scrapped by Coalition Government Estimated costs 20 Billion+ What went wrong?
Digital Health, Past, Present and Future 1) Top Down Management Managers know best No, they don t! What managers say goes. I don t think so mate! 2) Lack of User Engagement Users know what they want No workflow mapping 3) Poor Implementation No what users want Sometimes prevents users from doing their job Example, Cerner Millenium Initially free Final cost 7 million
Digital Health, Past, Present and Future New solution Personalised health and social care for all 2020 find your own solutions, and God speed All systems procured must: Meet technical standards for inter-operability (systems must be able to exchange data with other systems) Meet professional standards for the recording of patient data i.e. adoption of the Academy of Royal Medical Colleges standards for the clinical structure of patient records and a single clinical terminology (SNOWMED CT) Technical systems and applications will be kite marked The progress of agencies will be assessed in CQC inspections
Digital Health, Past, Present and Future New solution March 2015 all citizens to have access to GP records: April 2015 coordination of national and professional data standards to facilitate data sharing: 2018 all clinicians in primary, urgent and emergency care will operate without the use of paper records: Jeremy Hunt we will have a paperless NHS Simon Stevens we will have a mostly paperless NHS 2020 all care records will be digital, real time and interoperable: 2020 the entire health service will have adopted the SNOWMED clinical terminology.
Digital Health, Past, Present and Future My thoughts Patients access to the whole record Wearable tech supporting; Self management Management by clinicians More clinical input Expert knowledge of technology in house Solutions designed by clinicians for clinicians More clinical UX Designers
